• Health Express began more than a decade ago, offering free non-
emergency medical transportation to those with disabilities and seniors.
Due to the growing need, a collaborative partnership was formed to offer
the service to all underserved populations within Placer County and
beyond. We are currently providing approximately 750 rides per month.

• Sutter Davis Hospital has partnered with CommuniCare Health
Centers for more than 15 years to improve access to primary and
mental health care for the uninsured and underinsured residents of Yolo
County. CommuniCare is a Federally Qualified Health Center (FQHC)
that provides general medicine, pediatrics, immunizations, women’s
health services, prenatal care, reproductive health services, dental
care, optometry, behavioral health services, chronic disease
management and more.

• The Interim Care Program (ICP) provides access to care for vulnerable
and traditionally underserved residents. Starting in 2004, SMCS was a
founding partner in establishing an 18-bed shelter for homeless men and
women to recuperate from medical conditions and linking them to vital
community services.

• Senior Recreation and Respite Program (R&R) connects the senior
population in Placer County, with important social and educational
activities. R&R is designed to meet the needs of older adults with memory
or physical impairments and provides recreational, social and educational
activities for the participants and respite for caregivers to enjoy some free
time.

• The Transitional Care Program (TCP) provides access to care for
vulnerable and traditionally underserved residents. TCP links homeless
adults to vital community services while giving them a place to heal and
receive ‘wrap around care’. The most innovative aspect of this program is
the unique collaboration between Sutter Solano Medical Center, NorthBay
Healthcare, Kaiser Permanente, the Solano Coalition for Better
Health, Solano County and the Benicia CAC.

Amador, El Dorado, Nevada, Placer,
Sacramento, Solano, Sutter, Yolo and Yuba.

o Sutter North Medical Group
• Active MDs on Staff: >1,300 PCPs and Specialists
• Sutter Medical Foundation provides free or low cost digital
mammograms to underinsured women throughout the Sutter Health
Sacramento Sierra Region. All women who need follow up care are
connected with appropriate community partners, to ensure they are
wrapped with the necessary services, like insurance and follow up primary
care.

Where do we invest our dollars?
• SHSSR reinvests in healthcare for the underinsured, with 2.5 million
dollars provided for charity care on a weekly basis.
• SHSSR invests 7 billion dollars a year in new facilities.
• SHSSR makes strategic investments in specialty care and
groundbreaking medical research.
• SHSSR invests in critical Community Benefits programs and initiatives.
In 2011 alone:
o Sutter Medical Foundation made multiple contributions to the
greater community, for a net Community Benefit of $41,460,352.
o Sutter Health Sacramento Sierra Region provided $166,662,457 in
benefits for the poor and underserved, and $8,145,718 in benefits
for the broader community, for a total quantifiable Community
Benefit of $174,808,175.
Priority Areas of Focus
SHSSR is interested in supporting organizations that work within the
priority investment areas of our regional hospital system and care
centers. Those priority areas include:
• Access to primary care services within underserved communities
• Removal of barriers to access that occur among those with mental
health and addiction issues
• Programs that support seniors
• Programs that support individuals with chronic diseases
• Programs that provide prenatal and early childhood health
services, especially to mothers and families with limited resources
• Programs that support economic development and viability in the
communities we serve
Proposer Requirements
Our sponsorship commitments extend beyond our Community
Investment Awards (grant funding opportunities) in support of such
programs, and when sponsoring, we look for:
• Organizations that engage a broad constituency of support, including
but not limited to civic, community and regional leaders, as well as
foundations, non-profits and the business community.
• Events that celebrate successful health-related programs with proven
impact.
• Events that provide visibility to SHSSR representatives, either with a
presence at the event or an opportunity to highlight Sutter’s support of
the organization.
• Events and efforts that foster economic development and vitality, which
is imperative to the overall strength and health of our greater
community .
** Remember that if you have received a Community Investment
Award, you are not eligible for sponsorship funding.
Proposer Process
1. Complete this mandatory Sponsorship Webinar hosted by SHSSR.
2. Complete the Sponsorship Proposal form according to all posted rules
and guidelines available at the end of this Webinar.
3. Submit sponsorship proposals according to the Winter & Spring
timelines to shssrsponsorships@sutterhealth.org.
Note: We will only accept applications sent to the email address
specified above and the application must be sent by the person
making the request. Unfortunately, we are unable to accept
applications forwarded on the requestor’s behalf.
4. Sponsorship Proposals will be reviewed and awardees will be notified
within eight weeks of submission.

