MyStory Champion Commitment
MyStory Project UAMCH
MyStory Champion Description:
A MyStory Champion is a caregiver who is a role model for providing patient/family
centered care, and who is committed to strengthening the culture of care we provide the
pediatric patients and families. A MyStory Champion has the ability and willingness to
dedicate time, energy and talent to support the MyStory initiative and to assist in
sustaining and disseminating this initiative. MyStory Champions actively demonstrate the
University of Minnesota Amplatz Children’s Hospital’s vision: Together we transform the
health of children.
Qualifications of the individuals:
• Open to all interprofessional caregivers who work with pediatric patients at UAMCH
• Embraces change and acts as a change agent
• Positive, enthusiastic and approachable.
• Demonstrates commitment to professional growth
Actions reflect commitment to UMACH’s values of dignity, integrity, service and
compassion and UAMCH’s focused culture work.

• Strong interpersonal and collaboration skills
• Endorsed by peers, health team and management
Actions and words reflect positive working relationship with co-workers, other
disciplines and departments

Functions:
MyStory Champions serve as a liaison to interprofessional team members and work
collaboratively with pediatric inpatient unit/department leadership and MyStory PI
(Cheristi Cogentta-Rieke) to:
• Energize colleagues about the MyStory Initiative
Educate peers and interprofessional co-workers about the importance and value of
gathering and using the patient’s “story”
•
• Helps interprofessional team members recognize their role in the MyStory initiative
Enlist and engages unit/department based committees and councils to get onboard
with MyStory initiative
•
Responsibilities:
MyStory interprofessional Champions demonstrate their active involvement and
contributions by doing the following:
• Complete the MyStory education, play/practice with the MyStory tool in Epic
• Attend the MyStory kick-off event
Talk about MyStory with your peers – help them understand the importance of the
initiative
•
• Be available to answer questions during your shift(s) about MyStory
Provide peer-to-peer leadership for the MyStory education/go-
live/maintain/sustain/disseminate plans.
•
Contribute to either unit or departmental publications/ activities aimed at the
MyStory initiative
•

Formally updates co-workers about MyStory related activities on a routine basis at
MYSTORY INTERIM REPORT #1
meetings or through unit/department specific publications
•
Help co-workers be accountable for learning each patient’s story, using the MyStory
tool in Epic, and using the “story” for a means of consistent interactions with
•
patients/families.
Structure:
Each unit/department should have one to two champions. The champions should
be representative of the clinical area in regards to shifts and experience level (e.g.
•
experienced staff, newer staff, various shifts, etc)
Meetings: The MyStory Champions will meet for 1 hour every month for sharing of
ideas.
Compensation: Payment for hours will be from a budget outside of the clinical care
area.
