Background: Cholesterol-lowering foods may be more effective when consumed as combinations rather
than as single foods.
Objectives: Our aims were to determine the effectiveness of consuming a combination of cholesterol-
lowering foods (dietary portfolio) under real-world conditions and to compare these results with
published data from the same participants who had undergone 4-wk metabolic studies to compare the
same dietary portfolio with the effects of a statin.
Design: For 12 mo, 66 hyperlipidemic participants were prescribed diets high in plant sterols (1.0
g/1000 kcal), soy protein (22.5 g/1000 kcal), viscous fibers (10 g/1000 kcal), and
almonds (23 g/1000 kcal). Fifty-five participants completed the 1-y study. The 1-y data were
also compared with published results on 29 of the participants who had also undergone separate 1-mo
metabolic trials of a diet and a statin.
Results: At 3 mo and 1 y, mean (±SE) LDL-cholesterol reductions appeared stable at 14.0 ± 1.6% (P <
0.001) and 12.8 ± 2.0% (P < 0.001), respectively (n = 66). These reductions were less than those
observed after the 1-mo metabolic diet and statin trials. Nevertheless, 31.8% of the participants (n =
21 of 66) had LDL-cholesterol reductions of >20% at 1 y ( ± SE: –29.7 ± 1.6%). The LDL-cholesterol
reductions in this group were not significantly different from those seen after their respective
metabolically controlled portfolio or statin treatments. A correlation was found between total dietary
adherence and LDL-cholesterol change (r = –0.42, P < 0.001). Only 2 of the 26 participants with
<55% compliance achieved LDL-cholesterol reductions >20% at 1 y.
Conclusions: More than 30% of motivated participants who ate the dietary portfolio of
cholesterol-lowering foods under real-world conditions were able to lower LDL-cholesterol
concentrations >20%, which was not significantly different from their response to a first-
generation statin taken under metabolically controlled conditions.

Lowering LDL--Not Only How Low, But How Long?
Background A low plasma level of low-density lipoprotein (LDL) cholesterol is associated with reduced risk of coronary
heart disease (CHD), but the effect of lifelong reductions in plasma LDL cholesterol is not known. We examined
the effect of DNA-sequence variations S. Brown plasma levels of LDL cholesterol on the incidence of coronary

Methods We compared the incidence of CHD (myocardial infarction, fatal CHD, or coronary revascularization) over a
15-year interval in the Atherosclerosis Risk in Communities study according to the presence or absence of
sequence variants in the proprotein convertase subtilisin/kexin type 9 serine protease gene (PCSK9) that are
associated with reduced plasma levels of LDL cholesterol.
Results Of the 3363 black subjects examined, 2.6 percent had nonsense mutations in PCSK9; these mutations were
associated with a 28 percent reduction in mean LDL cholesterol and an 88 percent reduction in the risk of CHD
(P=0.008 for the reduction; hazard ratio, 0.11; 95 percent confidence interval, 0.02 to 0.81; P=0.03). Of the
9524 white subjects examined, 3.2 percent had a sequence variation in PCSK9 that was associated with a 15
percent reduction in LDL cholesterol and a 47 percent reduction in the risk of CHD (hazard ratio, 0.50; 95 percent
confidence interval, 0.32 to 0.79; P=0.003).
Conclusions These data indicate that moderate lifelong reduction in the plasma level of LDL cholesterol is
associated with a substantial reduction in the incidence of coronary events, even in populations with a
high prevalence of non–lipid-related cardiovascular risk factors.

Background— Changes in conventional lipid risk factors with gemfibrozil treatment only partially explain the
reductions in coronary heart disease (CHD) events experienced by men in the Veterans Affairs High-Density
Lipoprotein Intervention Trial (VA-HIT). We examined whether measurement of low-density lipoprotein (LDL) and
high-density lipoprotein (HDL) particle subclasses provides additional information relative to CHD risk reduction.
Methods and Results— This is a prospective nested case-control study of 364 men with a new CHD event (nonfatal
myocardial infarction or cardiac death) during a 5.1-year (median) follow-up and 697 age-matched controls.
Nuclear magnetic resonance (NMR) spectroscopy was used to quantify levels of LDL and HDL particle subclasses
and mean particle sizes in plasma obtained at baseline and after 7 months of treatment with gemfibrozil or
placebo. Odds ratios for a 1-SD increment of each lipoprotein variable were calculated with adjusted logistic
regression models. Gemfibrozil treatment increased LDL size and lowered numbers of LDL particles (–5%) while
raising numbers of HDL particles (10%) and small HDL subclass particles (21%). Concentrations of these LDL and
HDL particles achieved with gemfibrozil were significant, independent predictors of new CHD events. For total LDL
and HDL particles, odds ratios predicting CHD benefit were 1.28 (95% CI, 1.12 to 1.47) and 0.71 (95% CI, 0.61
to 0.81), respectively. Mean LDL and HDL particle sizes were not associated with CHD events.