What format do I submit my proposal in?
• There is now a Sponsorship Proposal form. Please visit
www.checksutterfirst.org/community to obtain the form.
How do I apply if my organization has multiple events that cover both
the Winter and Spring timelines?
• Please complete one application for all events year around. You must
include requested dollar amounts for each event.
If my event/organization has multiple sponsorship levels should I
include all options in the proposal?
• Yes, but please be sure to still request a specific dollar amount.
How do I report measurable outcomes for my event(s)?
• After your event has concluded, please report your measurable
outcomes by completing this survey.

If I received a Community Award Investment, can I apply for sponsorship?
• No. We appreciate your commitment to the community, but we can only
support your commitments in the form of Community Award Investments OR
sponsorships. If you have questions about this, email
shssrsponsorships@sutterhealth.org.
What is the difference between a Community Award Investment and a
sponsorship?
• Community Investments (grants) are based on our community health needs
assessment and focus on programs that increase access for the underinsured.
These are sponsorships that allow Sutter to collaborate with community
based organizations for their events and creates opportunities for Sutter to
support community.

Due to the growing need, a collaborative partnership was formed to offer the service to all underserved populations within Placer County and beyond. We are currently providing approximately 750 rides per month.

CommuniCare is a Federally Qualified Health Center (FQHC) that provides general medicine, pediatrics, immunizations, women’s health services, prenatal care, reproductive health services, dental care, optometry, behavioral health services, chronic disease management and more.

Sutter Medical Center Sacramento Site Specifics • Beds: 892 • Campuses o Sutter General Hospital o Sutter Memorial Hospital o Sutter Center for Psychiatry Quick Facts • Active MDs on Staff: 987 • Employees: 5,147 • The Interim Care Program (ICP) provides access to care for vulnerable and traditionally underserved residents. Starting in 2004, SMCS was a founding partner in establishing an 18-bed shelter for homeless men and women to recuperate from medical conditions and linking them to vital community services.

Sutter Roseville Medical Center Site Specifics • Beds: 332 • Ambulatory Surgery Center: 1 • Onsite Medical Office Buildings: 4 • NICU • Sutter Rehabilitation Institute Quick Facts • Active MDs on Staff: 330 • Employees: 1,456 • Senior Recreation and Respite Program (R&R) connects the senior population in Placer County, with important social and educational activities. R&R is designed to meet the needs of older adults with memory or physical impairments and provides recreational, social and educational activities for the participants and respite for caregivers to enjoy some free time.

Sutter Solano Medical Center Site Specifics • Acute Care Beds: 102 • Sutter Solano Cancer Center Quick Facts • Active MDs on Staff: 164 • Employees: 650 • 2008 Community Benefit Contribution: $12.7M • The Transitional Care Program (TCP) provides access to care for vulnerable and traditionally underserved residents. TCP links homeless adults to vital community services while giving them a place to heal and receive ‘wrap around care’. The most innovative aspect of this program is the unique collaboration between Sutter Solano Medical Center, NorthBay Healthcare, Kaiser Permanente, the Solano Coalition for Better Health, Solano County and the Benicia CAC.

