I. REGISTRATION PROCEDURES
A. For Domestic Worker – To Secure SS Number
Fill‐out and submit Personal Record Form (SSS Form E‐1) at any SSS Branch or Service Office
Submit photo copy and present original/certified true copy of any of the following Primary
Documents:
• Birth Certificate • Passport
• Baptismal Certificate • Professional Regulation Commission Card
• Driver’s License • Seaman’s Book
In the absence of Primary Documents, submit photo copy and present original/certified true
copy of TWO of the following Secondary Documents, BOTH should bear the name and at least
ONE should indicate the Date of Birth:
• ATM Card
• Bank Account Passbook • Police Clearance
• Birth/Baptismal Certificate of Children • Postal ID Card
• Marriage Contract • Voter’s ID/Affidavit
• NBI Clearance • Transcipt of School Records
• ID Card issued by Local Government Units (e.g. Barangay, Municipal/City)
(Please inquire from the SSS Branch for List of other Secondary Documents that may be
accepted)
In the absence of Primary/Secondary Documents, another option is to apply for Late Registration of
Birth at the Office of the Local Civil Registrar (LCR) of the place of birth.
If the required supporting document/s is/are not available at the time of registration, SS Number shall
still be issued. However, the SSS Form E‐1 shall be stamped to indicate the non‐submission of Primary or
Secondary Documents, which means that:
• Membership Status of Domestic Worker is Temporary
• The SS Number can only be used for contribution payment and employee reporting (by the
Employer).
• Submission of Primary or Secondary Documents is required for conversion of Membership
Status to Permanent
• Availment of SSS Benefits and Loans is only allowed for Permanent Membership Status, subject
to qualifying conditions.
Application for the Unified Multi‐Purpose ID (UMID) Card may be made if Membership Status is
Permanent, and upon posting of at least (1) monthly contribution. The Application for Social Security
Card Form (SSS Form E‐6) shall be used for this purpose.

III. PAYMENT PROCEDURES & SUBMISSION OF CONTRIBUTIONS COLLECTION LIST
It is the responsibility of the Household Employer to pay the monthly contributions of the Domestic
Worker (thru SSS Form R‐5) and to submit Contributions Collection List (SSS Form R‐3) covering said
payment.
1. Fill‐out Contributions Payment Return Form (SSS Form R‐5) and pay at:
• SSS Branches with Tellering Services, or
• Accredited Collecting Banks
2. Payment is to be made monthly and payment due date shall be as follows:

• Advance payments may be made, but said payments shall only be credited to the records of the
Domestic Worker as they fall due. Payments made beyond the deadline are charged a Penalty of
three percent (3%) per month.
3. Fill‐out and submit Contributions Collection List (SSS Form R‐3) quarterly, together with a copy of
the SSS Form R‐5 (machine‐validated or with Special Bank Receipt).
• SSS Form R‐3s covering the period/s for which advance payment/s were made may also be
simultaneously submitted but shall be posted to the Domestic Worker’s account as they fall
due.
4. The Contributions Collection List may also be submitted monthly and electronically through:
• Enrollment in the SSS Web facility (www.sss.gov.ph), or
• Participation in the e‐R3 (electronic R‐3) Program of the SSS
FORMS are available at all SSS Branches and Service Offices or may be downloaded from the SSS
Website (www.sss.gov.ph)
