Inflammatory back pain is a key feature of axial spondyloarthritis (SpA), which includes ankylosing spondylitis (AS). Diagnostic clues for SpA include inflammatory back pain, sacroiliitis and spondylitis seen on imaging, and enthesitis. Classification criteria have evolved from 1984 criteria focusing only on AS, to 2009 ASAS criteria capturing the broader concept of axial SpA. The diagnosis of axial SpA is supported by the presence of inflammatory back pain, HLA-B27 positivity and MRI evidence of sacroiliitis or spondylitis.
