3. Advanced Healthcare Directive (AHD)
• An AHD gives you the power to choose
the level of care you want if you are
unable to communicate.
• Without an AHD, you will probably receive
the most aggressive treatments, even
when the chances of helping you are very
small.
• AHD is used interchangeably with the term
“Living Will.”

4. Do Not Resuscitate (DNR) Directive
• A legal medical instruction to both your physician and
family
• Eliminates the potential for conflict and declares whether or
not you want to be resuscitated when you are unable to
speak for yourself
• The physician and family must honor the patient’s
declaration under the DNR
• You may request a DNR form from your physician to fill out
at home or in the hospital.
• You may also request a DNR device that is worn as a
necklace or bracelet in lieu of a DNR form.
• The DNR form must be signed and witnessed by at least
two people.
• If you decide that you want to be resuscitated, doctors will
administer cardiopulmonary resuscitation, or CPR.

5. Healthcare Proxy or
Healthcare Power of Attorney
• A healthcare proxy is also called
healthcare power of attorney
• It is a person who is legally authorized to
speak for you:
• should know you well
• be trusted to do what you want
• be able to remain focused in a crisis.

6. Supplemental Letter
• An additional letter, video or tape that you
may leave for important people in your life
and for those who will be making decisions
for you after your death.
• It is a way to express your personal thoughts
and feelings.
• There is no right or wrong way to write or
create a supplemental letter.
• Use the supplemental letter to let your loved
ones know more about you, how you think
and what your wishes for them may be.

7. Why You Need to Know Your State Laws
• Advance health directive forms vary by
state.
• Use the state form of your primary
residence to make sure your wishes are
carried out. Refer to:
• AHD In Your State – Part 1 (Alabama – Montana)
• AHD In Your State – Part 2 (Nebraska – Wyoming)
• If you live in more than one state, it’s a
good idea to fill out forms for each.