subclasses, which are not reflected by conventional lipoprotein cholesterol
measures, help to explain the demonstrated benefit of this therapy in
patients with low HDL cholesterol.

Effect of ACAT Inhibition on the Progression of Coronary Atherosclerosis

ABSTRACT
Background The enzyme acyl–coenzyme A:cholesterol acyltransferase (ACAT) esterifies cholesterol in a variety
of tissues. In some animal models, ACAT inhibitors have antiatherosclerotic effects.
Methods We performed intravascular ultrasonography in 408 patients with angiographically documented
coronary disease. All patients received usual care for secondary prevention, including statins, if indicated.
Patients were randomly assigned to receive the ACAT inhibitor pactimibe (100 mg per day) or matching
placebo. Ultrasonography was repeated after 18 months to measure the progression of atherosclerosis.
Results The primary efficacy variable analyzing the progression of atherosclerosis — the change in percent
atheroma volume — was similar in the pactimibe and placebo groups (0.69 percent and 0.59 percent,
respectively; P=0.77). However, both secondary efficacy variables assessed by means of intravascular
ultrasonography showed unfavorable effects of pactimibe treatment. As compared with baseline values, the
normalized total atheroma volume showed significant regression in the placebo group (–5.6 mm3, P=0.001)
but not in the pactimibe group (–1.3 mm3, P=0.39; P=0.03 for the comparison between groups). The
atheroma volume in the most diseased 10-mm subsegment regressed by 3.2 mm3 in the placebo group, as
compared with a decrease of 1.3 mm3 in the pactimibe group (P=0.01). The combined incidence of adverse
cardiovascular outcomes was similar in the two groups (P=0.53).
Conclusions For patients with coronary disease, treatment with an ACAT inhibitor did not improve the primary
efficacy variable (percent atheroma volume) and adversely affected two major secondary efficacy measures
assessed by intravascular ultrasonography. ACAT inhibition is not an effective strategy for limiting

Single-blind, placebo-controlled study to examine the effects of torcetrapib, a potent
inhibitor of CETP, on plasma lipoprotein levels in 19 subjects with low levels of HDL
cholesterol (<40 mg per deciliter [1.0 mmol per liter]), 9 of whom were also treated
with 20 mg of atorvastatin daily. All the subjects received placebo for four weeks and then
received 120 mg of torcetrapib daily for the following four weeks. Six of the subjects who did
not receive atorvastatin also participated in a third phase, in which they received 120 mg of
torcetrapib twice daily for four weeks. RESULTS: Treatment with 120 mg of torcetrapib
daily increased plasma concentrations of HDL cholesterol by 61 percent
(P<0.001) and 46 percent (P=0.001) in the atorvastatin and non-atorvastatin cohorts,
respectively, and treatment with 120 mg twice daily increased HDL
cholesterol by 106 % (P<0.001). Torcetrapib also reduced low-density lipoprotein
(LDL) cholesterol levels by 17 percent in the atorvastatin cohort (P=0.02). Finally, torcetrapib
significantly altered the distribution of cholesterol among HDL and LDL subclasses, resulting in
increases in the mean particle size of HDL and LDL in each cohort. CONCLUSIONS: In subjects
with low HDL cholesterol levels, CETP inhibition with torcetrapib markedly increased HDL
cholesterol levels and also decreased LDL cholesterol levels, both when administered as
monotherapy and when administered in combination with a statin

76. CONCLUSIONES
A pesar de la evidencia, no utilizamos todos
los medios a nuestro alcance para mejorar
el pronóstico de nuestros pacientes, a
nivel práctico somos poco ambiciosos en
nuestros objetivos y creemos más en la
mecánica que en la biología y más en el
tratamiento que en la prevención
