Overview
Hesley Village is situated in open countryside
within a large 100 acre site about two miles from
the small South Yorkshire villages of Tickhill and
Rossington. The service provides high standards
of accommodation, personal care and support for
younger adults with learning disabilities who often
(but not always) also have an autistic spectrum
disorder. We may occasionally provide services to
young adults aged 16 or 17 who are in transition
to adults services though generally we support
adults aged 18 and above.
There are onsite facilities to support people with
occupational and employment opportunities and
transport to support people to access services
and facilities locally. Hesley Village operates a
residential model. Most people have single person
accommodation although a small number of
houses are shared between a maximum of three
people.
We place a strong emphasis on supporting people
to develop the skills which will allow them, where
possible to lead more independent lives, to achieve
social inclusion and to be involved in meaningful
activity of their own choice. We aim to support
people to reach their full potential by providing:
•	High standards of individualised support
through the development of person centred
care pathways. We aim to ensure this is delivered
at all times in ways that are aimed at achieving
positive outcomes for people and that are fully
compliant with Essential Standards of Quality and
Safety (CQC 2010) and associated regulations
through regular monitoring, review and
consultation.
•	Options for people to develop their life skills and
access vocational and employment opportunities
in-house and in the wider community.
•	Clinical support and consultancy when it is
needed, to help people develop coping skills and
also to train our staff to intervene and support
effectively and help us provide an enabling
environment. Hesley Group’s clinical services
team includes speech and language therapists,
applied behaviour analysts, clinical psychologists,
communication co-ordinators and occupational
therapy services.
•	Preparation and support for people and for their
families when moving onto the next stage in
their lives by effective transition planning.
•	Support for people to achieve and maintain
the best possible health and access healthcare
services as needed.
•	A safe but least restrictive environment for people
through positive risk management, monitoring of
outcomes and regular review of risk assessment
and support plans.
Safeguarding
Hesley Group offers a person centred approach which
aims to support people to develop increasing levels of
independence and to create a safe environment for
them, in which and from which they can have the best
possible opportunities to maximise their potential. Our
philosophy is one of warmth, security, consistency,
understanding and trust. Our safeguarding
arrangements sit in this context, our policies,
procedures, staff training and quality assurance
provide a robust and regularly reviewed framework to
this philosophy.
Health and Safety
We are committed to ensuring the health, safety and
welfare of all those who live and work in, or otherwise
access our services. Hesley Village adheres to all
procedures in place under the Hesley Group’s Health
and Safety Policy.
The people we support all have complex needs, often
including behaviours that can challenge them and the
services that support them.
Our aim is to enable people to fulfil their potential;
to live their lives as positively as possible within an
environment that is appropriate to each person’s
individual needs; and to support people with their
health and personal care needs so they can achieve a
healthy and fulfilling lifestyle.
Through our Single Equality Scheme and Staff Code
of Practice we seek to ensure our services do not
discriminate against people who live in or otherwise
access our services; or the staff who support them,
because of their race, religion, culture, belief,
disability, sex or age.

Person Centred Focus
We plan and deliver services that are built around the person,
their needs, wishes and aspirations. Each person has a range of
plans that reflect their personal, social and health care needs,
therapeutic and behavioural support, communication plans, skills
acquisition and development. These plans are developed with
the person, as well as others important to them, including their
families and professionals who know them well. Plans are regularly
reviewed and updated through multi disciplinary team processes,
keeping the person receiving the support firmly at the centre of
all planning.
Clear policies are in place - such as People’s Rights and Having a
Say, Effective Behaviour Support, Capacity and Consent – that
clearly identify the expectation that we consult with the person
and/or their representatives. Where people cannot give consent
we always aim to deliver outcomes that are agreed to be in the
best interests of the person. At Hesley Village we have a Residents
Council (their chosen name) that meets regularly and acts as a self
advocacy group.
Our aim is to support people to achieve positive outcomes and
plans are outcome based, within an agreed framework of positive
and appropriate risk management.
Because the majority of people we support have complex
communication needs in addition to behaviour that may challenge,
we work to a communication policy and charter and aim to put the
person at the centre of everything we do.
We have a flexible and positively motivated staff team, trained
and skilled in effective person-centred therapeutic support and
communication with those who live at Hesley Village.
