Summary and Explanation
Toxoplasma gondii is an obligate
intracellular parasite capable of infecting
most mammals, including humans. The
organism is transmitted through ingestion
of insufficiently cooked meat. Throughout
the world, 1 to 90 percent of populations
may be infected,

with 25 to 30 percent of
the adult population infected in the United
States.

While toxoplasmosis may be
manifested in several forms, infections are
usually clinically inapparent, and latent
infections usually persist for life.

Overt
clinical symptoms are similar to infectious
mononucleosis, with lymphadenopathy,
fever, headache, malaise and sometimes
pneumonia and myocarditis.

As with other latent infections, acute
toxoplasma infection can pose a serious
threat to immunocompromised individuals
and newborns who acquire the infection in
utero. Immunosuppressed patients may
develop encephalitis, myocarditis or
pneumonitis.

Congenital infections
usually result as a consequence of
asymptomatic acute maternal infection.
This infection can cause premature
delivery, spontaneous abortion or
stillbirth.

Neonates may manifest
chorioretinitis, hydrocephaly,
microcephaly, cerebral calcification and
psychomotor retardation.

The majority of
congenitally infected children will not
exhibit any symptoms until later in life.

Management of toxoplasmosis requires
serological monitoring of infected
individuals,

as the organism is not readily
available for culture. Quantitative testing
for the presence of toxoplasma IgG can be
useful to determine prior infection and
indicate reactivation of the infection.
Accurate diagnostic information is
important, particularly during pregnancy,
as treatment with spiramycin can reduce
the risk to the fetus.

Principle of the Procedure
IMMULITE/IMMULITE 1000 Toxoplasma
Quantitative IgG is a solid-phase,
sequential chemiluminescent
immunoassay.
Incubation Cycles: 2 × 30 minutes.
Specimen Collection
The use of an ultracentrifuge is
recommended to clear lipemic samples.
Samples which are cloudy or have
particulate material should be clarified by
low-speed centrifugation.
Centrifuging serum samples before a
complete clot forms may result in the
presence of fibrin. To prevent erroneous
results due to the presence of fibrin,
ensure that complete clot formation has
taken place prior to centrifugation of
samples. Some samples, particularly
those from patients receiving
anticoagulant therapy, may require
increased clotting time.
Blood collection tubes from different
manufacturers may yield differing values,
depending on materials and additives,
including gel or physical barriers, clot
activators and/or anticoagulants.
IMMULITE/IMMULITE 1000 Toxoplasma
Quantitative IgG has not been tested with
all possible variations of tube types.

Volume Required: 10 µL of the prediluted
patient sample (1-in-21 dilution). (Sample
cup must contain at least 100 µL more
than the total volume required.)
Storage: 3 days at 2–8°C or
6 months at –20°C.

Patient Sample Dilution: Serum samples
must be prediluted 1-in-21 in IgG/IgM
Sample Diluent (LIGZ2). This dilution must
be performed manually e.g. by adding
20 µL pf patient sample to 400 µL of
Sample Diluent (LIGZ2, supplied in the
kit).
Alternatively, the IMMULITE 1000
Windows
®
system performs the dilution on
board, automatically, using LIGW
(provided in the IgG/IgM Sample Diluent
Module, L1KIGW1,5).
Printed results have been corrected for the
1-in-21 dilution.
Warnings and Precautions
For in vitro diagnostic use.
Reagents: Store at 2–8°C. Dispose of in
accordance with applicable laws.
Follow universal precautions, and handle
all components as if capable of
transmitting infectious agents. Source
materials derived from human blood were
tested and found nonreactive for syphilis;
for antibodies to HIV 1 and 2; for hepatitis
B surface antigen; and for antibodies to
hepatitis C.
Sodium azide, at concentrations less than
0.1 g/dL, has been added as a
preservative. On disposal, flush with large
volumes of water to prevent the buildup of
potentially explosive metal azides in lead
and copper plumbing.
Chemiluminescent Substrate: Avoid
contamination and exposure to direct
sunlight. (See insert.)
Water: Use distilled or deionized water.
The bead is coated with inactivated
toxoplasma antigen. However, caution is
advised because of the possible presence
of residual organism when working with, or
disposing of, the materials supplied.
The anti-toxoplasma IgG results
determined for a given specimen with
assays from different manufacturers can
vary due to differences in assay methods
and reagent specificity. Therefore, the
results reported by the laboratory to the
physician should include: "The following
results were obtained with the
IMMULITE/IMMULITE 1000 Toxoplasma
Quantitative IgG EIA. Results obtained
from other manufacturers' assay methods
may not be used interchangeably."
Materials Supplied
Components are a matched set. The
barcode labels are needed for the assay.
Toxoplasma Quantitative IgG Test
Units (LTXP1)
Each barcode-labeled unit contains one
bead coated with inactivated, partially
purified Toxoplasma gondii antigen (RH
strain tachyzoites from mouse
peritoneum). Stable at 2–8°C until
expiration date.
LKTXP1: 100 units. LKTXP5: 500 units.
Allow the Test Unit bags to come to room
temperature before opening. Open by
cutting along the top edge, leaving the
ziplock ridge intact. Reseal the bags to
protect from moisture.
Toxoplasma Quantitative IgG Reagent
Wedges (LTXPA, LTXPB)
With barcodes. LTXPA: 7.5 mL of a
protein-based buffer, with preservative.
LTXPB: 7.5 mL alkaline phosphatase
(bovine calf intestine) conjugated to
monoclonal murine anti-human IgG
antibody in buffer, with preservative. Store
capped and refrigerated: stable at 2–8°C
until expiration date. Recommended
usage is within 30 days after opening
when stored as indicated.

Assay Procedure
Note that for optimal performance, it is
important to perform all routine
maintenance procedures as defined in the
IMMULITE or IMMULITE 1000 Operator's
Manual.

dilutions, adjustment, assay and quality
control procedures.
Visually inspect each Test Unit for the
presence of a bead before loading it onto
the system.
Note that both Reagent Wedges A and B
must be loaded on the carousel to run this
assay. For IMMULITE 1000 Windows
®
,
the Sample Diluent Wedge must also be
loaded on the carousel.
Recommended Adjustment Interval:
4 weeks.
Quality Control Samples: The controls
supplied with the kit should be used as
quality control material to monitor assay
performance.
The negative and positive controls are
intended to monitor for substantial reagent
failure. The positive control provided will
not ensure precision of the cut-off.
In addition to the controls provided, users
may wish to run additional controls to
cover the upper range of the assay.
Additional controls may be tested in
accordance with guidelines or
requirements of local, state and/or federal
regulations or accrediting organizations.

It is also recommended that known
reactive and nonreactive specimens be
run periodically to assure pipetting
accuracy for the dilution step.
Interpretation of Results
IMMULITE/IMMULITE 1000 Toxoplasma
Quantitative IgG assay is standardized in
terms of the World Health Organization's

Third International Standard for Anti-
Toxoplasma Serum. The assay's cutoff is
determined from representative positive
and negative samples (confirmed by an
ELISA method) using the ROC (Receiver
Operating Characteristic) technique.
Positive: A result greater than or equal to
8 IU/mL is indicative of a past infection.
Negative: A result less than 6.5 IU/mL is
considered to be negative. Results less
than 6.5 IU/mL should be reported as
Negative only, and no numerical
concentration should be reported.
Indeterminate: A result greater than or
equal to 6.5 IU/mL and less than 8 IU/mL
is considered indeterminate.
Samples greater than 250 IU/mL should
be reported as "greater than 250 IU/mL".
Specimens reporting a result of greater
than 250 IU/mL, when tested at the
required 1-in-21 dilution, may be further
diluted with the IgG/IgM Sample Diluent.
The presence of IgG antibodies to
Toxoplasma gondii is an indication of
previous exposure to the organism. A
single specimen can be used to estimate
the serological status of the individual.
A result of "indeterminate" indicates an
intermediate level of Toxoplasma gondii
antibodies in the sample: the sample
should therefore be retested. Samples
which still test as "indeterminate" should
be examined by an alternate method, or a
second sample should be collected, if
possible, within a reasonable period of
time (e.g. one week).
Any interpretation of the results is not
recommended if the positive or negative
control falls outside the range specified in
the Quality Control section.
The magnitude of the reported IgG level
cannot be correlated to an endpoint titer.
Reports by the laboratory to the physician
should include: "The following results were
obtained with the IMMULITE/IMMULITE
1000 Toxoplasma Quantitative IgG EIA.
Values obtained from other manufacturers'
assay methods may not be used
interchangeably."
Expected Values
Individuals with the toxoplasma organism
may not exhibit detectable levels of IgG
antibody in the early stages of infection.
Levels of IgG antibody to Toxoplasma
gondii begin to rise one or two weeks after
infection. Peak levels are reached in 6 to 8
weeks, then gradually decline over a
period of months or even years. Low titers
are generally detectable for life. The
antibody titer does not correlate with
severity of illness.

