understand, many instances involved significant human-errors and many more involved
ineffective supervision/Leadership
• One of the common findings in the day to day business is, individual’s do not care for their
personal safety
• Also people generally tend not to share any bad-news with the senior management.
• In our business, we talk about Safety Statistics and do probability studies and believe the
time gap between two hazards/incidents is 10 years or 100 years, etc.
• We generally do not consider double jeopardy scenario as a credible event
• Incidents/accidents in the Recent past, if we analyze, we would realize theoretical statistics
have been absolutely wrong and the frequency/fact of such hazards is much more, nearly 20
major incidents in the last 20 years is a real horror for the industry