All staff attend training in relation to HELP (Hesley Enhancing
Lives Programme), a behaviour support approach based on TCI
(Therapeutic Crisis Intervention), which is BILD (British Institute of
Learning Disabilities) accredited. TCI is an internationally recognised,
crisis prevention and management system that reduces the need to
rely on high-risk interventions. TCI recognises that it is the actions
and reactions of those around people that strongly shape and
influence their behaviour, including their social interactions and
emotional responses. Its emphasis is on empathy and proactive
support, while physical interventions are very much considered a
last resort.
The care and well being of the people we support are paramount
and everything we do is around seeking to ensure that each person’s
best interests are met.
In order to keep people safe there are times when the use of physical
interventions may be needed. This would be as a last resort to
support people in crisis, in order to reduce both stress and risk. To do
this we follow BILD (British Institute of Learning Disabilities) guidance.
The Hesley Group have BILD accreditation for both children’s and
adult services.
Our approach encompasses physical interventions contained within
Therapeutic Crisis Intervention (TCI) strategies and those from
PROACT-SCIP (r) UK.
Any potential physical interventions are fully discussed with the
person, in respect of whom they are proposed and/ or their family
and other appropriate representatives, as far as possible, prior to
their application, which will continue to remain as infrequent as is
possible; and which will continue to be reviewed, with the ongoing
aim of reducing and stopping their use, as soon as this can safely be
done.
Both of these approaches can be explored further via the BILD

Accreditation.
The multidisciplinary therapeutic approach taken at Hesley Village
encompasses a wide range of expertise in addition to that of our
core staff teams and management; and also that available from
externally to the Village. Our own multidisciplinary team includes
the following:
Communication and Speech
and Language
We aim for a total communication environment and to this end
all staff are trained and supported in using augmented systems of
communication, such as signing and picture symbols.
A skilled and experienced team of speech and language therapists
(SALTs) are supported on a day to day basis by communication
coordinators. Our SALTs not only deal with issues relating to
communication; some are also specialists in dealing with the
eating and chewing or swallowing difficulties (dysphagia) that can
present problems for some of those who access our services.
Behaviour Support
People may access applied behaviour analysts (ABAs) on an
individual basis to allow a thorough assessment of their needs
to be made and for the function of the person’s behaviour to be
understood.
ABAs also train and support our staff to provide effective
techniques for helping to manage and lessen or change responses
such as physical aggression or self injury.
All staff are expected to use the person’s HELP Profile, an agreed
common approach, in response to a person’s behaviour and to
report where this is effective or otherwise.
Psychology
Our clinical psychologists work to support the emotional
health and wellbeing of people by helping each person and the
staff supporting them to understand their thoughts, feelings
and behaviours and manage these more effectively. Clinical
psychologists integrate information from the individual, their
family, and the people supporting them, with the results of
structured assessments of neurodevelopment, emotional
attachment patterns, cognitive (thinking) skills, and adaptive skills,
to develop a comprehensive understanding of each person’s needs
and how best to support them.
The resulting formulation is used to develop and implement
interventions which may be used with the individual (eg
non-directive counselling, or adapted cognitive-behavioural
inerventions), or with the staff supporting them (for example:
developing their understanding of how an individual’s needs/
abilities/diagnosis affect their life and how best to support them).
Clinical psychologists work with other members of the
multidisciplinary clinical team to meet the therapeutic needs of
those receiving support in Hesley Group services in a coordinated,
timely and holistic way.
Occupational Therapy
Our Occupational Therapy (OT) service at Hesley Village is key to
supporting active, meaningful lives, for everybody who lives with
us, through the Opportunities Team (see page 8 for more details)
The Occupational Therapy service also provides for individual
assessment, sensory needs and skills development; and the
provision of a safe environment and any equipment needed to
support this.
Psychiatry
We have a consultant psychiatrist, who holds regular reviews at
Hesley Village and advises our service and supports people who
require psychiatric input to remain emotionally well.
For emergency psychiatric support people at Hesley Village can
access local NHS services, with whom we have strong and positive
links.
Therapeutic Support
HesleyGroup|8 Active, Meaningful Lives
Many people come to live at Hesley Village after long
periods, perhaps years, of being unsettled and being
unable to achieve their full potential. For others a move
to Hesley Village comes from more settled circumstances
but still represents a significant transition, particularly in
the context of the multiple and complex needs which are
the experience of all those who live with us.
