El procedimiento anestésico se inicia de manera estánar,reaizando una intubación orotraqueal con tubo flexometálico del diámetro adecuado. Posteriormente se realiza una incisión de unos 2-3 cm en uno de los lados de la región submentoniana1 o bien en la línea media7,8 a unos 2 cm por debajo de la mandíbula. Desde allí se lleva a cabo disección roma con una pinza hemostática larga, que se desliza por el borde interno del cuerpo mandibular hasta el suelo de la boca, penetrando en él entre el conducto de Wharton y el nervio lingual y la mandíbula lungs are ventilated with an FIO 2 of 1.0 throughout the procedure. La complicacion más frecuentes es la infección de la herida quirúrgica, que suele curar sin dejar secuelas estéticas7,10. En este sentido, es mejor no suturar la herida cervical, pues casi siempre se establece una pequeña fístula oro-cutánea que cierra mejor por segunda intención que si realizamos la sutura primaria. Otras complicaciones menos frecuentes son la lesión del nervio lingual y del conducto de Wharton, que se evitan fácilmente si en la disección seguimos la cortical interna mandibular.
