2. ABOUT THE COMPANY
Innovative Concepts, Inc. was incorporated on March 26, 2001, by
Ms. Casey C. Simmons, GPC. The company provides Grant Consulting Services to
Healthcare and Non-Profit Organizations. Depending on the client’s needs these
Grant Consulting Services may consist of Writing, Administration, Research, or a
combination. We specialize in projects that seek Public Funding (Federal, State,
Local), particularly for construction and/or rehabilitation in an area of the Community
with great need.
Innovative Concepts, Inc. was located in the State of Florida until 2008 when we
moved to New York State. Our offices are located in the Flatiron/Chelsea area of New
York, NY. We continue to provide Grant Consulting Services to our Florida Clients.
Since 2001 Innovative Concepts, Inc. has directly assisted Local Communities and
healthcare and non-profit organizations with the Pre-Award and Post-Award (i.e.
application preparation, submission, administration, and close-out) process. On
occasion only Pre-Award (writing) or Post-Award (administration) services were
provided.

Broward Health, Ft. Lauderdale, Florida
The non-profit community health system in Broward County, Florida, has more
than 30 facilities. Writing, proofreading/major edits, and collaboration of project
ideas are provided on an as needed basis. Examples of recent projects
include; HRSA Patient Navigator Program for Cancer/Chronic Diseases and
HRSA Women of Color with HIV/AIDS, Robert Wood Johnson Foundation’s
Local Initiative Funding Partners ‘Kinship Cares’ Program and RWJF Aligning
Forces for Quality, HHS Healthcare Facilities Emergency Partnership Project,
and Ryan White Electronic Records System Enhancements.
Central Harlem Senior Citizens Centers, New York, NY
Innovative Concepts, Inc. works side by side with the Executive Director to
prepare and writes grants for the Senior Citizen Center located in the Harlem
neighborhood of Manhattan. Currently, focus is being placed on grants to fund
the development and implementation of a Volunteer Program.
Clark Communities, Inc., Tallahassee and Ft. Lauderdale, FL
CCI has multiple projects at any given time. CCI’s clients are Small Cities and
Counties, as well as one Entitlement City, throughout Florida. Grant writing and
administration assistance with Community/Economic Development,
Neighborhood Revitalization, Disaster Recovery Projects, and Rural
Development is provided to CCI.

5. Innovative Concepts, Inc. also works with CCI’s sister company- CRA
Community Development Group, Inc., a Non-Profit Organization planning
Affordable Housing developments. Innovative Concepts, Inc. helped with
organizational development and filing of its 501(c)(3) status with the Internal
Revenue Service.
Police Athletic League of New Jersey, Freehold, NJ
Innovative worked as a sub-contractor for grant management services of a U.S.
Department of Justice Office of Justice Programs grant for the organization’s
Afterschool Initiative for at risk youth so the project stayed in compliance and
met all objectives. Grant writing and management services are being provided
for a second round of funding, from the same agency, to further enhance and
expand the Afterschool Initiatives.

available to purchase an Assisted Living Facility and make necessary Capital

programs including HIV/AIDS education and prevention and tutoring/mentoring to
young minority women.
Dress For Success, New York, New York, NY.
Proposal and program review conducted for the Not-For-Profit organization that
provides interview attire and skills; as well as a referral network for employment
retention.

6. Guardian Care Nursing and Rehabilitation, Miami Shores, FL.
A Research and Feasibility Study was drafter for the Skilled Nursing Home. This
document detailed funding options for Diversity in Nursing Education scholarships.
Grant writing was also provided.
HospiceCare of Southeast Florida, Inc., Ft. Lauderdale, Florida
Innovative has assisted HospiceCare in seeking funding for many different
projects. Examples are: Hazard Mitigation, Bereavement and Educational
Outreach, Healthcare Technology, General Programs, and the planning phase
of securing funding for the Construction/Rehabilitation of a Free Standing
Facility. Innovative continues to work with the non-profit Hospice and Palliative
Care organization on an as needed basis.
Miami Jewish Home and Hospital, Inc., Miami, Florida
Research and grant writing services for general support funding and specific
projects were provided. The organization has a nursing home, hospital,
Alzheimer’s unit, and several residences.

