Fizik mayenede inspeksiyonla hastanin yüz ifadesinden içinde bulundugu agrili durmun ciddiyeti
anlasilabilir.Ciltte solukluk ve sarilik akla maligniteyi getirmelidir.Kuru, atonik, kaba ve soluk cilt kronik
böbrek yetmezliginde görülür. Perinefritik apsede lomber bölge derisinde ödem
gözlenebilir.Apse psoas kasina kadar ulasirsa konkavitesi o tarafa bakan skolyoz görülebilir. Ciltde
sitrialarin bulunmasi ve ensede bufalo hörgücü surrenal hiperplazisini akla getirir.Jinekomasti, bir endokrin
bozuklugun yaninda adrenal tümör ve testis tümörü için uyarici özellik tasir.Yasli erkeklerde Jinekomasti,
siroz da veya prostat kanseri nedeniyle yapilan östrojen tedavisine bagli olabilir.Alt ekstremitelerde ödem,
kardiak yetmezlikte , venöz yetmezlikte veya genitoüriner tümörlerin metastazlarina bagli lenfatik
obstrüksiyonlarda görülür. Bu nedenle bütün sistemler bastan asagi dikkatle gözlenmeli ve anormal
durumlarin ürogenital sistemle iliskisinin olup olmadigi açiklanmaya çalisilmalidir.
BÖBREK MUAYENESI
Böbreklerin palpasyonu hasta sert bir zemin üzerinde yatarken bir el bögrün arkasinda digeri önünüde
olacak sekilde yapilir.Bu muayenede bir kitle hissedilirse büyük olasilikla böbrege ait hidronefroz, polikist
veya tümör olabilir.Derin inspirasyon esnasinda böbrek bir miktar asagi itildigi için, özellikle sag böbregin
palpasyonu biraz daha kolay olur. Böbrek sadece önden palpe edilmeye çalisilirsa,her iki böbregide
büyüten polikistik böbrek hastaligi hepato-splenomegali ile karisabilir. Ayrica sadece önden ve
beklenmiyen bir lokalizasyonda palpe edilen bir kitle ise, büyük olasilikla çekum karsinomu, apendiks
apsesi veya rejiyonal enterit gibi intraperitoneal bir kitledir.Büyümüs böbrek genellikle bir el önde, digeri
arkada iken palpe edilebilir. Böbreklerin palpasyonu kas yapisi gelismis olanlarda ve sisman kisilerde
dahada zordur.Fazla dogum yapmis zayif kadinlarda kolaydir.Yeni doganlarda veya küçük çocuklarda
karinda ele gelen bir kitle oldugu zaman hidronefroz olgusunu ayirt etmek için transilümünasyon degerli bir
yöntem olabilir.
Psödorenal agrinin ayiriminda öykü özelliklerine ilaveten kostalomber eklem üzerine vurmakla veya
bastirmakla, ayrica subkostal sinirlerin innerve ettigi abdomen veya bögür bölgesinin cilt ve ciltaltini
çimdiklemek veya sikmakla kostalomber bölgede agrinin olusmasi gerçek renal agridan ayirici özellik
arzeder.
Bazan oldukça büyük böbrek hidronefroza bagli ise palpasyonla anlasilamiyabilir, bu durumda
perküsyonla hem önden hemde arkadan sinirlari saptanabilir. Renal travmayi takiben bögür bölgesinde
büyüyen kitlenin sinirlarida ancak perküsyonla saptanir, zira hassasiyet ve adele spazmi uygun
palpasyona engel teskil eder.Ayrica renal travmadan sonra açilan damar agzindaki olusan hematomun
basisi kanamayi durdurmustur yapilan asiri palpasyon bu söz konusu olan hematomun basisini giderebilir
ve tekrar kanamaya sebep olabilir.
MESANE MUAYENESI
Mesane bos iken muayene edilemez,ancak içerisinde 150 ml. idrar bulundugunda perküsyonla varligi
anlasilabilir.Akut veya genellikle kronik idrar retansiyonunda mesane asiri derecede dolar ve bazen
göbege kadar uzanir ki bu durum inspeksiyonla fark edilir. Kronik idrar retansiyonlarinda mesane duvari
gevsek oldugu için palpe etmek güç olabilir, bu durumda perküsyon tercih edilen muayene yöntemi