The prevalence of IgG antibody to
Toxoplasma gondii will vary with age and
geographical location. Studies were
conducted with specimens from presumed
healthy, asymptomatic subjects from the
northwestern US. The subjects consisted
of 69 individuals with ages ranging from
15 to 64 years. (Samples had been
previously collected and stored frozen at
–20°C). The prevalence of toxoplasma
reactivity, based on the results from the
IMMULITE Toxoplasma Quantitative IgG
assay, was found to be 56% at this
geographical location.
Additional studies were conducted with
specimens from 42 pregnant women from
the same geographical location, with ages
ranging from 16 to 41 years. (Samples
had been previously collected and stored
frozen at –20°C). The prevalence of
toxoplasma reactivity for this specific
population was 5%, based on the results
from the IMMULITE Toxoplasma
Quantitative IgG assay. (See “Expected
Values” graph.)
Consider these limits as guidelines only.
Each laboratory should establish its own
reference ranges.
Limitations
The use of IMMULITE/IMMULITE 1000
Toxoplasma Quantitative IgG to diagnose
recent infection by testing paired sera has
not been validated.
The results of the test must be taken
within the context of the patient's clinical
history, symptomology and other
laboratory findings.
The presence of IgG antibodies in a single
specimen is not sufficient to distinguish
between active and past infection.
Patients suspected of having a primary or
active toxoplasma infection should be
tested for the presence of IgM antibodies
to Toxoplasma gondii.
For the determination of seroconversion
from nonreactive to reactive, a second

serum sample should be drawn three to
four weeks after the acute, nonreactive
sample, during the convalescent stage of
the infection. The acute phase sample
should be stored and tested in parallel
with the convalescent sample.
Individuals with an acute toxoplasma
infection may not exhibit any detectable
IgG antibodies at the early stage of
infection.
The results in HIV patients, in patients
undergoing immunosuppressive therapy,
or in patients with other disorders leading
to immunosuppression, should be
interpreted with caution.
The performance characteristics of this
assay have not been established for use
with specimens from neonates, cord
blood, pretransplant patients, or body
fluids other than serum, such as urine,
saliva or amniotic fluid.
Heterophilic antibodies in human serum
can react with the immunoglobulins
included in the assay components causing
interference with in vitro immunoassays.

33.] Samples from patients routinely
exposed to animals or animal serum
products can demonstrate this type of
interference potentially causing an
anomalous result. These reagents have
been formulated to minimize the risk of
interference; however, potential
interactions between rare sera and test
components can occur. For diagnostic
purposes, the results obtained from this
assay should always be used in
combination with the clinical examination,
patient medical history, and other findings.

Results are expressed in IU/mL. (Unless
otherwise noted, all were generated on
serum samples collected in tubes without
gel barriers or clot-promoting additives.)
Calibration Range: 5.0 to 250 IU/mL.
Standardized in terms of WHO 3rd IS for
Anti-Toxoplasma Serum, Human.
Analytical Sensitivity: ≤ 5.0 IU/mL.
Precision: Statistics were calculated for
controls and samples processed in 5
replicates each on five separate days.

“Interassay Precision” table.)
Note that the negative control and Sample
1 were consistently below 5 IU/mL (the
lower limit of the assay's calibration range)
for all replicates in every run.
Specificity: The specificity of IMMULITE
Toxoplasma Quantitative IgG assay was
evaluated by testing 50 specimens
positive for cytomegalovirus (CMV),
varicella-zoster virus (VZV), Epstein-Barr
virus (EBV), Mycoplasma pneumoniae
and parvovirus B19. With these
specimens, IMMULITE Toxoplasma
Quantitative IgG and a commercially
available enzyme immunoassay for
Toxoplasma IgG (Kit D) showed 96%
agreement (48/50).
Alternate Sample Type: To assess the
effect of alternate sample types, blood
was collected from 16 volunteers into
plain, heparinized and Becton Dickinson
SST
®
vacutainer tubes. All samples were
assayed by the IMMULITE Toxoplasma
Quantitative IgG procedure, with the
following results.
(Heparin) = 1.02 (Serum) + 0.13 IU/mL
r = 1.00
(SST) = 0.98 (Plain Tubes) – 0.06 IU/mL
r = 1.00

In another experiment, blood was
collected from 16 laboratory volunteers
into plain and EDTA and vacutainer tubes.
All samples were assayed by the
IMMULITE Toxoplasma Quantitative IgG
procedure, with the following results.

Clinical Performance
The clinical performance of the IMMULITE
Toxoplasma Quantitative IgG assay was
studied using a total of 199 serum
specimens, assayed at each of three sites,
two of which were outside the US. Of the
199 specimens (previously collected and
stored frozen at –20°C), 61 were obtained

from women who presented for obstetric
and gynecological examination, 42 were
obtained from pregnant women, and the
remainder were obtained from patients
with miscellaneous diseases and
conditions (AIDS, anemia, arthritis,
fatigue, headache, lymphoma, meningitis,
pneumonia and others). There were 37
male and 162 female subjects, with ages
ranging from 15 to 68 years.
One of the retrospective studies was
conducted at a university medical center in
the northwestern US. The 199 specimens
were evaluated nonconcurrently with the
IMMULITE Toxoplasma Quantitative IgG
procedure and Kit A, a commercially
available Toxoplasma IgG enzyme-linked
fluorescent immunoassay (ELFA).

The two discordant specimens, negative
by IMMULITE but positive by Kit A, were
further evaluated by Kit B, a commercially
available Toxoplasma IgG microparticle
enzyme immunoassay (MEIA), and found
to be positive.
In the second retrospective study,
conducted outside the US, 194 of the 199
specimens (insufficient sample volume for
5 specimens) were evaluated with the
IMMULITE Toxoplasma Quantitative IgG
assay and Kit B.

Of the seven discordant specimens, three
of the five negative by IMMULITE but
positive by Kit B were negative by Kit A.
The two specimens positive by IMMULITE
but negative by Kit B were positive by
Kit A.
In the third retrospective study, also
conducted outside the US, 160 of the 199
specimens (insufficient sample volume for
39 specimens) were evaluated with the
IMMULITE Toxoplasma Quantitative IgG
assay and Kit C, a commercially available
enzyme immunoassay.

The two cases positive by the IMMULITE
but negative by Kit C, were positive by
Kit A. Of the two cases negative by
IMMULITE but positive by Kit C, one was
positive by Kit A and the other negative by
Kit A. The four cases negative by
IMMULITE but equivocal by Kit C were all
negative by Kit A. Thus, the IMMULITE
results agree with Kit A in seven of the
eight cases of discordance with Kit C.