As a first step we seek to support people through this
transition, to achieve stability, initially within the Hesley
Village community, as a firm foundation for each person’s
future progress. We have many years’ experience of
facilitating and supporting these transitions and achieving
stability. Because people’s needs, wishes, and abilities
vary, we support people in individualised and flexible
ways. Our aim is for them to have the best quality of life
possible.
We place a strong emphasis on people having meaningful
and active lives. We are keen to support activity
away from Hesley Village, as well as within the Village
itself. Some people have currently, or have had, work
placements in nearby Tickhill and Doncaster and also
Sheffield. Others participate in employment nearer to
home, which can vary from helping with the recruitment
and selection process for new staff, to giving support with
the recycling programme.
People are supported to access the full range of activities
and amenities in the local community, as well as at more
distant locations; and also at Hesley Village, where arts
and crafts or horticulture, cooking and other activities are
available on site, as well as a cinema, function room (in
the Village Hall), hairdresser and beautician, grocery shop,
post office and cash machine. The local Doncaster bus has
a stop at Hesley Village.
The whole process of supporting active and meaningful
lives for those who live at Hesley Village, has been greatly
enhanced by the development of the Opportunities Team,
which comprises a specialist Occupational Therapist and
Occupational Therapy Assistant, together with a team
of facilitators, who develop an overall plan of activities,
run sessions (including group sessions and outreach
sessions in people’s own homes); and bring people in from
outside Hesley Village to run time-limited and focussed
courses; from all of which Care Managers can develop
individualised timetables for each person who lives in the
Village.
This service is currently provided through each week; and
sits alongside other individualised, particularly leisure,
activity.
The plan is for the Opportunities Team to extend their
provision, to cover weekends too.
The Opportunities Team link in with the multidisciplinary
(staff and clinical) team, who identify where particular
activities may have therapeutic benefit for a particular
person.
There is a mixture of drop-in activities, for example on
an evening; and regular therapeutic activities delivered
through the day, both by Hesley Village staff, as well as
by external people, including drama, football skills (with
the involvement of Doncaster Rovers) and music, for
example.
The Opportunities Team also run special events on
a monthly basis, such as themed, cultural meals, an
Olympic week in 2012, a Jubilee Celebration, a Christmas
Party and others.
People are supported to enjoy social events and to
make friends. We aim to extend and develop people’s
experiences of the wider community outside of home.
Our hope and aim is for people to develop appropriate
levels of increasing independence and meaningful social
inclusion.
Sometimes it can be understandably difficult for the
parents themselves, of adults with such complex needs,
to smoothly manage the transition to adulthood and all
that this entails. We are sensitive to these issues at Hesley
Village; and seek to understand and value family input
and insights around this process.
Developing Pathways
When people come to live at Hesley Village, we develop
with them, their families and other involved key
parties, a pathway plan that includes desired outcomes
and plans for the future. This may be a planned and
progressive move toward living in a community setting
with the right support, such as stepping down into a less
intensive residential service or into supported living for
example, perhaps within Hesley Group services or the
services of other providers, with whom we seek to work
in a positive and cooperative way. Progress against these
goals is measured on an ongoing basis and annually as
part of the formal review process, keeping the person at
the centre of all planning.
Reviews
Each person has a formal annual review, usually with
their family and the funding authority representative. We
support people to express their views as a key part of this
– where at all possible people will be supported to take
part in their review, including with the involvement of
advocates, where this need is identified.
Additionally a rolling programme of case review is under-
taken including the multidisciplinary team, to make sure
people’s care and support needs are being met and/or
adjusted on a continuing basis, with the aim of ensuring
we fulfil our commitment to each person, of providing
the best possible care and support at all times; and that
this continues to evolve in accordance with the pathway
plan.

individuality through supporting them in relation to their
culture, belief, race, disability, sex or sexual orientation.

Living at Hesley Village
whether in respect of the decoration of their rooms
or other issues relating to their homes or with regard
to their lives in more general terms.
The Village is in close vicinity to the local village of
Tickhill and Doncaster town centre is easily accessed
by the bus route that stops in the car park of the
Village.
