2. Hand-foot syndrome (HFS or Palmar-Plantar Erythrodysesthesia) is one of the
side effects of oral chemotherapy (capecitabin (Xeloda®).
It is characterised by palmar-plantar erythema, which may progress to scaling and
ulceration, sometimes painful and with functional repercussions.
These skin lesions are classified according to 3 grades.
The treatment of HFS is not subject to any particular protocol and remains based on
local treatments (such as moisturising creams) and/or adjustment of the chemotherapy
dosage.
The cream emulsion* (hyperoxygenated essential fatty acid glycerides) moisturises
and repairs the skin.

3. OBJECTIVE
To assess the efficacy of the cream emulsion* in HFS (Hand-Foot Syndrome)
in breast cancer patients.

4. METHODOLOGY
As part of CISCO nursing
consultations at the Outpatient
Chemotherapy Clinic (Institut Curie,
Paris):
• Inclusion of patients treated with
capecitabin and presenting grade 1 or
2 HFS (Hand-Foot Syndrome)
• Initiation of the cream emulsion*
treatment at an initial visit (D0)
• Cream emulsion* treatment duration
of 6 to 9 weeks
• A clinical, photographic and quality-
of-life assessment (EQ-5D descriptive
scale) was documented at each visit.
D0: Presence of erythema on the
inner surface of the hand
D56: Disappearance of erythema

5. RESULTS
25 women with breast cancer treated with
capecitabin:
• Hands: mostly grade 1 HFS (Hand-Foot
Syndrome)
• Feet: mostly grade 2 HFS
Average age: 64 years (range: 48 - 86 years)
Average follow-up period = 49.5 days (median
43 days).
Three patients were not seen again for the
final visit.
The cream emulsion* was applied 2 to 4
times daily in the majority of cases.

6. All the symptoms of HFS (Hand-Foot Syndrome) demonstrated an improvement
following application of this cream, particularly signs such as hyperkeratosis, skin
cracks and erythema.
RESULTS

7. RESULTS
This reduction in chemotherapy-related side effects is accompanied by an improvement
in Quality of Life, particularly in terms of pain:

8. The patients noted an overall improvement in their quality of life between the initial visit
and the final visit.
Improvement of 10 points compared to median VAS** score. At the end of follow-up,
almost 60% of the patients considered that their HFS symptoms had improved from the
second week of application (18.2% of whom after less than a week).
No worsening of HFS was reported. No local adverse effects related to the cream were
observed during follow-up

RESULTS

9. CONCLUSION
The cream emulsion*, combined with regular nursing and medical follow-up and
patient education, offers genuine benefits for the local management of Hand-foot
syndrome and appears to improve patients' quality of life.
Applied to the hands and feet, the cream emulsion* corrects dry skin and promotes
the healing of superficial trophic disorders of the skin.