Klinische Relevanz
Toxoplasma gondii ist ein obligater
intrazellulärer Parasit, der die meisten
Säugetiere – einschließlich Menschen –
befällt und zu Infektionen führt. Der
Erreger wird durch den Verzehr von
ungenügend gekochtem Fleisch
übertragen. Weltweit sind möglicherweise
zwischen 1 und 90 Prozent aller
Populationen infiziert

, in den USA sind es
25–30 % der erwachsenen Bevölkerung.

Toxoplasmosen können sich in
verschiedenen Formen manifestieren,
normalerweise sind die Infektionen jedoch
klinisch unauffällig und in ihrer latenten
Form lebenslang präsent.

Zu den offenen
klinischen Symptomen zählen infektiöse
Mononukleosen mit Lymphadenopathien,
Fieber, Kopfschmerzen, Unwohlsein und
gelegentlich auch Lungenentzündungen
und Myokarditiden.

So wie andere latente Infektionen können
auch akute Toxoplasmosen bei
immungeschwächten Patienten und in
utero infizierten Neugeborenen eine
ernsthafte Bedrohung darstellen. Bei
immunsupprimierten Patienten können
Enzephalitis, Myokarditis oder
Pneumonitis entstehen.

Kongenitale
Infektionen sind normalerweise das
Resultat einer asymptomatischen akuten
Infektion der Mutter. Diese Infektionen
könen zu Frühgeburten, Spontanaborten
oder Totgeburten führen.

Bei
Neugeborenen können Chorioretinitiden,
Hydrozephalie, Mikrozephalie, zerebrale
Kalkablagerungen sowie
psychomotorische Behinderungen
auftreten.

Die Mehrzahl der kongenital
infizierten Kinder zeigt erst im späteren
Leben Symptome.

Da der Erreger von Toxoplasmosen nicht
ohne weiteres kultiviert werden kann,
müssen die infizierten Patienten
serologisch überwacht werden.

Quantitative Bestimmungen von
Toxoplasma-IgG können nützlich sein, um
frühere und reaktivierte Infektionen
anzuzeigen. Genaue diagnostische Daten
sind vor allem bei Schwangerschaften
wichtig, da eine Behandlung mit
Spiramycin das Risiko für den Fetus
herabsetzen kann.

Methodik
Der IMMULITE/IMMULITE 1000
Toxoplasma IgG quantitativ ist ein
Festphasen-, sequenzieller
Chemilumineszens-Immunoassay.
Inkubationszyklen: 2 × 30 minuten.
Probengewinnung
Der Einsatz einer Ultrazentrifuge wird zur
Klärung von lipämischen Proben
empfohlen.

Trübe oder partikelhaltige Proben sollten
bei niedriger Geschwindigkeit zentrifugiert
werden, bis sie klar sind
Die Zentrifugation der Serumproben vor
dem völligen Abschluss der Gerinnung
kann zu Fibringerinnseln führen. Um
fehlerhaften Analysenergebnissen infolge
von Gerinnseln vorzubeugen, ist
sicherzustellen, dass die Gerinnung vor
der Zentrifugation der Proben vollständig
abgeschlossen ist. Insbesondere Proben
von Patienten unter Antikoagulantien-
therapie können eine verlängerte
Gerinnungszeit aufweisen.
Blutentnahmeröhrchen von verschiedenen
Herstellern können differierende Werte
verursachen. Dies hängt von den
verwendeten Materialien und Additiven
(Gel oder physische Trennbarrieren,
Gerinnungsaktivatoren und /oder
Antikoagulantien) ab.
IMMULITE/IMMULITE 1000 Toxoplasma
IgG quantitativ sind nicht mit allen
möglichen Röhrchenvariationen
ausgetestet worden. Details der
getesteten Röhrchenarten sind dem
Kapitel "Alternative Probenarten" zu
entnehmen.
Erforderliche Menge: 10 µl der
vorverdünnten Patientenprobe (1:21
Verdünnen). (Inhalt der Probenschale
muss mindestens 100 µl über der
erforderlichen Gesamtmenge liegen.)
Lagerung: 3 Tage bei 2–8°C oder
6 Monate bei –20°C.

Verdünnung der Patientenproben:
Serumproben müssen 1:21 im IgG/IgM
Probenverdünnungsreagenz (LIGZ2)
vorverdünnt werden. Diese Verdünnung
ist manuell durchzuführen, z.B. durch
Zugabe von 20 µl der Patientenprobe in
400 µl Probenverdünnungsreagenz
(LIGZ2, im Kit enthalten).
Alternativ, bei Verwendung des IMMULITE
1000 Windows, erfolgt die Verdünnung
automatisch on-board. Das System
verwendet LIGW, Inhalt des IgG/IgM
Probenverdünnungsreagenz-Moduls
(L1KIGW1,5).
Die ausgedruckten Resultate
entsprechend der 1:21 Verdünnung
korrigiert sind.
Hinweise und
Vorsichtsmaßnahmen
Zur In-vitro-Diagnostik.
Reagenzien: Bei 2–8°C lagern. Unter
Einhaltung der geltenden gesetzlichen
Vorschriften entsorgen.
Die generell geltenden
Vorsichtsmaßnahmen sind einzuhalten
und alle Komponenten als potenziell
infektiös zu behandeln. Alle aus
menschlichem Blut gewonnenen
Materialien wurden auf Syphilis,
Antikörper gegen HIV-1 und HIV-2,
Hepatitis-B-Oberflächenantigen und
Hepatitis-C-Antikörper untersucht und
negativ befundet.
Bestimmten Komponenten wurde
Natriumazid (<0,1 g/dl) hinzugefügt. Um
die Bildung von explosiven Metallaziden in
Blei- und Kupferrohren zu vermeiden,
sollten die Reagenzien nur zusammen mit
großen Wassermengen in die Kanalisation
gespült werden.
Chemilumineszenz-Substrat:
Kontamination und direkte
Sonneneinstrahlung vermeiden. Siehe
Packungsbeilage.
Wasser: Destilliertes oder deionisiertes
Wasser verwenden.
Die Phase ist mit inaktiviertem
Toxoplasma-Antigen beschichtet. Wegen
des potenziellen Vorhandenseins
überlebender Mikroorganismen ist jedoch
beim Arbeiten mit und beim Entsorgen von
den im Lieferumfang enthaltenen
Materialien Vorsicht geboten.
Unterschiede in der jeweiligen Methodik
oder der Spezifität der Reagenzien
könnten jedoch dazu führen, das die mit
Testsystemen von verschiedenen
Herstellern ermittelten Anti-
Toxoplasmose-IgG-Ergebnisse für
dieselben Proben nicht einheitlich sind.
Die vom Labor an den Arzt
weitergegebenen Ergebnisse sollten
daher den folgenden Passus enthalten:
„Die folgenden Ergebnisse wurden mit
dem IMMULITE/IMMULITE 1000-
Testsystem zur quantitativen Bestimmung
von Toxoplasmose-IgG erzielt." Sie sind
nicht mit den Ergebnissen der
Testsysteme anderer Hersteller
austauschbar.

Testdurchführung
Für eine optimale Funktion des Gerätes ist
unbedingt zu beachten, dass die
Wartungen, wie im IMMULITE oder
IMMULITE 1000-Handbuch beschrieben,
regelmäßig durchgeführt werden.
Das Handbuch für das IMMULITE bzw.
IMMULITE 1000 enthält die Anweisungen
für: Vorbereitung, Geräteeinstellungen,
Verdünnungen, Kalibrierung,
Testdurchführung und Qualitätskontrollen.
Überprüfen Sie jedes Testeinheit auf das
Vorhandensein der Polystyrol-Kugel vor
dem Einsetzen in das Gerät.
Beachten Sie, dass zur Durchführung
dieses Tests beide Reagenzbehälter
(A und B) auf das Karussell geladen
werden müssen. Beim IMMULITE 1000
Windows muss der
Probenverdünnungsreagenz-Behälter
ebenfalls auf dem Reagenzienkarussell
platziert sein.
Empfohlenes Kalibrationsintervall:
4 Wochen.
Proben zur Qualitätskontrolle: Die im
Lieferumfang enthaltenen Kontrolle(n)
verwenden.
Die Negativ- und Positivkontrollen dienen
zur Überwachung der Reagenzien auf
wesentliche Mängel. Die gelieferte
Positivkontrolle eignet sich nicht dazu, die
Präzision des Cutoff sicherzustellen.
Zusätzlich zu den gelieferten Kontrollen
können zur Abdeckung des höheren
Messbereiches weitere Kontrollen
eingesetzt werden. Entsprechend den
Richtlinien bzw. Vorschriften von
regionalen bzw. staatlichen Behörden
oder zertifizierenden Stellen können
weitere Kontrollen getestet werden.