There are a range of purpose-built workshops
to accommodate a wide variety of activities in
positive environments. Excellent on-site facilities are
available to complement and enrich the lives of the
people we support including a bistro, cinema, shop,
bank/post office, therapy suite, hairdressers, clothes
shop, bar and village hall.
At Hesley Village, people live in single flats or
three bedroom houses or bungalows. The choice
of accommodation for each person takes into
account their wishes and preferences, their needs,
capabilities, age and social development. Each
person has their own bedroom and bathroom or
ensuite which ensures dignity and respect for the
individual who may need intimate care. Modern
fitted kitchens facilitate learning opportunities
for cooking and spacious dining areas, lounges
and gardens offer warm and welcoming areas to
provide a homely setting.
We aim to empower the person to take greater
control over their own life; wherever possible we
seek to enable people to make their own choices,

Working Together
Coming to Live at Hesley Village
A referral for someone to come and live at Hesley
Village will come through the placing authority
that will be funding the placement. The placing
authority is requested to provide relevant
information which we will review to ensure that
Hesley Village is a suitable provider for the referred
person.
People will have a thorough assessment (for which
there is no charge) prior to potentially being
offered a place within Hesley Village.
The assessment will cover all health and social
needs and all information will be treated
confidentially. Our assessment process will help
to ensure that Hesley Village is able to meet the
person’s needs and is used to inform their initial
personal plan, should they come to our services.
The assessment is carried out by an assessment
team from Hesley Village and includes staff
members from the Clinical and Care Management
teams. They will arrange to meet the person and as
many people involved in their care as possible, to
see how best their needs and wishes can be met.
This is the start of the person-centred planning that
ensures each person is treated as an individual.
If it is felt that Hesley Village is the best option for
the person, the placing authority will be informed
and the transition process begins. This will be
managed by a senior manager at Hesley Village who
will work with the person, family/carers and the
placing authority to ensure the move is as stress-
free as possible.

Staff and Support
Hesley Village has a highly experienced,
comprehensive staff team, comprising managerial
and support staff who work closely with the
multidisciplinary clinical team (described on
page 7). The recruitment process is thorough
and Disclosure Barring Service (DBS) checks are
undertaken in respect of all employees who
have opportunities for unsupervised contact
with vulnerable people, prior to joining the
Hesley Group. Every new member of staff has
a comprehensive induction programme and
continual staff development; and regular
supervision and appraisal ensures their skills and
knowledge are always current and relevant to the
work undertaken.
Family and friends
Keeping in touch is essential for most people and
we work hard to positively support contact for
those who live at Hesley Village and those who
are important to them.
Each person has their own plan that identifies
how they will be supported to maintain
relationships and link with family and friends,
through means such as Skype, e-mail, text
messaging, telephone, letters and cards.
We support visits to the family home and also
welcome visits from family and friends to Hesley
Village. There are two “log cabin” style houses
available for visiting families free of charge
(subject to availability).
Families of people living at Hesley Village have
also formed their own forum, which is facilitated
by Hesley but run independently by the families
themselves.
Other providers and transition
When people move from another service to
Hesley Village, or from Hesley Village elsewhere,
we work very hard to ensure a smooth transition
takes place and that people have the best
possible experience in what can be a stressful
time of their lives.
To support people’s health and social care and
other needs, we seek to work closely and co-
operatively with everyone concerned, including
the person themselves, their family/carers, their
social worker and other representatives and local
NHS services, for example.
Hesley Group takes pride in a low staff turnover. A
number of staff have achieved long-term service
awards for 10 and 20 years which promotes
stability, consistency and trust which, in turn,
support positive provision for those who live at
Hesley Village.

respectful relationships between
those who live at Hesley Village
and their families, our staff and key
external agencies, as fundamental
to delivering the best possible
service.

Quality Assurance  Policy
We use a range of policy and guidance
processes to manage our services safely and
effectively. Examples of key policy areas are:

The Hesley Group has a formal policy and
procedure for complaints . A copy of this and
our complaints form is available on request.
The Hesley Group has a Quality Team that
develops policy and best practice and
initiatives, assesses how well we are doing in
meeting our objectives and complying with
national standards, including the management
of health and safety and keeping people safe.
