2. Casus
• Vrouw, 28 jaar, negroïde (Zuid-Afrika)
• Komt op SEH met visusdaling OD>OS sinds 1 dag
• Voorgeschiedenis:
• HIV-positief, behandeling sinds 2002
• Reumatische klachten
• Tuberculose, behandeld vanaf 22/5/12
• Medicatie:
• Chronisch HIV medicatie: truvada, raltegravir, ritonavir, darunavir
• Chronisch medicatie bij reumatische klachten: plaquenil 200 mg
• TB medicatie: isoniazide, rifabutine, pyridoxine
3. Oogheelkundig onderzoek
• Fotofobie ++
• Visus OD: lichtperceptie
• Visus OS: 0.6
• Media OD: cornea-oedeem, VOK vol cellen met hypopyon
• Media OS: enkele cellen in de VOK
• Fundus OD: niet te beoordelen
• Fundus OS: geen duidelijke afwijkingen
4. Differentiaal diagnose
• Auto-immuun
• gezien artritis eerder of toch reactieve artritis door
andere oorzaak (infectieus?)
• Infectieus
• TBC, herpes (HSV/VZV), toxoplasmose, CMV, lues
• Medicamenteus
• tgv rifabutin?
5. Plan
• Start Pred Forte 8 dd OD, 3 dd OS, Ultracortenol oogzalf
a.n. OD, Atropine 1% OD
• VOK punctie met PCR op HSV, VZV, CMV,
toxoplasmose en serologie op antistoffen tegen
Treponoma pallidum
• Overleg infectioloog: rifabutin is diezelfde ochtend al uit
baxters gehaald en vervangen door moxifloxacine.
6. Vervolg
• PCR negatief
• Geen aanwijzingen auto-immuun aandoening bij eerder
onderzoek
• Verbetering uveïtis beeld OD bij Pred Forte a 1 uur,
Ultracortenol zalf en atropine 1 dd OD
• Toch Rifabutin gebruik als oorzaak?
7. Uveitis bij Rifabutin gebruik
• Fulminante uveitis anterior met soms vitritis
• Perifere gelige endotheel deposities variërend in grootte
• Kan zich ook monoculair presenteren
• T ½: 35-40 uur
• Behandelen met steroïden
• Enigszins dosis-afhankelijk
• Verbetert altijd na staken rifabutin, recidiveert vaak bij
continueren/hervatten.

12. Vervolg casus
• Drie dagen later ook fulminante uveitis anterior OS
• Verbetering op topicale sterioden ODS in afbouwschema
• Na 1 maand:
• Visus OD 0.8++
• Visus OS 1.0+
• Media ODS
• geen cellen of Tyndall in voorste oogkamer, oude
glasvochttroebelingen, zonder tekenen van actieve
ontsteking.
• Fundus ODS
• geen haarden
