2. Personal Planning and Self-Directed
Initiatives
Families from throughout Chatham-Kent receive respite, community
support, advocacy, personal and life planning through
Community Living Wallaceburg’s Individual and Family Directed
Supports. All resources are managed and directed by families or
guardians, who engage their own in-home worker. Recruitment and
training assistance is offered with support through respiteservices.com

4. Special Services at Home
SSAH
This program assists children, youth with disabilities who live at home with their
families. SSAH’s focus is to provide funds to purchase supports not available
elsewhere in the community and is a model for individualized, consumer-
directed funding.

Community Living Wallaceburg is currently serving 159 families in
Chatham-Kent who receive Special Services at Home funding.
•
Movement of people off waitlist is continuing and commitment from Ministry
to have no waitlist within the next one year.

5. Family Managed Respite
FMR
FMR enables a wide range of self-directed funding providing
family members or guardians with an opportunity to have a
periodic break from the daily responsibility of care giving for
their family member who has an intellectual disability.

Community Living Wallaceburg is currently serving 73 families
in Chatham-Kent who receive Family Managed Respite
funding.

6. Out of Home Respite
OHR
Community Living Wallaceburg extends funding for families
or guardians to purchase planned and coordinated respite
supports for their children with complex multiple needs.
OHR is a respite service provided in any location outside the
family’s home.

Community Living Wallaceburg is currently serving 30
families in Chatham-Kent who receive Out of Home
Respite funding.

7. Specialized Respite Support
SRS
SRS provides family members or guardians with children or
youth who have exceptional social, emotional and
behavioural needs with opportunities to plan periodic breaks
from care giving.
•
Community Living Wallaceburg is currently serving 23
families in Chatham-Kent who receive Specialized Respite
Support funding.

8. Autism Spectrum Disorder Respite Support
ASDR
ASD Respite Support offers a flexible mechanism to families or
guardians caring for children and youth, who have a diagnosis within
the Autism Spectrum Disorder & are under the age of 18, to receive
temporary relief from the physical and emotional demands involved
with caring for children with ASD.

Community Living Wallaceburg is currently serving 56 families in
Chatham-Kent who receive Autism Spectrum Disorder Respite
Support funding.

through the Family Registry and Support Worker Database. It is the perfect place
to look when families want to hire a qualified, experienced in-home support
worker and for in-home support workers who want to advertise their credentials,