The team consults with people using the service
and their families/representatives.
The Quality Governance Committee oversees all
Hesley Group services and has an independent
Chair and two independent persons to advise
and take an impartial view on the progress
and achievement of our services. They hold
the senior management of the organisation to
account.

  • 5. HesleyGroup|5 Overview Hesley Village is situated in open countryside within a large 100 acre site about two miles from the small South Yorkshire villages of Tickhill and Rossington. The service provides high standards of accommodation, personal care and support for younger adults with learning disabilities who often (but not always) also have an autistic spectrum disorder. We may occasionally provide services to young adults aged 16 or 17 who are in transition to adults services though generally we support adults aged 18 and above. There are onsite facilities to support people with occupational and employment opportunities and transport to support people to access services and facilities locally. Hesley Village operates a residential model. Most people have single person accommodation although a small number of houses are shared between a maximum of three people. We place a strong emphasis on supporting people to develop the skills which will allow them, where possible to lead more independent lives, to achieve social inclusion and to be involved in meaningful activity of their own choice. We aim to support people to reach their full potential by providing: • High standards of individualised support through the development of person centred care pathways. We aim to ensure this is delivered at all times in ways that are aimed at achieving positive outcomes for people and that are fully compliant with Essential Standards of Quality and Safety (CQC 2010) and associated regulations through regular monitoring, review and consultation. • Options for people to develop their life skills and access vocational and employment opportunities in-house and in the wider community. • Clinical support and consultancy when it is needed, to help people develop coping skills and also to train our staff to intervene and support effectively and help us provide an enabling environment. Hesley Group’s clinical services team includes speech and language therapists, applied behaviour analysts, clinical psychologists, communication co-ordinators and occupational therapy services. • Preparation and support for people and for their families when moving onto the next stage in their lives by effective transition planning. • Support for people to achieve and maintain the best possible health and access healthcare services as needed. • A safe but least restrictive environment for people through positive risk management, monitoring of outcomes and regular review of risk assessment and support plans. Safeguarding Hesley Group offers a person centred approach which aims to support people to develop increasing levels of independence and to create a safe environment for them, in which and from which they can have the best possible opportunities to maximise their potential. Our philosophy is one of warmth, security, consistency, understanding and trust. Our safeguarding arrangements sit in this context, our policies, procedures, staff training and quality assurance provide a robust and regularly reviewed framework to this philosophy. Health and Safety We are committed to ensuring the health, safety and welfare of all those who live and work in, or otherwise access our services. Hesley Village adheres to all procedures in place under the Hesley Group’s Health and Safety Policy. The people we support all have complex needs, often including behaviours that can challenge them and the services that support them. Our aim is to enable people to fulfil their potential; to live their lives as positively as possible within an environment that is appropriate to each person’s individual needs; and to support people with their health and personal care needs so they can achieve a healthy and fulfilling lifestyle. Through our Single Equality Scheme and Staff Code of Practice we seek to ensure our services do not discriminate against people who live in or otherwise access our services; or the staff who support them, because of their race, religion, culture, belief, disability, sex or age. HesleyGroup|6 The People who live at Hesley Village Age range: 18 years and over Sex: Mixed Capacity: 76 people, living in high quality single person flats and 3 person houses or bungalows People who live at Hesley Village all have complex needs, including behaviour that may challenge and a learning disability, often in association with autism.