Ferner empfiehlt es sich, regelmäßig
Testansätze mit bekannten reaktiven und
nichtreaktiven Proben durchzuführen, um
die Pipettiergenauigkeit für den
Verdünnungsschritt zu gewährleisten.
Interpretation der Ergebnisse
Das quantitative IMMULITE/IMMULITE
1000-Testsystem zur quantitativen
Bestimmung von Toxoplasma-IgG ist
entsprechend der Dritten Internationalen
WHO-Norm für Anti-Toxoplasma-Serum
standardisiert. Der Cutoff des Testsystems
wird mit Hilfe der ROC-Technik (Receiver
Operating Characteristic) aus
repräsentativen positiven und negativen
Proben (verifiziert mittels ELISA) ermittelt.
Positiv: Ein Ergebnis von ≥ 8 IU/ml weist
auf eine vergangene Infektion hin.
Negativ: Ein Ergebnis von unter 6,5 IU/ml
gilt als negativ. Ergebnisse von unter
6,5 IU/ml sollten lediglich als negativ
ausgewiesen werden, ohne dabei die
gemessene Konzentration numerisch
darzustellen.
Grenzwertig: Ein Ergebnis von
≥ 6,5 IU/ml und < 8 IU/ml gilt als
grenzwertig.
Proben mit mehr als 250 IU/ml sollten
dementsprechend (d.h. „höher als
250 IU/ml“) ausgewiesen werden. Proben,
die bei der erforderlichen Autoverdünnung
von 1:21 mehr als 250 IU/ml aufweisen,
können mit dem IgG/IgM-
Probenverdünner weiterverdünnt werden.
Sind IgG-Antikörper gegen Toxoplasma
gondii vorhanden, so deutet dies auf eine
frühere Exposition gegen den Erreger hin.
Einzelproben eignen sich ausschließlich
zur Bewertung des serologischen Status
der betreffenden Person.
Ein „indeterminate“ // „unentschiedenes“
Ergebnis weist auf einen Zwischenwert bei
den Antikörpern gegen Toxoplasma gondii
in der Probe hin: Die Probe sollte daher
erneut getestet werden. Ergebnis bringen,
sind entweder mit einer alternativen
Methode zu testen, oder, sofern dies
möglich ist, innerhalb eines vernünftigen
Zeitraums (z.B. eine Woche) mit einer neu
entnommenen zweiten Probe zu
ergänzen.
Von einer Interpretation der Ergebnisse
wird abgeraten, wenn die Positiv- oder
Negativkontrollen außerhalb des im
Abschnitt „Quality Control“ //

„Qualitätskontrolle“ angegebenen
Bereichs liegen.
Die Größenordnung der ausgewiesenen
IgG-Konzentration kann nicht mit einem
Endtiter korreliert werden.
Die vom Labor an den Arzt
weitergegebenen Ergebnisse sollten
daher den folgenden Passus enthalten:
„Die folgenden Ergebnisse wurden mit
dem IMMULITE/IMMULITE 1000-
Testsystem zur quantitativen Bestimmung
von Toxoplasmose-IgG erzielt." Sie sind
nicht mit den Ergebnissen der
Testsysteme anderer Hersteller
austauschbar.
Referenzwerte
Im Frühstadium einer Infektion mit dem
Toxoplasma-Erreger sind möglicherweise
keine nachweisbaren Konzentrationen an
IgG-Antikörpern vorhanden. Die
Konzentrationen an IgG-Antikörpern
gegen Toxoplasma gondii steigen
erstmals ein bis zwei Wochen nach der
Primärinfektion an. Die Spitzenwerte
werden innerhalb von 6–8 Wochen
erreicht und fallen dann über Monate oder
sogar Jahre hinaus allmählich wieder ab.
Niedrige Titer bleiben generell für den
Rest des Lebens nachweisbar. Es besteht
keine Korrelation zwischen Antikörpertiter
und dem Schweregrad der Erkrankung.

Die Prävalenz von IgG-Antikörpern für
Toxoplasma gondii kann abhängig vom
Alter und der geographischen Lage
variieren. Es wurden mit gesunden,
asymptomatischen Patienten aus dem
Nordwesten der USA Studien
durchgeführt. Die Patienten setzten sich
aus 69 Personen im Alter zwischen 15
und 64 Jahren zusammen. (Die Proben
wurden vorher gesammelt und bei –20°C
eingefroren gelagert). In dieser
geographischen Gegend wurde eine
Prävalenz der Toxoplasmosereaktivität
von 56% gefunden.
Weiterhin wurden Studien mit
Patientenseren von schwangeren Frauen
im Alter zwischen 16 und 41 Jahren aus
der gleichen geographischen Region
durchgeführt. (Die Proben wurden vorher
gesammelt und bei –20°C eingefroren
gelagert). Die Prävalenz der
Toxoplasmosereaktivität dieser speziellen
Bevölkerungsgruppe lag bei 5%,
basierend auf den Resultaten des
IMMULITE Toxoplasma quantitativem IgG
Test. (siehe grafik „Grenzen der
Methode“)
Betrachten Sie diese Grenzwerte nur als
Richtlinien. Jedes Labor sollte eigene
Referenzbereiche ermitteln.
Grenzen der Methode
Der Einsatz des IMMULITE/IMMULITE
1000 Testsystems zur quantitativen
Bestimmung von Toxoplasma-IgG ist nicht
validiert, was die Diagnose kürzlicher
Infektionen mittels gepaarten Seren
betrifft.
Die Testergebnisse sind vor dem
Hintergrund der klinischen Anamnese, der
Beschwerden des Patienten sowie
weiterer Laborbefunde zu bewerten.
Das Vorhandensein von IgG-Antikörpern
in Einzelproben reicht nicht aus, um
zwischen einer aktiven und einer
vergangenen Infektion zu unterscheiden.
Patienten mit Verdacht auf eine Primär-
bzw. aktive Infektion sollten auf das
Vorhandensein von IgM-Antikörpern
gegen das Toxoplasma gondii untersucht
werden.
Zur Bestimmung einer Serokonversion
von nichtreaktiv zu reaktiv sollte während
der Rekonvaleszenzphase drei bis vier
Wochen nach der akuten nichtreaktiven
Probe eine zweite Serumprobe
genommen werden. Die Probe aus der
Akutphase sollte gelagert und parallel zur
Rekonvaleszenzprobe getestet werden.
Die Ergebnisse bei HIV-Patienten sowie
behandlungs- oder krankheitsbedingt
immunsupprimierten Patienten sollten
zurückhaltend interpretiert werden.
Im Frühstadium einer akuten
Toxoplasmose sind möglicherweise keine
nachweisbaren IgG-Antikörper vorhanden.
Die Eckdaten dieses Testsystems wurden
nicht für den Gebrauch mit Proben von
Neugeborenen, Nabelschnurblut,
Patienten mit bevorstehender

