Home care provided for those who feel
they will need support with daily tasks

We will work with you to put a care
plan in place that ensures you are
taken care of as soon as you are
discharged from hospital to allow the
best chance of healing and recovery.
12. Your home care plan
A care package is set-up following a full
discussion with the client about their
requirements and home care expectations.
Our team follow a stringent process which
allows them to set up a suitable personalised
plan.
The first stage of a care plan is a home visit
for an informal chat about what you are
looking for from your care package and the
number of expected daily visits required.
13. Confirmation of care plan
One of our Field Supervisors will come out
to :
• carry out a complete Care Needs
Assessment to confirm your Personal
Care Plan,
• carry out a Health and Safety
assessment (to ensure you and our care
workers are able to work safely in your
home),
• issue a copy of our terms of business
and answer any questions.
14. Your care plan and
funding
No mater whether your package is
funded by Direct Payments, Continuing
Care, Independent Living Funds, Social
Services, Personal Budget or you are
Privately Funded – we always start out
with the same care needs assessment
plan.