  • 7. HesleyGroup|7 Person Centred Focus We plan and deliver services that are built around the person, their needs, wishes and aspirations. Each person has a range of plans that reflect their personal, social and health care needs, therapeutic and behavioural support, communication plans, skills acquisition and development. These plans are developed with the person, as well as others important to them, including their families and professionals who know them well. Plans are regularly reviewed and updated through multi disciplinary team processes, keeping the person receiving the support firmly at the centre of all planning. Clear policies are in place - such as People’s Rights and Having a Say, Effective Behaviour Support, Capacity and Consent – that clearly identify the expectation that we consult with the person and/or their representatives. Where people cannot give consent we always aim to deliver outcomes that are agreed to be in the best interests of the person. At Hesley Village we have a Residents Council (their chosen name) that meets regularly and acts as a self advocacy group. Our aim is to support people to achieve positive outcomes and plans are outcome based, within an agreed framework of positive and appropriate risk management. Because the majority of people we support have complex communication needs in addition to behaviour that may challenge, we work to a communication policy and charter and aim to put the person at the centre of everything we do. We have a flexible and positively motivated staff team, trained and skilled in effective person-centred therapeutic support and communication with those who live at Hesley Village. All staff attend training in relation to HELP (Hesley Enhancing Lives Programme), a behaviour support approach based on TCI (Therapeutic Crisis Intervention), which is BILD (British Institute of Learning Disabilities) accredited. TCI is an internationally recognised, crisis prevention and management system that reduces the need to rely on high-risk interventions. TCI recognises that it is the actions and reactions of those around people that strongly shape and influence their behaviour, including their social interactions and emotional responses. Its emphasis is on empathy and proactive support, while physical interventions are very much considered a last resort. The care and well being of the people we support are paramount and everything we do is around seeking to ensure that each person’s best interests are met. In order to keep people safe there are times when the use of physical interventions may be needed. This would be as a last resort to support people in crisis, in order to reduce both stress and risk. To do this we follow BILD (British Institute of Learning Disabilities) guidance. The Hesley Group have BILD accreditation for both children’s and adult services. Our approach encompasses physical interventions contained within Therapeutic Crisis Intervention (TCI) strategies and those from PROACT-SCIP (r) UK. Any potential physical interventions are fully discussed with the person, in respect of whom they are proposed and/ or their family and other appropriate representatives, as far as possible, prior to their application, which will continue to remain as infrequent as is possible; and which will continue to be reviewed, with the ongoing aim of reducing and stopping their use, as soon as this can safely be done. Both of these approaches can be explored further via the BILD website – www.bild.org.uk – in the section: Physical Intervention Accreditation. The multidisciplinary therapeutic approach taken at Hesley Village encompasses a wide range of expertise in addition to that of our core staff teams and management; and also that available from externally to the Village. Our own multidisciplinary team includes the following: Communication and Speech and Language We aim for a total communication environment and to this end all staff are trained and supported in using augmented systems of communication, such as signing and picture symbols. A skilled and experienced team of speech and language therapists (SALTs) are supported on a day to day basis by communication coordinators. Our SALTs not only deal with issues relating to communication; some are also specialists in dealing with the eating and chewing or swallowing difficulties (dysphagia) that can present problems for some of those who access our services. Behaviour Support People may access applied behaviour analysts (ABAs) on an individual basis to allow a thorough assessment of their needs to be made and for the function of the person’s behaviour to be understood. ABAs also train and support our staff to provide effective techniques for helping to manage and lessen or change responses such as physical aggression or self injury. All staff are expected to use the person’s HELP Profile, an agreed common approach, in response to a person’s behaviour and to report where this is effective or otherwise. Psychology Our clinical psychologists work to support the emotional health and wellbeing of people by helping each person and the staff supporting them to understand their thoughts, feelings and behaviours and manage these more effectively. Clinical psychologists integrate information from the individual, their family, and the people supporting them, with the results of structured assessments of neurodevelopment, emotional attachment patterns, cognitive (thinking) skills, and adaptive skills, to develop a comprehensive understanding of each person’s needs and how best to support them. The resulting formulation is used to develop and implement interventions which may be used with the individual (eg non-directive counselling, or adapted cognitive-behavioural inerventions), or with the staff supporting them (for example: developing their understanding of how an individual’s needs/ abilities/diagnosis affect their life and how best to support them). Clinical psychologists work with other members of the multidisciplinary clinical team to meet the therapeutic needs of those receiving support in Hesley Group services in a coordinated, timely and holistic way. Occupational Therapy Our Occupational Therapy (OT) service at Hesley Village is key to supporting active, meaningful lives, for everybody who lives with us, through the Opportunities Team (see page 8 for more details) The Occupational Therapy service also provides for individual assessment, sensory needs and skills development; and the provision of a safe environment and any equipment needed to support this. Psychiatry We have a consultant psychiatrist, who holds regular reviews at Hesley Village and advises our service and supports people who require psychiatric input to remain emotionally well. For emergency psychiatric support people at Hesley Village can access local NHS services, with whom we have strong and positive links. Therapeutic Support