olmalidir.Özellikle mesane tümörünün varligindan süphe edildigi durumlarda rektal veya vajinal yolla
anestezi altinda bimanuel muayene oldukça önemlidir. Tümör kitlesinin mobil veya fikse olmasi invazyon
derecesi hakkinda bize bilgi verebilir.
PENIS MUAYENESI
Hasta sünnetsizse sünnet derisi geri çekilerek pis kokulu akintinin sebebi olarak tümör veya balanitis
görülebilir.Fimozis nedeniyle sünnet derisi geri çekilemiyorsa fimotik halkanin insizyonla açilmasi veya
sünnet yapilmasi gerekir.Orta derecede fimotik halkanin bulundugu durumlarda hastanin kendisi veya
ebeveyni tarafindan geriye çekildiginde hemen eski konumuna getirilmezse, fimotik halka sulkus
koranariusu sikar ve glansta olusan ödem prepusyumun geri çekilmesini zorlastirir veya imkansiz kilar,bu
duruma parafimozis denir ve hastayi rahatsiz edici agrili bir durumudur.Böyle bir durumda fimotik halka
kesilerek açilir veya sünnet yapilir.Meatal stenoz durumu inspeksiyonla anlasilabilir. Böyle oldugunda
meatus toplu igne basi büyüklügünde ve konjesyone olabilir ve üriner obstrüksiyon semptomlarina yol
açar.Idrar akiminin karakterini anlamak için, erkek hastalar mümkünse idrar yaparken izlenmelidir.Normal
idrar akiminin yeterli kalinlik ve kavsi vardir ve kesintisizdir. Ayrica meatusun anormal açilma (hipospadias
veya epispadias) durumlarida inspeksiyonla anlasilir.Genellikle hipospadis durumunda, meatal stenoz ve
penisin ventral tarafa bükülmesi( penil kordi) birlikte görülür.
Genellikle hipopitüitarizim sonucu olan infantilizm ileri derecede küçük genital organlarla
karakterizedir.Virilizm infantilizimin tam tersidir ve asiri androjen üretimine bagli olan bu durum genellikle
2. adrenokortikal tümör ile birlikte görülür, ayrica benign tipte puberte prekoks sonucuda olabilir.Penisin
büyüklügü veya küçüklügü gibi, hastanin kendi kanaatinin olusturdugu psikojenik sorunlar o denli genistir
ki burada tümü ile ele alinmasi olanaksizdir.Ancak kisaca denilebilirki böyle bir hastayi muayene ederken
hastanin açik veya gizli endisesini gidermek üzere hem profesyonel ve hemde kisisel açidan objektif bir
yaklasim gerekir.
Penis korpusunun palpasyonunda korpus kavernozumlari ilgilendiren çesitli büyüklükte sertlikler
hissedilebilir, ki bunlar Peyroni hastaligi için yeterli bir bulgudur. üretra boyunca gergin indurasyonlar
üretral striktüre bagli periüretrit bulgusu olabilir.
SKORTUM VE MUHTEVASININ MUAYENESI
Skrotum cildinin tümör ve infeksiyon durumlari oldukça nadirdir bazen küçük sebasiyal kistler
görülür.Perineal hipospadias oldugunda skrotum bifid görünümdedir.
Testisler, fizik muayene esnasinda uygulanan basiya veya hastaliklarinin tunika albugineada olusturdugu
gerginlige son derece hassastir.Bu sebeble muayene iki elin parmak uçlariyla dikkatlice yapilmalidir.Testis
içerisinde agrisiz sert kitlenin varligi halinde aksi ispat edilene kadar tümör düsünülmelidir.Testiste agrisiz
kitle durumunda testis tümörü veya sifiliz gomu söz konusudur. Testisler skrotum lojunda
bulunamiyabilir,bu durumda retraktil testis , inmemis testis (kriptorsidizm),anorsizim veya ektopik testis
akla gelir.Inmemis testisde genellikle organ kasik bölgesinde palpe edilir.Skrotum içinde palpe edilen
testisin büyüklügü ve kivamina dikkat edilmelidir.Sert ve atrofik testis primer infertilitenin sebebi olabilir.
