Hemorrhagic stroke or stroke of unknown origin at any time

Recent major trauma/surgery/head injury (within preceding 3 weeks)
Gastro-intestinal bleeding within the last month
Known bleeding disorder
Aortic dissection
Non-compressible punctures (e.g. liver biopsy, lumbar puncture)
Relative contraindications
Transient ischemic attack in preceding 6 months
Oral anticoagulant therapy
Pregnancy or within 1 week post partum
Refractory hypertension (SBP>180 mmHg and/or DBP > 110 mmHg, all effort need to be
made to normalize BP with IV Nitrates or beta blockers as appropriate prior initiation of
thrombolytics)
Advanced liver disease.
Infective endocarditis
Active peptic ulcer
Refractory resuscitation or traumatic resuscitation (greater than 10 minutes)
History of chronic severe, poorly controlled hypertension

Doses of Antithrombin Co-therapies
With Fibrinolytic Treatment
Enoxaparin:
Inpatients < 75 years and creatinine levels ≤ 2.5 mg/mL or ≤ 221 µmol/L
(men) or ≤ 2 mg/ml or 177 µmol/L (women): i.v. bolus of 30 mg followed 15
min later by s.c. dose of 1 mg/kg every 12 h until hospital discharge for a
maximum of 8 days. The first two S.C. doses should not exceed 100 mg.
In patients > 75 years: no i.v. bolus; start with first S.C. dose of 0.75 mg/kg
with a maximum of 75 mg for the first two S.C. doses.
In patients with creatinine clearance of < 30 mL/min, regardless of age, the S.C.
doses are repeated every 24 h
Heparin:
i.v. bolus of 60 U/kg with a maximum of 4000 U followed by an i.v. infusion of
12 U/kg with a maximum of 1000 U/h for 24 to 48 h. Target aPTT: 50-70 s to
be monitored at 3, 6, 12 and 24 h
Fondaparinux: 2.5 mg i.v. bolus followed by a s.c. dose of 2.5 mg once daily up to 8 days or
hospital discharge if creatinine ≤ 3 mg/mL or 265 µmol/L

18. 2. ต้ องติดตามคลื่นไฟฟ้ าหัวใจ 12 Lead ทุกๆ 30 นาทีภายในช่วงเวลา 90-120 นาทีหลังให้
ยาละลายลิ่มเลือด เพื่อประเมินการเปิ ดหลอดเลือดหัวใจ หากอาการเจ็บเค้ นอกลดลง และคลื่นไฟฟ้ า
หัวใจแสดง ST segment ลงต่าลงอย่างน้ อยร้ อยละ 50 ภายในช่วงเวลา 90-120 นาทีหลังเริ่มให้ ยา
ละลายลิ่มเลือด แสดงว่าหลอดเลือดหัวใจน่าจะเปิ ด
3. ควรส่งต่อผู้ป่วยเพื่อทาการขยายหลอดเลือดหัวใจในสถานพยาบาลที่มีความพร้ อมโดยเร็ว
ที่สุด หากอาการเจ็บเค้ นอกไม่ดีข้ ึน และมีสัญญาณของการเปิ ดหลอดเลือดภายในช่วงเวลา 90-120

120 นาทีหลังได้ รับยาละลายลิ่มเลือด
3. ภาวะหัวใจเต้ นผิดจังหวะ ได้ แก่
3.1.Accelerated idioventricular rhythm
3.2. Frequent premature ventricular complexes (พบได้ ถ่ีมากขึ้นกว่าเดิม 2 เท่าภายใน 90
นาทีหลังให้ ยาละลายลิ่มเลือด
3.3. Nonsustained ventricular tachycardia (NSVT)
4. ระดับ cardiac enzyme CK-MB จะขึ้นสูงสุดประมาณ 12 ชั่วโมงหลังอาการเจ็บหน้ าอก