  • 8. HesleyGroup|8 Active, Meaningful Lives Many people come to live at Hesley Village after long periods, perhaps years, of being unsettled and being unable to achieve their full potential. For others a move to Hesley Village comes from more settled circumstances but still represents a significant transition, particularly in the context of the multiple and complex needs which are the experience of all those who live with us. As a first step we seek to support people through this transition, to achieve stability, initially within the Hesley Village community, as a firm foundation for each person’s future progress. We have many years’ experience of facilitating and supporting these transitions and achieving stability. Because people’s needs, wishes, and abilities vary, we support people in individualised and flexible ways. Our aim is for them to have the best quality of life possible. We place a strong emphasis on people having meaningful and active lives. We are keen to support activity away from Hesley Village, as well as within the Village itself. Some people have currently, or have had, work placements in nearby Tickhill and Doncaster and also Sheffield. Others participate in employment nearer to home, which can vary from helping with the recruitment and selection process for new staff, to giving support with the recycling programme. People are supported to access the full range of activities and amenities in the local community, as well as at more distant locations; and also at Hesley Village, where arts and crafts or horticulture, cooking and other activities are available on site, as well as a cinema, function room (in the Village Hall), hairdresser and beautician, grocery shop, post office and cash machine. The local Doncaster bus has a stop at Hesley Village. The whole process of supporting active and meaningful lives for those who live at Hesley Village, has been greatly enhanced by the development of the Opportunities Team, which comprises a specialist Occupational Therapist and Occupational Therapy Assistant, together with a team of facilitators, who develop an overall plan of activities, run sessions (including group sessions and outreach sessions in people’s own homes); and bring people in from outside Hesley Village to run time-limited and focussed courses; from all of which Care Managers can develop individualised timetables for each person who lives in the Village. This service is currently provided through each week; and sits alongside other individualised, particularly leisure, activity. The plan is for the Opportunities Team to extend their provision, to cover weekends too. The Opportunities Team link in with the multidisciplinary (staff and clinical) team, who identify where particular activities may have therapeutic benefit for a particular person. There is a mixture of drop-in activities, for example on an evening; and regular therapeutic activities delivered through the day, both by Hesley Village staff, as well as by external people, including drama, football skills (with the involvement of Doncaster Rovers) and music, for example. The Opportunities Team also run special events on a monthly basis, such as themed, cultural meals, an Olympic week in 2012, a Jubilee Celebration, a Christmas Party and others. People are supported to enjoy social events and to make friends. We aim to extend and develop people’s experiences of the wider community outside of home. Our hope and aim is for people to develop appropriate levels of increasing independence and meaningful social inclusion. Sometimes it can be understandably difficult for the parents themselves, of adults with such complex needs, to smoothly manage the transition to adulthood and all that this entails. We are sensitive to these issues at Hesley Village; and seek to understand and value family input and insights around this process. Developing Pathways When people come to live at Hesley Village, we develop with them, their families and other involved key parties, a pathway plan that includes desired outcomes and plans for the future. This may be a planned and progressive move toward living in a community setting with the right support, such as stepping down into a less intensive residential service or into supported living for example, perhaps within Hesley Group services or the services of other providers, with whom we seek to work in a positive and cooperative way. Progress against these goals is measured on an ongoing basis and annually as part of the formal review process, keeping the person at the centre of all planning. Reviews Each person has a formal annual review, usually with their family and the funding authority representative. We support people to express their views as a key part of this – where at all possible people will be supported to take part in their review, including with the involvement of advocates, where this need is identified. Additionally a rolling programme of case review is under- taken including the multidisciplinary team, to make sure people’s care and support needs are being met and/or adjusted on a continuing basis, with the aim of ensuring we fulfil our commitment to each person, of providing the best possible care and support at all times; and that this continues to evolve in accordance with the pathway plan.   • 9. HesleyGroup|9 Living at Hesley Village whether in respect of the decoration of their rooms or other issues relating to their homes or with regard to their lives in more general terms. The Village is in close vicinity to the local village of Tickhill and Doncaster town centre is easily accessed by the bus route that stops in the car park of the Village. There are a range of purpose-built workshops to accommodate a wide variety of activities in positive environments. Excellent on-site facilities are available to complement and enrich the lives of the people we support including a bistro, cinema, shop, bank/post office, therapy suite, hairdressers, clothes shop, bar and village hall. At Hesley Village, people live in single flats or three bedroom houses or bungalows. The choice of accommodation for each person takes into account their wishes and preferences, their needs, capabilities, age and social development. Each person has their own bedroom and bathroom or ensuite which ensures dignity and respect for the individual who may need intimate care. Modern fitted kitchens facilitate learning opportunities for cooking and spacious dining areas, lounges and gardens offer warm and welcoming areas to provide a homely setting. We aim to empower the person to take greater control over their own life; wherever possible we seek to enable people to make their own choices,

HesleyGroup|10 Working Together Coming to Live at Hesley Village A referral for someone to come and live at Hesley Village will come through the placing authority that will be funding the placement. The placing authority is requested to provide relevant information which we will review to ensure that Hesley Village is a suitable provider for the referred person. People will have a thorough assessment (for which there is no charge) prior to potentially being offered a place within Hesley Village. The assessment will cover all health and social needs and all information will be treated confidentially. Our assessment process will help to ensure that Hesley Village is able to meet the person’s needs and is used to inform their initial personal plan, should they come to our services. The assessment is carried out by an assessment team from Hesley Village and includes staff members from the Clinical and Care Management teams. They will arrange to meet the person and as many people involved in their care as possible, to see how best their needs and wishes can be met. This is the start of the person-centred planning that ensures each person is treated as an individual. If it is felt that Hesley Village is the best option for the person, the placing authority will be informed and the transition process begins. This will be managed by a senior manager at Hesley Village who will work with the person, family/carers and the placing authority to ensure the move is as stress- free as possible. You are more than welcome to visit us to find out more about this process and to ask any questions at all about Hesley Village. Staff and Support Hesley Village has a highly experienced, comprehensive staff team, comprising managerial and support staff who work closely with the multidisciplinary clinical team (described on page 7). The recruitment process is thorough and Disclosure Barring Service (DBS) checks are undertaken in respect of all employees who have opportunities for unsupervised contact with vulnerable people, prior to joining the Hesley Group. Every new member of staff has a comprehensive induction programme and continual staff development; and regular supervision and appraisal ensures their skills and knowledge are always current and relevant to the work undertaken. Family and friends Keeping in touch is essential for most people and we work hard to positively support contact for those who live at Hesley Village and those who are important to them. Each person has their own plan that identifies how they will be supported to maintain relationships and link with family and friends, through means such as Skype, e-mail, text messaging, telephone, letters and cards. We support visits to the family home and also welcome visits from family and friends to Hesley Village. There are two “log cabin” style houses available for visiting families free of charge (subject to availability). Families of people living at Hesley Village have also formed their own forum, which is facilitated by Hesley but run independently by the families themselves. Other providers and transition When people move from another service to Hesley Village, or from Hesley Village elsewhere, we work very hard to ensure a smooth transition takes place and that people have the best possible experience in what can be a stressful time of their lives. To support people’s health and social care and other needs, we seek to work closely and co- operatively with everyone concerned, including the person themselves, their family/carers, their social worker and other representatives and local NHS services, for example. Hesley Group takes pride in a low staff turnover. A number of staff have achieved long-term service awards for 10 and 20 years which promotes stability, consistency and trust which, in turn, support positive provision for those who live at Hesley Village. We see positive, trusting and respectful relationships between those who live at Hesley Village and their families, our staff and key external agencies, as fundamental to delivering the best possible service.
  • 11. HesleyGroup|11 Quality Assurance Policy We use a range of policy and guidance processes to manage our services safely and effectively. Examples of key policy areas are: • Managing risk • Behaviour support • Care planning • Single equality scheme • Health and safety arrangements • Capacity and consent • Safeguarding adults The Hesley Group has a formal policy and procedure for complaints . A copy of this and our complaints form is available on request. The Hesley Group has a Quality Team that develops policy and best practice and initiatives, assesses how well we are doing in meeting our objectives and complying with national standards, including the management of health and safety and keeping people safe. The team consults with people using the service and their families/representatives. The Quality Governance Committee oversees all Hesley Group services and has an independent Chair and two independent persons to advise and take an impartial view on the progress and achievement of our services. They hold the senior management of the organisation to account.