Epididimler testislerden yumusak kivamda ve testisin arka yüzünde bazen de ondan ayri olarak palpe
edilir.Epididimler büyüklük ve kivam bakimindan dikkatlice muayene edilmelidir. Epididimlerde agrisiz
indurasyonun palpasyonu halinde, akla enfeksiyon (tüberküloz, sistozomiazis veya nonspesifik) gelmelidir,
zira pirimer tümörü oldukça nadirdir.Epididimitin akut safhasinda testisden palpasyonla ayirt edilmesi asiri
hassasiyetinden dolayi güçtür.
Duktus deferensler inguinal kanal çikisindan epididimlere kadar basparmak ve isaret parmagi arasinda
palpe edilmelidir.Kronik infeksiyonunda kalinlasma ve tüberküloz durumunda fuziform genislemeler dikkati
çeker.
Hasta ayakta muayene edilirken testisin üst tarafinda varikosele ait variköz venlerin paketi
görülebilir.Venlerdeki dilatasyonun derecesi yatar durumda azalir ve valsalva manevrasi ile artar.
Skrotal bir kitlenin varligi halinde transiluminasyon müsbet ise hidrosel veya spermatosel akla
gelir.Aspirasyon sivisinda sperm görülmesi spermatoseli akla getirebilir.Transiluminasyon negatif ise
hematosel, skrotal herni veya büyük bir testis tümörü akla gelmelidir.Hidrosel durumundan dolayi testis
palpe edilemiyorsa aspirasyondan sonra palpe edilmesi uygundur zira testikuler tümörlerin % 10 unda
reaktif hidrosel bulunabilir.
REKTAL MUAYENE
Hastada idrar yolu enfeksiyonu düsünülüyorsa rektal muayeneden daha dogrusu prostat glandinin
parmakla palpasyonundan önce idrar örnegi alinmalidir.Bu son derece önemlidir,zira prostatik masaj veya
palpasyon bile prostatik sekresyonu üretra içine akitir.Bu sekresyon iltihapli ise rektal muayeneden sonra
alinan idrar örnegini kontamine eder.
Rektal muayenede amaç sadece prostatin palpasyonu olmamali, perineal ve perianal inspeksiyon
yapilmalidir. Inspeksiyonla kasinma sebebiyle irritasyon veya skatrizasyon nedbeleri, kondilomata, fissur,
fistül ve eksternal hemoroidler görülebilir.Rektal muayene ile anal stenoz, internal hemoroid, mukozal
fissur, fistül, polip veya rektal malignensi ekarte edilmelidir.Bu sözkonusu patolojik durumlarin varligi
halinde anogenital sendrom (prostatosis) denilen durum açiklanabilir. Üriner sfinkterle anal sifinkterin
anatomik yakinligi, embriyolojik gelisim ve otonomik ve somatik innervasyonlarinin müsterekliginden dolayi
anusun agrili ve irritatif durumlari siklikla anogenital sendroma sebep olur.Bu sendromda hiçbir ürogenital
patolojik bulgu olmadigi halde isemeyle ilgili yakinmalar olur.Irrite olmus anal sifinkterden kaynaklanan
inhibitör stimulasyonlar pudental sinir yoluyla sakrospinal miksiyon merkezine tasinir ve parasempatik
pelvik sinirle inhibitor impulslar mesaneye ulasir.Ayrica deneysel olarak anal sifinkterin elektriksel
stimulasyonu inhibitör mesane cevabina yolaçtigi gösterilmistir.
Genital bölgenin muayenesini genellikle dikkatli rektal muayene izlemelidir.Muayeneye baslamadan önce
hastanin hem fizik hemde emosyonel duyarliligini dikkate almaliyiz.Ne yapmak istendigi ve bunun
gerekliligi kisa ve basitçe açiklanmali, böylece muayenenin daha az rahatsiz edici olmasi saglanir.Rektal
muayene ya hasta yan yatarken bacaklari fleksiyonda veya ayakta öne dogru egilmis, elleri bir sandalye
veya yataga dayanmis durumda iken yapilir. Eldivenli isaret parmagimiz iyice yaglandiktan sonra anüs
halkasinin ön kenarina konur ve yavasca simfizis pubise dogru itilir.Anal kanal kaslari gevsedikçe isaret