Spezifität: Die Spezifität des IMMULITE
Toxoplasma Quantitativ IgG Tests wurde
mit 50 Proben, die positiv für Cytomegalie
(CMV), Varizelle zoster (VZV), Epstein-
Barr-Virus (EBV), Mycoplasma
pneumoniae und Parvovirus B19 waren,
evaluiert. Diese Proben zeigten mit dem
IMMULITE Toxoplasma Quantitativ IgG
Test eine 96%ige Übereinstimmung
(48/50) mit einem kommerziell erhältlichen
Toxoplasma IgG Enzymimmunotest.
Alternativer Probentyp: Um die
Auswirkungen von verschiedenen
Probenarten zu untersuchen, wurde Blut
von 16 Freiwilligen in Röhrchen ohne
Additiva, in Heparin- und Becton
Dickinson SST
®
Vacutainer-Rörchen
gesammelt. Alle Proben wurden mit dem
Toxoplasma IgG Verfahren für den
IMMULITE getestet, mit der folgenden

Klinische Studien
Die klinischen Leistungsdaten des
IMMULITE Toxoplasma Quantitativ IgG
Test wurdem mit 199 Serumproben aus
drei verschiedenen Regionen, von denen
zwei außerhalb der USA stammten,
geprüft. Die 199 Patientenproben (die
Proben wurden vorher gesammelt und bei
–20°C eingefroren gelagert) wurden von
Frauen erhalten, die aus derzeitigen
Entbindungs- und gynäkologischen
Untersuchungen stammten. 42 Proben
wurden von schwangeren Frauen
erhalten, und die restlichen Proben
stammten von Patienten mit
unterschiedlichen Krankheiten und
Gegebenheiten (AIDS, Anämie, Arthritis,
Müdigkeitssyndrom, Kopfschmerzen,
Lymphom, Meningitis, Lungenentzündung
und andere Krankheiten). Es handelte
sich um 37 männliche und 162 weibliche
Patienten im Alter von 15 bis 68 Jahren.
Eine der retrospektiven Studien wurde an
einer Medizinischen Universität im
Nordwesten der USA durchgeführt. Die
199 Blutproben wurden mit dem
IMMULITE Toxoplasma Quantitativ IgG
Test und einem kommerziell erhältlichen

Probenvolumen) mit dem IMMULITE
Toxoplasma IgG-Test und einem
kommerziell erhältlichen
Enzymimmunoassay Kit C, evaluiert.

Fällen mit negativem IMMULITE und
grenzwertigen Ergebnis in Kit C, waren
alle Proben negativ in Kit A. Demnach
stimmen die IMMULITE Ergebnisse in
sieben von acht Fällen mit Kit A überein,
die diskrepant im Kit C liefen.

Resumen y Explicación
del Test
Toxoplasma gondii es un parásito
intracelular obligado, capaz de infectar a
la mayoría de los mamíferos, incluyendo
al humano. El organismo se transmite por
ingestión de carne poco cocinada. Por
todo el mundo, del 1 al 90% de la
población puede ser infectada,

con un 25
– 30 por ciento de la población adulta de
Estados Unidos infectada.

Mientras que
la toxoplasmosis puede manifestarse de
diversas formas, las infecciones son
normalmente clínicamente inaparentes, y
las infecciones latentes persisten
normalmente durante toda la vida.

Los
síntomas clínicos son similares a los de
una infección por mononucleosis, con
linfadenopatía, fiebre, dolor de cabeza,
malestar general y algunas veces
neumomía y miocarditis.

Al igual que otras infecciones latentes, la

información precisa del diagnóstico es

espiramicina puede reducir los riesgos
para el feto.

Principio del análisis
IMMULITE/IMMULITE 1000 Toxoplasma
Cuantitativa IgG es un inmunoanálisis
quimioluminiscente en fase sólida
secuencial.
Ciclos de incubación: 2 × 30 minutos.
Recogida de la muestra
Se recomienda el uso de una
ultracentrífuga para aclarar las muestras
lipémicas.
Las muestras que estén turbias o tengan
un material particular deben aclararse
mediante una centrifugación a baja
velocidad.
La centrifugacion de las muestras de
suero antes de que se forme el coagulo
puede ocasionar la presencia de fibrina.
Para evitar resultados erroneos debidos a
la presencia de fibrina, asegurarse que se
ha formado el coagulo completamente
antes de centrifugar las muestras.
Algunas muestras, particularmente
aquellas de pacientes sometidos a terapia
anticoagulante, pueden requerir mayor
tiempo de coagulacion.
Los tubos para recoger sangre de
distintos fabricantes pueden producir
valores diferentes, dependiendo del
material del tubo y de los aditivos,
incluyendo barreras de gel o barreras
físicas, activadores de la coagulación y/o
anticoagulantes. El IgG Cuantitativo para
Toxoplasma IMMULITE/IMMULITE 1000
no ha sido analizado con todos los
distintos tipos de tubos. Para obtener

Volumen requerido: 10 µl de la muestra
de paciente prediluida (1 por 21 dilución).
(El recipiente de la muestre debe
contener, como mínimo, 100 µl más que
el volumen total requerido).
Conservación: 3 días a 2–8°C, o
6 meses a –20°C.

Dilución de Muestras de Pacientes: Las
muestras de suero deben ser prediluídas
1-en-21 con Diluyente de Muestra IgG/IgM
(LIGZ2). Esta dilución debe ser realizada
manualmente, p.e. añadiendo 20 µl de
muestra del paciente con 400 µl de
Diluyente de Muestra (LIGZ2,
suministrados en el kit).

Como alternativa, utilizando IMMULITE
1000 Windows
®
, la dilución la realiza el
sistema automaticamente en el equipo
usando LIGW, incluido en el Modulo de
Diluyente de Muestra IgG/IgM
(L1KIGW 1,5).
Los resultados imprimidos han sido
corregidos para una dilución de 1-en-21.
Advertencias y Precauciones
Para uso diagnóstico in vitro.
Reactivos: Mantener a 2–8°C. Desechar
de acuerdo con las normas aplicables.
Siga las precauciones universales y
manipule todos los componentes como si
fueran capaces de transmitir agentes
infecciosos. Los materiales derivados de
sangre humana han sido analizados y son
negativos para sífilis; para anticuerpos
frente al HIV 1 y 2; para el antígeno de
superficie de hepatitis B y para los
anticuerpos de hepatitis C.
Se ha usado Azida sodica, en
concentraciones menores de 0,1 g/dl,
como conservante. Para su eliminacion,
lavar con grandes cantidades de agua
para evitar la constitucion de residuos de
azidas metalicas, potencialmente
explosivas, en las cañerías de cobre y
plomo.
Sustrato quimioluminiscente: evite la
contaminación y exposición a la luz
directa del sol. (Ver el prospecto.)
Agua: Usar agua destilada o desionizada.
La bola está recubierta con antígeno
inactivado de toxoplasma. Sin embargo,
se recomienda precaución ante la posible
presencia de organismo residual cuando
trabajamos o deshechamos los materiales
suministrados.
La concentración de IgG contra
Toxoplasma en una muestra dada,
determinada mediante ensayos de
distintos fabricantes, puede variar debido
a diferencias en los métodos de los
ensayos y a la especificidad del reactivo.
Por lo tanto, en el informe de resultados
del laboratorio para el médico debe
incluirse: "Los siguientes resultados se
obtuvieron con EIA IgG Cuantitativo
contra Toxoplasma IMMULITE/IMMULITE
1000. No es posible intercambiar los
resultados obtenidos por métodos de
ensayo de otros fabricantes".

del valor de corte.
Además de los controles provistos, los
usuarios pueden desear correr controles
adicionales para cubrir el intervalo
superior del ensayo. Pueden analizarse

