MEMORANDUM OF AGREEMENT
KNOW ALL MEN THESE PRESENTS:
This Memorandum of Agreement made and entered into by and between:
QUEZON CITY POLYTECHNIC UNIVERSITY, existing under the laws of the Republic
of the Philippines with address at 673 Quirino Highway, San Bartolome, Novaliches,
Quezon City 1116, represented herein by its OJT Adviser for Department of
Information Technology, _ PROF. NORILYN E. CASTRO , hereinafter referred to as
ACADEMIC INSTITUTION
and
QUEZON CITY POLYTECHNIC UNIVERSITY existing under the laws of the Republic
of the Philippines, with address at 673 Quirino Highway, San Bartolome, Novaliches,
Quezon City, represented hereinafter by its , _ PROF. NORILYN E. CASTRO ,
hereinafter referred to as PRACTICUM COOPERATOR;
WITHNESSETH
WHEREAS, the ACADEMIC INSTITUTION needs to send its student candidates for
graduation under its Bachelors Program to a company where the students will
undertake Practicum, which defined briefly, is the practical application of previously
studied theory;
WHEREAS, the ACADEMIC INSTITUTION hereby, request the PRACTICUM
COOPERATOR to accept its students to undertake Practicum; studies within their
corporate premises;
WHEREAS, the PRACTICUM COOPERATOR accedes to the request of the
ACADEMIC INSTITUTION regarding the Practicum students;
NOW, THEREFORE, for and in consideration of the foregoing premises, the parties
agree as follows:
1. The ACADEMIC INSTITUTION shall
1.1. Submit to the PRACTICUM COOPERATOR, the biodata and all the
documents including waivers of prospective Practicum students, which

shall serve as the basis of the PRACTICUM COOPERATOR to review,
select, and approve/reject specific students for practicum;
1.2. Designate the Academic Practicum Adviser who shall be reporting to the
Practicum Coordinator, all matters involving the scope and coverage of
the Practicum and its progress;
1.3. Cause of the acceptance, in writing, by the Practicum students of the
following:
a. the policy of the PRACTICUM COOPERATOR regarding data acquired
or gathered during the period of the Practicum;
b. the pertinent rules and regulations of the PRACTICUM
COOPERATOR applicable to the Practicum students;
c. the liability of the Practicum students for damage to property or for
personal injury by the student’s intentional or negligent act.
2. The PRACTICUM COOPERATOR shall
2.1. Review, select, and approve/reject specific students for Practicum;
2.2. Designate form among the employees, retainers or consultants, the
Company/Agency Practicum Adviser who shall perform the following
duties in conjunction with the Practicum:
a. review the objectives and coverage of the Practicum of each Practicum
student; subsequently approved/reject the coverage of the Practicum
period;
b. act as the Supervisor of the Practicum student while the practicum
student is within the premises of the PRACTICUM COOPERATOR,
and introduce or accompany the Practicum student to other operating
units of the PRACTICUM COOPERATOR as the need arises relative
to Practicum.
c. review results or reports prepared by the Practicum student regarding
Practicum prior to submission to the Academic Practicum Adviser of
the ACADEMIC INSTITUTION or to any newspaper or journal.
d. accomplish the evaluation form at the end of each Practicum stage,
and submit this to the Academic Practicum Adviser of the ACADEMIC
INSTITUTION.
3. Both the ACADEMIC INSTITUTION and the PRACTICUM COOPERATOR also
agree as follows:
3.1. Cognizant that the Practicum period covers one academic term, the
PRACTICUM COOPERATOR shall allow the Practicum student of the

ACADEMIC INSTITUTION to undertake Practicum for one academic term;
the ACADEMIC INSTITUTION shall exert all effort so that the Practicum
student undertakes Practicum in one academic term.
3.2. Although the minimum requirement 486 hours, the place where the 486
hours are spent shall be mutually agreed upon by the Practicum student of
the ACADEMIC INSTITUTION and the Company / Agency Practicum
Adviser of the PRACTICUM COOPERATOR, with due consideration to
the fact that the Practicum student is already a professional and may be
exempt from the use of time cards.
3.3. Regarding compensation / monetary allowance, the PRACTICUM
COOPERATOR is under no obligation to give such to the Practicum
student. If and when compensation/monetary allowance is given to the
Practicum student, such should be communicated in writing to the
ACADEMIC INSTITUTION and related policies included therein.
3.4. The PRACTICUM COOPERATOR is under no obligation to assist the
Practicum student regarding materials and equipment which are not
normally used in their day – to – day operations; the need for such
materials and equipment should be discussed at the early stages of the
Practicum so that sourcing and cost responsibility can be threased out at
the earliest possible time.
3.5. Any intellectual property right arising from the practicum shall be owned
equally by the Practicum student, the ACADEMIC INSTITUTION and the
PRACTICUM COOPERATOR. The registration of the intellectual property
right shall be jointly undertaken by the ACADEMIC INSTITUTION and the
PRACTICUM COOPERATOR with the assistance of the Practicum
student. All costs relative to said registration shall be shared equally by
the ACADEMIC INSTITUTION and the PRACTICUM COOPERATOR.
The internal policies of the ACADEMIC INSTITUTION and the
PRACTICUM COOPERATOR shall govern the ownership sharing of the
Practicum Coordinator with the Academic Practicum Adviser and the
Company / Agency Practicum Adviser respectively.
3.6. In the event that the practicum coverage shall become multidisciplinary
(i.e. not covered by a single discipline such as engineering, etc) or that the
estimated Practicum period shall span more than two academic terms
thus necessitating the deployment of more than one practicum student,
the Company Agency Practicum Adviser shall establish the standards with
which a particular Practicum student shall be evaluated, in coordination
with the particular Academic Practicum Adviser; after the establishment of

the agreed standards, the Practicum student shall be informed about
these and shall be asked to concur in writing.
4. This agreement shall take effect immediately upon signing hereof and shall
continue to be in effect as long as the Course Program of the ACADEMIC
INSTITUTION is in existence; provided that either party may seek to terminate
this agreement by giving notice to the other party at least one academic term (as
described in section 3.1 hereof) prior to the affectivity of termination.
5. This agreement may be revised or amended in part by written consent of both
parties.
6. Attached to this Agreement as the Annexes A and B are the written authorities of
the signatories of this instrument granted by the respective Board of Regents of
the ACADEMIC INSTITUTION and the PRACTICUM COOPERATOR.
