Why Root Cause Analysis ?
• Performance Analysis teams were
established in 2003 at Dorsey and 2004
for Radisson/Henday.
• The teams were developed to analyze
HVDC system performance.

• Review all forced outages.
• Initiate a root cause analysis for high impact
events and all inadvertent outages.
• Review reliability and performance issues that
may cause future forced outages.
• Document and track forced outages (and the
analysis) to ensure that the recommended
action is being taken.