7. ABOUT THE PRESIDENT
Casey C. Simmons, GPC, serves as the President of Innovative Concepts, Inc. The
Florida Native holds a Bachelor of Business Administration with a concentration in
Management from Stetson University, as well as a Master of Science in Health
Services Administration from Barry University. Ms. Simmons is also possesses the
Grant Professional Certified (GPC) credentials from the Grant Professionals
Certification Institute (GPCI).
She gained vast experience in the areas of grant writing and administration by
working for CRA Consulting, Inc. (now Clark Communities, Inc.). Ms. Simmons
worked directly with the President and other Senior Staff on Grants provided to CCI’s
clients from the Florida Department of Community Affairs Small Cities Community
Development Block Grant (CDBG) Program, Housing and Urban Development
Entitlement CDBG, Department of Environmental Protection, and the U.S.D.A. Rural
Development Administration.
Ms. Simmons also served as the Project Coordinator for the Florida Partnership for
End-of-Life Care, one of 23 states in the country who participated in the Community
State Partnerships for End-of-Life Care that were awarded a total of $11.25 million.
There she was the coordinator for five “partnering” Florida organizations, agencies,
and universities, as well as 19 community coalitions, and was responsible for the
disbursement of funds, monitoring of existing funds, and general program compliance,
as well as serving as a representative nationwide.
While at Stetson University Ms. Simmons had an Internship with the non-profit
organization Home And Neighborhood Development (HAND). Serving as the
Administrative Intern for HAND she gained experience working with low and moderate
income citizens of Volusia County, Florida. With the Executive Director, she sought to
aid these citizens with down payment assistance, coordinated with the Fanny Mae
Foundation, for the construction and/or rehabilitation of housing, and assistance with
HUD Section 8 minimum housing standards.

12. APPROACH TO SERVICES PROVIDED
Innovative Concepts, Inc. will work with the client to determine what service(s) is desired
and appropriate for the organization’s specific needs. The below provides descriptions for
each service.
Research
Meetings with organization and other parties, if applicable, to gain information.
Conduct extensive search for all available funding opportunities that are most
feasible for all programs identified.
Determine where findings fit among Strategic Plan and/or Fund Development
Plan.
Create Fund Development Calendar to be incorporated with Strategic Plan or
as a standalone document. This calendar details all identified grants, their
deadlines, and expected time period for receiving funding awards.
Draft comprehensive summary of all funding opportunities including: (1)
Name and contact information, (2) Deadline, (3) History of awards and average
number of awards provided each year, (4) Restrictions and eligibility criteria, (5)
Appropriate first approach, and (6) application.
Present study in electronic and hard copy format.
Grant Writing- Preliminary Services
Work with client to compile components for needs assessment.
Obtain data on specific population groups (i.e. demographics, income status,
health needs, etc.). This could include conducting household surveys or
researching census data, statistics, evidence based best practices, etc.

13. Attend public meetings, as needed, and assist with advertising public notices to
meet Rules/Regulations.
Meet with other parties to develop budget and scope of the project, as well as
determining fit among Strategic Plan.
Attend application workshops and/or other sessions providing pertinent
information about the grant submittal.
Review Notice of Funding Availability (NOFA) and/or Request For
Proposals/Application (RFP/RFA). Outline all necessary pieces of the
application such as: Narrative, Needs Assessment, Approach, Goals and
Objectives, Logic Model, Evaluation, Timeline, and Budget and Budget
Narrative.
Grant Writing
Participate in all meetings- either in person or via conference call/video.
Write grant application according to Request For Proposal, Notice of Funding
Availability, Rules/Regulations, and other information provided.
Assist the client with securing leverage/matching funds, if applicable, to
increase the value of the project and provide a stronger competitive position for
award.
Obtain all required attachments.
Provide several drafts for review by the client.
Grant Management
Take all measures necessary to ensure that the grant project/program complies
with Foundation, State/Local, or Federal grant guidelines as stated in the award
agreement. This could include the following:

14. Prepare Environmental Review Record in conjunction with the Project Engineer
(if applicable) and submit for Release Of Federal Funding, or other approval.
Prepare and submit quarterly & annual reports as required.
Monitor the disbursement of grant funds.
Prepare for, and acting as the organization’s representative, at Monitoring/Site
Visits by the grant agency or foundation.
Analyze progress of the project to ensure that set milestones/objectives are
being met in a timely manner.
Prepare close-out report that details all met goals and objectives and
received/disbursed funds.
Provide Project Status Reports on a monthly basis, or as directed by the client,
to the organization so that they are kept abreast of efforts being made.
Assist the organization/client in reviewing their Strategic Plan, Comprehensive
Plan, Consolidated Plan, and others necessary for grant application submittal.
Innovative will assist in preparing these documents, if necessary.
Project/Program Evaluation
Conduct Full Independent Outside Evaluation of grant project/program.
Elements to be evaluated are: Qualitative and quantitative goals and
objectives; Disbursement of funds; Timeliness work.
Review of above elements to be conducted once per quarter via “desk
monitoring.” In addition, at least one “on-site monitoring” will be made.

Write Evaluation section of the grant proposal/application, if requested.