Interpretación de los
Resultados
El ensayo IgG Cuantitativo para
Toxoplasma IMMULITE/IMMULITE 1000
está estandarizado en términos del Tercer
Estándar Internacional para Suero de
Anti-Toxoplasma de la Organización
Mundial de la Salud. El valor de corte del
ensayo se determinó utilizando muestras
positivas y negativas representativas
(confirmadas por el método ELISA)
utilizando la técnica ROC (Curva de
rendimiento diagnóstico).
Positivo: Un resultado superior o igual a
8 IU/ml es indicativo de una infección
pasada.
Negativo: Un resultado inferior a
6,5 IU/ml es considerado negativo.
Resultados inferiores a 6,5 IU/ml deben
comunicarse solamente como Negativos,
y no debe comunicarse la concentración.
Indeterminado: Un resultado superior o
igual a 6,5 IU/ml e inferior a 8 IU/ml se
considera indeterminado.
Las muestras con valores superiores a
250 IU/ml deben comunicarse como
“superiores a 250 IU/ml". Las muestras
que dieron resultados superiores a
250 IU/ml, cuando se examinan con la
autodilución necesaria 1-en-21, pueden
necesitar diluciones adicionales con el
diluyente para muestras IgG/IgM.
La presencia de anticuerpos IgG contra
Toxoplasma gondii es indicativo de una
exposición previa al organismo. Puede
utilizarse sólo una única muestra para
estimar el estado serológico del individuo.
Un resultado “indeterminado” indica un
nivel intermedio de anticuerpos contra
Toxoplasma gondii en la muestra: la
muestra debe volverse a analizarse. Las
muestras que vuelvan a dar un resultado
“indeterminado” deben examinarse por un
método alternativo, o debe recogerse una
segunda muestra, si es posible, en un
plazo de tiempo razonable (p.e. una
semana).
No se recomienda interpretar los
resultados si el control positivo o negativo
cae fuera del margen especificado en la
Sección Control de Calidad.
La magnitud del nivel de IgG comunicado
no puede relacionarse con un título final.
Por lo tanto, en el informe de resultados
del laboratorio para el médico debe
incluirse: "Los siguientes resultados se
obtuvieron con EIA IgG Cuantitativo
contra Toxoplasma IMMULITE/IMMULITE
1000." No es posible intercambiar los
resultados obtenidos por métodos de
ensayo de otros fabricantes.
Valores Esperados
Individuos con el organismo toxoplasma
pueden no mostrar niveles de anticuerpo
IgG detectables en las fases tempranas
de la infección. Los niveles de IgG para
Toxoplasma gondii empiezan a elevarse 1
o 2 semanas después de la infección. Los
niveles máximos se alcanzan en 6 – 8
semanas, después descienden
gradualmente en los meses y años
posteriores. Se detectan títulos bajos
generalmente durante toda la vida. El
título de anticuerpo no guarda relación
con la severidad de la enfermedad.

La preponderancia de anticuerpos IgG
anti-Toxoplasma gondii variará con la
edad y la zona geográfica. Se condujeron
estudios con muestras tomadas de
sujetos presumiblemente sanos y
asintomáticos del noroeste de los EE.UU.
Este grupo de personas se compuso de
69 individuos de 15 a 64 años de edad.
(Las muestras fueron previamente
recolectadas y guardadas congeladas a –
20ºC). Se encontró que la preponderancia
de reactividad al toxoplasma, basada en
los resultados del ensayo cuantitativo IgG
anti -Toxoplasma IMMULITE, fue de 56%
en esta zona geográfica.
Se realizaron estudios adicionales con
muestras provenientes de 42 mujeres
embarazadas de 16 a 41 años de edad de
la misma zona geográfica. (Las muestras
fueron previamente recolectadas y
guardadas congeladas a –20º C). La
preponderancia de reactividad al
toxoplasma para esta población
específica fue del 5%, basándose en los
resultados del ensayo cuantitativo IgG anti
-Toxoplasma IMMULITE. (Ver el gráfico
de “Expected Values”),
Estos límites han de considerarse sólo
como una guía. Cada Laboratorio deberá
establecer sus propios rangos de
referencia.

Limitaciones
El uso de IgG Cuantitativo Toxoplasma
IMMULITE/IMMULITE 1000 para
diagnosticar una infección reciente
analizando parejas de sueros no se ha
validado.
Los resultados del análisis deben
contemplarse en el contexto del historial
clínico de los pacientes, de su
sintomatología y de los demás hallazgos
del laboratorio.
La presencia de anticuerpos IgG en una
sola muestra no es suficiente para
distinguir entre una infección activa y una
antigua. Los pacientes de los que se
sospeche que padecen una infección
primaria o activa deben analizarse para
detectar la presencia de anticuerpos IgM
para Toxoplasma gondii.
Para determinar la seroconversión de no
reactivo a reactivo, debe recogerse una
segunda muestra de suero tres o cuatro
semanas después de la infección,
muestra no reactiva, durante la etapa de
convalecencia de la infección. La muestra
de la fase aguda debe almacenarse y
analizarse paralelamente a la muestra de
la fase de convalecencia.
Los individuos con infección aguda por
toxoplasma puede que no presenten
niveles detectables de anticuerpos IgG en
la fase temprana de la infección.
Los resultados en pacientes infectados
con el virus VIH, pacientes bajo una
terapia inmunosupresiva, o en pacientes
con otros desórdenes que causen
inmunosupresión, deben interpretarse con
precaución.
No se han establecido las características
de funcionamiento del ensayo para
utilizarlo con muestras de neonatos,
sangre del cordón umbilical, pacientes
pretrasplante, u otros fluidos distintos del
suero, tales como orina, saliva o líquido
amniótico.
Los anticuerpos heterofílicos en el suero
humano pueden reaccionar con las
inmunoglobulinas de los componentes del
ensayo provocando interferencias con los
inmunoanálisis in vitro. [Ver Boscato LM,
Stuart MC. Heterophilic antibodies: a
problem for all immunoassays. Clin Chem
1988:34:27-33.] Las muestras de los
pacientes que frecuentemente están
expuestos a animales o a productos
séricos animales pueden presentar este
tipo de interferencia que potencialmente
ocasione un resultado anómalo. Estos
reactivos han sido formulados para
minimizar el riesgo de interferencia, no
obstante, pueden darse interacciones
anómalas entre sueros conflictivos y los
componentes del ensayo. Con fines de
diagnóstico, los resultados obtenidos con
este ensayo siempre deben ser usados en
combinación con el examen clínico, la
historia médica del paciente y cualquier
otro dato clínico relevante.

(48/50).

Rendimiento Clínico
Se estudió el rendimiento clínico del
ensayo cuantitativo IgG anti-Toxoplasma
IMMULITE, usando un total de 199
muestras de suero. Cada una de estas
muestras fue analizada en tres sitios, dos
de los cuales se encuentran fuera de los
EE.UU. De las 199 muestras (previamente
recolectadas y guardadas congeladas a
–20°C ), 61 se obtuvieron de mujeres que
se presentaron para un examen obstétrico
y ginecológico, 42 se obtuvieron de
mujeres embarazadas y el resto se obtuvo
de pacientes con distintas enfermedades
y afecciones (SIDA, anemia, arteritis,
fatiga, dolor de cabeza, linfoma,
meningitis, neumonía y otros). El grupo
consistió de 37 hombres y 162 mujeres de
15 a 68 años de edad.
Uno de los estudios retrospectivos fue
conducido en el centro médico de una
universidad en el noroeste de los EE.UU.
Las 199 muestras fueron evaluadas en
forma no simultánea con el procedimiento
cuantitativo IgG anti-Toxoplasma
IMMULITE y el Kit A, un ensayo de
inmunofluorescencia ligado a una enzima
para IgG anti -Toxoplasma (ELFA)
comercialmente disponible.

199 muestras (39 muestras tuvieron
insuficiente volumen de muestra) fueron
analizadas con el ensayo cuantitativo IgG
anti-Toxoplasma IMMULITE y el Kit C, un
inmunoensayo enzimático comercialmente
disponible.

Introduction
Toxoplasma gondii est un parasite
intracellulaire capable d'infecter la plupart
des mammifères, y compris l'homme. Le
parasite se transmet par l'ingestion de
viande insuffisamment cuite. Dans le
monde, 1 à 90 % des populations est
susceptible d'être infecté,

25 à 30 % de la
population adulte étant infecté au Etats-
Unis.