Sutter Medical Foundation • Counties Include: Amador, El Dorado, Nevada, Placer, Sacramento, Solano, Sutter, Yolo and Yuba. • Locations: 52 • Medical Groups: o Sutter Medical Group o Sutter North Medical Group • Active MDs on Staff: >1,300 PCPs and Specialists • Sutter Medical Foundation provides free or low cost digital mammograms to underinsured women throughout the Sutter Health Sacramento Sierra Region. All women who need follow up care are connected with appropriate community partners, to ensure they are wrapped with the necessary services, like insurance and follow up primary care.

Where do we invest our dollars? • SHSSR reinvests in healthcare for the underinsured, with 2.5 million dollars provided for charity care on a weekly basis. • SHSSR invests 7 billion dollars a year in new facilities. • SHSSR makes strategic investments in specialty care and groundbreaking medical research. • SHSSR invests in critical Community Benefits programs and initiatives. In 2011 alone: o Sutter Medical Foundation made multiple contributions to the greater community, for a net Community Benefit of $41,460,352. o Sutter Health Sacramento Sierra Region provided $166,662,457 in benefits for the poor and underserved, and $8,145,718 in benefits for the broader community, for a total quantifiable Community Benefit of $174,808,175.
  • 13. Priority Areas of Focus SHSSR is interested in supporting organizations that work within the priority investment areas of our regional hospital system and care centers. Those priority areas include: • Access to primary care services within underserved communities • Removal of barriers to access that occur among those with mental health and addiction issues • Programs that support seniors • Programs that support individuals with chronic diseases • Programs that provide prenatal and early childhood health services, especially to mothers and families with limited resources • Programs that support economic development and viability in the communities we serve

Proposer Requirements Our sponsorship commitments extend beyond our Community Investment Awards (grant funding opportunities) in support of such programs, and when sponsoring, we look for: • Organizations that engage a broad constituency of support, including but not limited to civic, community and regional leaders, as well as foundations, non-profits and the business community. • Events that celebrate successful health-related programs with proven impact. • Events that provide visibility to SHSSR representatives, either with a presence at the event or an opportunity to highlight Sutter’s support of the organization. • Events and efforts that foster economic development and vitality, which is imperative to the overall strength and health of our greater community . ** Remember that if you have received a Community Investment Award, you are not eligible for sponsorship funding.
  • 15. Proposer Process 1. Complete this mandatory Sponsorship Webinar hosted by SHSSR. 2. Complete the Sponsorship Proposal form according to all posted rules and guidelines available at the end of this Webinar. 3. Submit sponsorship proposals according to the Winter & Spring timelines to shssrsponsorships@sutterhealth.org. Note: We will only accept applications sent to the email address specified above and the application must be sent by the person making the request. Unfortunately, we are unable to accept applications forwarded on the requestor’s behalf. 4. Sponsorship Proposals will be reviewed and awardees will be notified within eight weeks of submission.
  • 16. Sponsorship Awards Awardees may be provided some or all of the following resources and tools: • SHSSR messaging • SHSSR logos • SHSSR videos • Final reporting guidelines, timelines and tools

  • 19. Frequently Asked Questions What format do I submit my proposal in? • There is now a Sponsorship Proposal form. Please visit www.checksutterfirst.org/community to obtain the form. How do I apply if my organization has multiple events that cover both the Winter and Spring timelines? • Please complete one application for all events year around. You must include requested dollar amounts for each event. If my event/organization has multiple sponsorship levels should I include all options in the proposal? • Yes, but please be sure to still request a specific dollar amount. How do I report measurable outcomes for my event(s)? • After your event has concluded, please report your measurable outcomes by completing this survey.

Frequently Asked Questions (continued) If I received a Community Award Investment, can I apply for sponsorship? • No. We appreciate your commitment to the community, but we can only support your commitments in the form of Community Award Investments OR sponsorships. If you have questions about this, email shssrsponsorships@sutterhealth.org. What is the difference between a Community Award Investment and a sponsorship? • Community Investments (grants) are based on our community health needs assessment and focus on programs that increase access for the underinsured. These are sponsorships that allow Sutter to collaborate with community based organizations for their events and creates opportunities for Sutter to support community.