parmagi daha derine sokulur.
3. Bu muayenede anal sifinkter tonusunun degerlendirilmesi oldukça önemlidir.Sfinkter tonus azalmasi veya
kaybi nörojenik bozuklugu akla getirir. Anal sifinker tonus eksikligi söz konusu oldugunda, perianal ve
perineal sahalarin (semer sahasinin) his kusuru ve bulbokavernöz refleks muayenesi ihmal edilmemelidir.
Prostat glandinin büyüklügü takriben dörer santimetre boyutlarindadir.Gland hiperlaziye ugrayinca lateral
sulkuslar derinlesirken median sulkus oblitere olur.Prostatik hiperplazinin klinik önemi glandin büyüklügü
ile degil semptomlarin siddeti ve rezidüel idrar miktariyla degerlendirilir.Hastada akut idrar retansiyonu
veya üretral degerlendirilmesi yaniltici olur.Normal olarak prostat glandinin kivami, parmaklarimizi
oppozisyon haline getirdigimizde kontrakte tenar bölgenin sertligini andirir.Koit eksikliginde veya
enfeksiyon nedeniyle bozulan drenaj durumunda prostat glandi yumusak kivamda hissedilebilir.Prostat
taslarinda, karsinomasinda, kronik nonspesifik infekisyonlarinda ve tüberkülozunda glandin yüzeyinde sert
nodül palpe edilebilir. Ayirmak güç olmakla beraber enfeksiyöz orijinli nodüller glandin yüzeyinden daha
yüksektir ve sertligi normal dokuya dogru tedricen azalir, karsinomada nodül yüksek degildir ve sertligi
keskin sinirlidir.Ilerlemis karsinomada normalde bir miktar mobil olan gland fiksedir ve tamamen tas
sertligindedir.Prostat taslarinin varliginda, taslar genellikle çok sayida oldugundan palpasyonda
krepitasyon hissi algilanir.
Mesane tabani altinda bulunan seminal veziküller normalde rektal muayenede palpe edilemezler.Kronik
infeksiyon (özellikle tüberküloz veya sistozomiyazis) mevcudiyetinde ve prostat karsinomasinin metastazi
halinde prostat glandinin üstünde veziküloseminalisler sert olarak palpe edilebilir.Seminal veziküllerin
primer tümörleri oldukça nadirdir, fakat ileri derecede hipospadiasli hastalarda prostatin üstünde bazan
kistik bir kitlenin varligi veziküloseminalis kistlerini akla getirir.
Prostat infeksiyonlarinda tani için prostat sekresyonu gerekli olabilir.Bu durumda prostat masajina bas
vurulur,fakat akut prostatit ve akut prostatosistitis durumlarinda prostat masaji yapilmamalidir. Prostat
masajinda temel manevra, prostatik üretra içine sekresyonu akitabilmek için isaret parmagimizla glanda
sikica bastirmaktir.Glandin lateral ve üst tarafindan parmagimizin yuvarlama hareketiyle median sulkusa
dogru bastirilir ve iki taraftanda bu hareket yapildiktan sonra median sulkusa yukardan asagiya dogru
bastirilarak parmagimiz rektumdan çikatilir. Bazi hastalarda fazla miktarda prostat sekresyonu çikartilirken
bazilarinda çok az veya hiç elde edilemez.Bu bir dereceye kadar tatbik edilen masajin kuvvetine
baglidir.Hiç sekresyon alinamazsa, masajdan sonra alinan inisiyal idrar örneginin santrifüj çökelegi prostat
sekresyon olarak degerlendirilir. Sekresyonun mikroskopik muayenesi küçük büyütme altinda
yapilir.Normal sekresyonun mikroskopik muayenesinde refraktil, eritrositlere benzeyen fakat onlardan
daha küçük olan çok sayida lesitin cisimcikleri, birkaç lökosit, epitel hücreleri ve sperm ve nadir korpora
amilosealar görülür.Çok sayida (her küçük büyütme sahasinda 8-10 dan fazla) lökosit görülmesi patolojik
bir durum olup prostatit teshisini destekler.Prostatik sekresyon kültürü yapmak için, üretranin mekanik