Pour la France, 30 à 60 % de la
population est infectée. Même si la
toxoplasmose peut se manifester sous
plusieurs formes, les individus infectés
restent généralement asymptomatiques et
une infection latente persiste
habituellement toute la vie.

Les
symptômes cliniques visibles sont
similaires à ceux de la mononucléose
infectieuse, avec une lymphadénopathie,
de la fièvre, des céphalées, une sensation
de malaise et, parfois, une pneumonie et
une myocardite.

Comme toute infection latente, une
infection aiguë par Toxoplasma gondii
peut représenter un risque sérieux pour
un individu immunodéprimé ou pour un
nouveau-né infecté in utero. Les patients
immunodéprimés infectés sont
susceptibles de développer une
encéphalite, une myocardite ou une
pneumopathie inflammatoire.

Les
infections congénitales proviennent
habituellement d'une infection maternelle
aiguë asymptomatique. Cette infection
peut se traduire par une naissance
prématurée, un avortement spontané ou
la naissance d'un enfant mort-né.

Le
nouveau-né peut présenter une
choriorétinite, une hydrocéphalie, une
microcéphalie, des calcifications intra-
cérébrales et un retard psychomoteur.

plupart des enfants infectés in utero sont
asymptomatiques dans les premiers

Dans la mesure où le parasite n'est pas
facile à mettre en culture, la prise en
charge de la toxoplasmose demande un
contrôle sérologique régulier des individus
infectés.

Un dosage quantitatif d'IgG

gondii peut être utile pour mettre en
évidence une infection antérieure et
révéler une réactivation de l'infection. Un
diagnostic précis est important, en
particulier au cours de la grossesse,
puisqu'un traitement par la spiramycine
pourra réduire les risques encourus par le
fœtus.

Principe du test
IMMULITE/IMMULITE 1000
Toxoplasmose IgG quantitatif est un
immunodosage chimiluminescent
séquentiel, en phase solide.
Cycles d'incubation : 2 × 30 minutes.
Recueil des échantillons
Il est recommandé de clarifier les
échantillons hyperlipémiques par
ultracentrifugation.
Les échantillons troubles ou présentant
des particules en suspension devront être
clarifiés par centrifugation à vitesse
réduite.
La centrifugation des échantillons
sériques avant la formation complète du
caillot peut entraîner la présence de
fibrine. Pour éviter les résultats erronés
dûs à la présence de fibrine, s'assurer de
la formation complète du caillot avant de
centrifuger les échantillons. Certains
échantillons, en particulier ceux provenant
de patients sous anti-coagulants, peuvent
nécessiter un temps plus long pour la
formation du caillot.
Des tubes pour prélèvements sanguins
provenant de fabricants différents peuvent
donner des résultats différents, selon les
matériaux et additifs utilisés, y compris
gels ou barrières physiques, activateurs
de la coagulation et/ou anticoagulants. Le
coffret Toxoplasmose IgG quantitatif
IMMULITE/IMMULITE 1000 n'a pas été
testé sur tous les types de tubes
possibles. Veuillez consulter le chapitre
intitulé Autres Types d'Échantillons pour
plus de renseignements sur les tubes qui
ont été évalués.
Volume nécessaire : 10 µl d 'échantillon
pré-dilué (dilution au 1:21). (L'unité-
échantillon doit contenir au moins 100 µl
de plus que le volume total nécessaire.)
Conservation : 3 jours à +2°C/+8°C ou
6 mois à –20°C.

Dilution des échantillons de patients :
Les échantillons doivent être dilués au
1/21
ème
avec du diluant échantillon
IgG/IgM (LIGZ2). Cette dilution doit être
faite manuellement. Exemple : ajouter
20 µl d'échantillon à 400 µl de diluant
échantillon (LIGZ2) fournis avec le coffret.
Le système IMMULITE 1000 Windows

réalise également les dilutions
automatiquement, en utilisant le diluant
échantillon IgG/IgM (LIGW) fourni dans le
coffret (L1KIGW1,5).
Les résultats imprimés ont été corrigés en
tenant compte d'une dilution au 1/21
ème
.
Précautions d'emploi
Réservé à un usage diagnostique in vitro.
Réactifs : conserver les réactifs à +2/
+8 °C. Eliminer les déchets conformément
à la réglementation en vigueur.
Respecter les précautions d'emploi et
manipuler tous les composants du coffret
comme des produits potentiellement
infectieux. Les réactifs dérivés de produits
humains et utilisés dans ce coffret ont subi
un test sérologique pour la Syphilis et des
tests de dépistage pour les anticorps anti-
VIH1 et 2, anti-HCV et pour l'antigène de
surface de l'hépatite B, qui se sont tous
avérés négatifs.
De l'azide de sodium à des concentrations
inférieures à 0,1 g/dl a été ajouté comme
conservateur ; lors de l'élimination,
l'évacuer avec de grandes quantités d'eau
pour éviter une accumulation d'azides
métalliques explosifs dans les
canalisations.
Substrat chimiluminescent : éviter la
contamination et l'exposition directe au
soleil. (Voir notice).
Eau : utiliser de l'eau distillée ou
désionisée.
La bille est revêtue d'antigène de
Toxoplasma gondii inactivé. Cependant,
des parasites ayant pu subsister, la
prudence est recommandée lors de
l'utilisation ou de l'élimination des produits
fournis.
Les résultats d'IgG dirigées contre
Toxoplasma gondii obtenus sur un
échantillon donné, avec les tests de

différents fabricants, pourront présenter
une disparité en raison de différences de
méthodologie et de spécificité des réactifs.

Toxoplasmose IgG quantitatif. Les

Standard International de l'OMS pour les
anticorps sériques anti-toxoplasmose. Le
seuil du dosage est déterminé par courbe
ROC à partir d'échantillons positifs et
négatifs représentatifs (confirmé par
dosage ELISA).
Positif : un résultat supérieur ou égal à
8 UI/ml indique une infection passée.
Négatif : un résultat inférieur à 6,5 UI/ml
est considéré comme négatif. (Les
résultats inférieurs à 6,5 UI/ml doivent être
rendus comme Négatif sans mentionner
de concentration numérique).
Indéterminé : un résultat supérieur ou
égal à 6,5 UI/ml et inférieur à 8 UI/ml est
considéré comme indéterminé.
Une concentration supérieure à 250 UI/ml
devra être rapportée comme « supérieure
à 250 UI/ml ». Un échantillon dont la
concentration, obtenue sur une dilution au
1:21 par l'appareil, est supérieure à
250 UI/ml, devra être dilué d'un facteur
supplémentaire avec le diluant échantillon
IgG/IgM.
La présence d'anticorps IgG anti-
Toxoplasma gondii est révélatrice d'un
contact antérieur avec le parasite. Un seul
échantillon peut uniquement servir à
évaluer le statut sérologique de l'individu.
Un résultat « indéterminé » indique des
taux d'anticorps anti-Toxoplasma gondii
intermédiaires : l'échantillon devra donc
être dosé de nouveau. Les échantillons
qui restent alors « indéterminés » devront

être testés par une autre méthode, ou un
autre prélèvement devra être obtenu – si
possible – dans un délai raisonnable (une
semaine, par exemple).
Toute interprétation de résultats est
déconseillée si les contrôles positif ou
négatif donne un résultat situé en dehors
de l'intervalle de référence défini à la
section « Contrôle de qualité ».
L'importance du taux d'IgG rapporté n'est
pas corrélée à un titrage en point final.
Les résultats communiqués au médecin
par le laboratoire devront comporter la
mention suivante : « Les résultats suivant
ont été obtenus avec le dosage
IMMULITE/IMMULITE 1000
Toxoplasmose IgG quantitatif. Les
résultats obtenus avec des procédés de
dosage d'autres fabricants ne doivent pas
y être substitués. »
Valeurs de référence
Il est possible que les IgG spécifiques ne
soient pas détectables dans les phases
précoces de l'infection par Toxoplasma
gondii. En cas de suspicion d'infection au
stade précoce, il faudra impérativement
effectuer un dosage IgM anti-Toxoplasma
gondii. Les taux d'IgG anti-Toxoplasma
gondii commencent à augmenter une à
deux semaines après la contamination. La
réponse anticorps atteint son maximum en
6 à 8 semaines, puis diminue
progressivement sur plusieurs mois, voire
plusieurs années. Le titre d'anticorps n'est
pas corrélé à la gravité de la maladie.

