This is the only line and block chart you’ll see and it simply depicts the key components of our MHS. A key point here is to show you that the USFHP is a fully integrated part of our MHS. FOR RESERVE COMPONENT (National Guard and Reserve) AUDIENCES: Emphasize the use of Reserve Select and Retired Reserve in the context of CONTINUITY OF CARE using JH Health System MEMBER ORGANIZATIONS (NEW correct terminology) where appropriate. The Johns Hopkins USFHP Network is a full service network , including all Johns Hopkins Community Physicians locations ( 22 ) , serving as the largest primary care provider group for the Plan in Maryland, plus other select network community primary care ( total 80 ) and specialty providers, overall totaling 3,000 throughout our service area. Our service area includes all of MD and parts of DC, DE, South Central PA , Northern VA, and Northeast WV. As a member, you also have access to the entire Johns Hopkins Health System, including our Johns Hopkins Health Care and Surgery Centers (see below), as well as 40+ hospitals throughout our service area, including The Johns Hopkins Hospital, Johns Hopkins Bayview, Howard County General Hospital , Suburban Hospital and Sibley Memorial Hospital. The USFHP is a comprehensive health care plan, including A fully integrated health system Community locations Easy-to-make appointments – most practices also offer evening appointments at least one night per week. Upon enrollment, you have the opportunity to select your USFHP Primary Care Doctor. The USFHP offers comprehensive preventive health care. Prescriptions are filled through Rite Aid pharmacies, some of which are co-located with our practices. I’ll cover the DoD-designated co-pays later. No claims to file. In addition, There are no pre-existing condition restrictions For Medicare-eligible beneficiaries: [INCLUDE WHEN APPROPRIATE.] if you join USFHP on or before Sep 30, 2012 , you can remain with the USFHP beyond age 65/for life with or without Medicare Part B. [Once enrolled in Medicare, the USFHP is your primary coverage and Medicare may be used for benefits not covered by the Plan, such as limited chiropractic services.] If NOT a USFHP member on or before Sep 30, 2012, you are NOT eligible to join the Plan For Under age 65 beneficiaries, if you join USFHP after Sep 30, 2012 , you must disenroll & enroll in Medicare at age 65 [We encourage you to enroll in Medicare Part B so you do not incur a penalty for delayed enrollment (10% per each 12-month period beyond the original eligibility date). However, Medicare enrollment is NOT a prerequisite.] Beyond the TRICARE Prime benefits, John Hopkins has negotiated discounted rates for dental and vision services: Two free dental cleanings per year. Dental care is through United Concordia Dental Value Network providers in our area. Annual eye exam is covered. Other vision services are through the Block Vision Network providers and Hopkins Wilmer Eye Institute Optical Shops (in MD). If you need Specialist Care, you can use neighborhood specialists in our network or request to be referred into the Hopkins Health System . USFHP member satisfaction remains high… NEXT SLIDE

All these beneficiary categories are eligible to enroll in the USFHP. [ INCLUDE WHEN APPROPRIATE] Retirees and retiree family members who are 65 or older, or who will turn 65 before or in Sep 2012, you must enroll prior to Sep 30, or you will no longer be eligible for the USFHP. If you are under 65 and enroll after 30 Sep 2012, you must disenroll from the USFHP and enroll in Medicare and be eligible for TRICARE For Life. PLEASE SEE THE NDAA HANDOUT/INSERT IN OUR PACKET.