3. Complaint Urological complaint(s) can be categorized into one or more of: Pain Changes in the
act of micturition Changes in gross appearance of urine Changes in function and/or appearance

4. Pain Pain arising from the GU tract may be quite severe and is usually associated with
Obstruction: Ureteric stone. Urinary retention. Inflammation parenchymatous organs inflamm.
produces severe pain (pyelonephritis, prostatitis, and epididymitis) Inflammation of the mucosa
of a hollow viscus such as the bladder or urethra usually produces discomfort. Tumors of GU
tract usually do not cause pain unless they produce obstruction or extend to adjacent nerves.

5. Renal Pain Site: ipsilateral costovertebral angle just lateral to the sacrospinalis muscle and
beneath the 12th rib. Pain due to inflammation is usually steady ( dull ache ) due to acute
4. distention of the renal capsule, Pain due to obstruction fluctuates in intensity ( colicky) . may be
associated with gastrointestinal symptoms because of reflex stimulation of the celiac ganglion.
Radiation: across the flank anteriorly toward the upper abdomen and umbilicus and may be to

6. Differential Diagnosis of Renal Pain 1) Pain of intraperitoneal origin (perforated duodenal ulcer
or pancreatitis) has the following characters: Radiates into the back, but the site of greatest pain
and tenderness is in the epigastrium. Radiates into the shoulder because of irritation of the
diaphragm and phrenic nerve. 2) Renal pain may also be confused with pain resulting from
irritation of the costal nerves (radicular pain), most commonly T10-T12. However, the pain is not

7. Ureteral Pain usually acute and secondary to obstruction. Distribution of the pain according to
site: In upper ureteral obstruction, the pain may be referred to the scrotum in the male or the
labium in the female. Midureter obstruction: on the right side is referred to the right lower
quadrant (McBurney's point) and simulate appendicitis; on the left side to left lower quadrant.
Lower ureteral obstruction causes bladder irritability (frequency, urgency, and suprapubic

8. Vesical and Prostatic pain Vesical pain due to retention or inflammation. Constant suprapubic
pain that is unrelated to urinary retention is seldom of urologic origin. Inflammatory conditions
of the bladder usually produce intermittent suprapubic discomfort. Prostatic pain due to
inflammation with secondary edema and distention of the prostatic capsule. poorly localized to

9. Penile and Testicular Pain Penile pain Pain in the flaccid penis is referred pain from bladder or
urethra and maximally at the urethral meatus. Pain in the erect penis is usually due to Peyronie's
disease or priapism. Scrotal pain Primary pain arises from within the scrotum: Acute epididymitis
or torsion of the testis. Chronic scrotal pain (dull, heavy) due to hydrocele or varicocele.