La prévalence des anticorps anti-
Toxoplasma gondii varie avec l'âge et la
localisation géographique. Des études ont
été menées sur des échantillons
provenant de sujets asymptomatiques en
bonne santé apparente, du nord-ouest
des Etats-Unis. Les sujets, au nombre de
69, étaient âgés de 15 à 64 ans. (Les
échantillons avaient été recueillis et
congelés à –20 °C avant d'être dosés). La
prévalence de la séropositivité pour
Toxoplasma gondii, sur la base des
résultats obtenus par le dosage
IMMULITE Toxoplasmose IgG quantitatif,
est de 56 % dans cette population.
Des études complémentaires ont été
menées sur des échantillons prélevés
chez 42 femmes enceintes, âgées de 16 à
41 ans, habitant la même région. (Les
échantillons avaient été recueillis et
congelés à –20 °C avant d'être dosés). La
prévalence de la séropositivité pour
Toxoplasma gondii, sur la base des
résultats obtenus par le dosage
IMMULITE Toxoplasmose IgG quantitatif,
est de 5 % pour cette population. (Voir le
graphique “Expected Values”.)
Utiliser ces valeurs à titre indicatif
uniquement. Chaque laboratoire devrait
établir ses propres valeurs de référence.
Limites
L'utilisation du dosage
IMMULITE/IMMULITE 1000
Toxoplasmose IgG quantitatif pour le
diagnostic d'une infection récente en
dosant des paires de sérums n'a pas été
validée.
Les résultats doivent impérativement être
interprétés dans le contexte clinique du
patient, de la symptomatologie et d'autres
résultats de laboratoire.
La mise en évidence d'IgG spécifiques
dans un échantillon unique ne permet pas
de distinguer une infection active d'une
infection passée. Les patients suspects de
primo-infection ou d'infection active
doivent faire l'objet d'une recherche d'IgM
anti-Toxoplasma gondii.
Pour mettre en évidence une
séroconversion, un second échantillon
sérique devra être prélevé trois ou quatre
semaines après le prélèvement négatif de
la phase aiguë, au cours de la phase de
convalescence de l'infection. L'échantillon
de phase aiguë doit être conservé et testé
en parallèle avec l'échantillon de la phase
de convalescence.
Il est possible que les IgG spécifiques ne
soient pas détectables dans la phase
précoce d'une infection aiguë par
Toxoplasma gondii.
Les résultats obtenus chez des patients
séropositifs pour le VIH, des patients
suivant un traitement immunosuppresseur
ou des patients ayant d'autres troubles
conduisant à une immunosuppression,
devront être interprétés avec prudence.
Les performances de ce dosage n'ont pas
été évaluées pour l'utilisation
d'échantillons provenant de sang de
cordon, de nouveau-nés, de patients
allant recevoir une transplantation ou pour
des liquides biologiques autres que le

sérum, comme l'urine, la salive ou le
liquide amniotique.
Les anticorps hétérophiles du sérum
humain peuvent réagir avec les
immunoglobulines faisant partie des
composants du coffret et interférer avec
les immunodosages in vitro. [Voir Boscato
LM, Stuart MC. Heterophilic antibodies: a
problem for all immunoassays. Clin Chem
1988:34:27-33.] Les échantillons
provenant de patients fréquemment
exposés aux animaux ou aux produits
sériques d'origine animale peuvent
présenter ce type d'interférence pouvant
potentiellement donner un résultat
anormal. Ces réactifs ont été mis au point
afin de minimiser le risque d'interférence,
cependant des interactions potentielles
entre des sérums rares et les composants
du test peuvent se produire. Dans un but
diagnostique, les résultats obtenus avec
ce dosage doivent toujours être utilisés en
association avec un examen clinique,
l'histoire médicale du patient et d'autres
résultats.
Performances du test
Consulter les tableaux et graphiques pour
obtenir les données représentatives des
performances du test. Les résultats sont
donnés en UI/ml. (En l'absence de
précision supplémentaire, tous les
résultats ont été obtenus sur des
échantillons sériques prélevés sur tubes
sans gel, ni activateur de la coagulation).
Domaine de mesure : 5,0 – 250 UI/ml.
Standardisé par rapport au Troisième
Standard International de l'OMS pour les
anticorps sériques anti-toxoplasmose chez
l'homme.
Sensibilité analytique : ≤ 5,0 UI/ml.
Précision : les statistiques ont été
réalisées sur les résultats de 5 doublets
testés chacun pendant 5 jours. (Voir le
tableau « Intraassay Precision » et
«Interassay Precision».)
N.B. : le contrôle négatif et l'échantillon
n° 1 étaient toujours inférieurs à 5 UI/ml
(la limite inférieure du domaine de mesure
pour tous les doublets de chaque série).
Spécificité : La spécificité du test
IMMULITE Toxoplasmose IgG quantitatif a
été évaluée sur 50 échantillons positifs
pour le cytomégalovirus (CMV), le virus de
la varicelle et du zona (VZV), le virus
Epstein-Barr (EBV), pour Mycoplasma
pneumoniae et le parvovirus B19. Avec
ces échantillons, le dosage IMMULITE
Toxoplasmose IgG quantitatif et un
immunodosage pour le dosage des IgG
anti-Toxoplasmose (coffret D) avaint une
concordance de 96% (48/50).
Autres types d'échantillons : pour
estimer l'effet de l'utilisation de différents
type d'échantillons, 16 volontaires ont été
prélevés sur tubes secs, héparinés et sur
tubes vacutainer SST
®
Becton Dickinson.
Tous les échantillons puis dosés selon le

Performance clinique
La performance clinique du dosage
IMMULITE Toxoplasmose IgG quantitatif a
été étudiée sur un total de 199
échantillons sériques, dosés chacun par
trois centres investigateurs, dont deux
situés en dehors des Etats-Unis. Sur les
199 échantillons (recueillis et congelés à
-20 °C avant d'être dosés), 61 avaient été
prélevés chez des femmes venues
consulter pour un examen obstétrique ou
gynécologique, 42 avaient été prélevés
chez des femmes enceintes et le reste
provenait de patients présentant divers
états cliniques (SIDA, anémie, artérite,
fatigue, céphalées, lymphome, méningite,
pneumonie, etc.). On comptait 37 hommes
et 162 femmes, âgés de 15 à 68 ans.
Une des études rétrospectives a été
menée dans un centre hospitalo-
universitaire du Nord-ouest des Etats-

Unis. Les 199 échantillons ont été
analysés avec le dosage IMMULITE
Toxoplasmose IgG quantitatif et avec le
coffret A, un dosage immunofluorescent
(ELFA) du commerce.

menée en dehors des Etats-Unis, 194
échantillons sur 199 (5 échantillons
avaient un volume insuffisant) ont été
dosé avec le test IMMULITE
Toxoplasmose IgG quantitatif et avec le
coffret B.

positifs avec le coffret A.
Dans la troisième étude rétrospective,
également menée en dehors des Etats-
Unis, 160 échantillons sur 199 (39
échantillons étaient d'un volume
insuffisant) ont été analysés avec le
dosage IMMULITE Toxoplasmose IgG
quantitatif et avec le coffret C, un dosage
immuno-enzymatique du commerce.

divergence avec le coffret C.