10. Irritative Symptoms Frequency The normal adult voids five or six times per day, with a
volume of approximately 300 mL with each void. Urinary frequency is due either to increased
urinary output (polyuria), detrusor instability or to decreased bladder capacity. Magdy Fath-Alla,

11. Irritative Symptoms Nocturia is nocturnal frequency. Normally, adults arise no more than
twice at night to void (decrease urine concentration with age). Nocturia occur in the patient with
congestive heart failure and peripheral edema, old aged and drinking fluids at night. Dysuria is
painful urination commonly referred to the urethral meatus. at the start of urination indicate
urethral pathology. at the end (strangury) indicate bladder origin. Dysuria is frequently
accompanied by frequency & urgency. Urgency the sudden intense desire to void that the

12. Obstructive Symptoms Urinary hesitancy delayed start of urination. Decreased force and
caliber of urinary stream is due to bladder outlet obstruction and commonly results from benign
prostatic hyperplasia (BPH) or a urethral stricture. Straining refers to the use of abdominal
musculature to urinate. Intermittency means interrupted stream. Postvoid dribbling refers to the

13. Incontinence Urinary incontinence is the involuntary loss of urine Continuous Incontinence
most commonly due to: urinary tract fistula (as vesico-vaginal fistula usually due to gynecologic
surgery, radiation, or obstetric trauma. ectopic ureter that opens either at the urethra or the
female genital tract. Stress Incontinence is sudden leakage of urine with coughing, sneezing,
exercise (increase intra-abdominal pressure). most common in women after childbearing or

14. Incontinence Urge Incontinence loss of urine preceded by a strong urge t o void. Due to:
cystitis, neurogenic bladder. Overflow Urinary Incontinence Due to advanced urinary retention
and high residual urine volumes. Urine dribble due to bladder overflow. Enuresis micturition that

15. Changes in the gross appearance of urine Hematuria the presence of blood in the urine > 3
RBCs per high-power microscopic field (HPF). painless hematuria in adults, should be regarded as
a symptom of urologic malignancy until proved otherwise. In evaluating hematuria: gross or
microscopic. Initial, terminal or allthrough. Painful or not. Associated with clots or not and shape

16. Changes in the gross appearance of urine Cloudy Urine Pyuria Pyuria is a urinary tract
infection in which large quantities of white blood cells cause urine to have a cloudy appearance.
Microscopic examination of the urine will demonstrate pus cells. Phosphaturia due to
precipitation of phosphates in alkaline urine. Acidification of urine with acetic acid at the time of

17. Abnormal appearance and/or function of the male external genitalia Male sexual dysfunction
and impotence impotence is the inability to achieve and maintain an erection adequate for
intercourse. Other male sexual disorders, including loss of libido, absence of emission, absence
of orgasm & premature ejaculation. Bloody ejaculate (Hematospermia) refers to the presence of
blood in the seminal fluid. It almost always results from nonspecific inflammation of the prostate

18. Abnormal appearance and/or function of the male external genitalia Penile complaints
Cutaneous lesions. Penile curvature. Urethral discharge most common symptom of venereal
infection. purulent thick, profuse, and yellow to gray discharge is typical of gonococcal urethritis.
scanty and watery discharge is due to nonspecific urethritis. bloody discharge suggests
carcinoma of the urethra. Scrotal complaints Cutaneous lesions. Absent or retractile testis.

19. Systemic symptoms Fever and chills occur with infection anywhere in the GU tract but most
common with pyelonephritis, prostatitis, or epididymitis. Fever, weight loss, and malaise are
nonspecific systemic manifestations of: acute and chronic inflammation, renal failure, and

21. General Examination Apperance: distressed due to pain or does appear unwell suggesting
systemic illness and possibly renal failure. Complexion: Pallor is evidence of anemia (due to
hematuria). Vital data: blood pressure. Signs of dehydration: dry mouth and tongue may indicate
renal failure or polyuria of diabetes. Cervical lymph nodes enlargment due to metastatic spread

23. Abdominal Examination General Palpation Use warm hands. Examine the tender areas last.
Light palpation then deep. Check for guarding, rigidly and rebound tenderness. Determine for
any mass: site, tenderness, size and shape, surface (irregular or smooth), edge (regular or
irregular), consistency (soft or hard), mobility, whether pulsatile or ballotable. Differential

enlargement or masses) by with a hand posteriorly lifting up the kidney towards the examining

Bladder base Anal tone. Anal pathology. A hard area in either or both lobes suggests a cancer
