The 2013-2014 Annual Report for Life Management Center of Northwest Florida. Life Management Center is the community behavioral and mental health center providing services in Bay, Calhoun, Gulf, Holmes, Jackson and Washington Counties in Northwest Florida.
2. Dear Friends,
Do you know what events occurred in 1954? During that year William Holden and Audrey Hepburn won best actor and
actress awards at the Oscars, the Supreme Court ruled on Brown v. Board of Education, and ground breaking began
on Disneyland. Those are memorable historic events and there was also an event in Bay County that we believe was
memorable. That is when we saw the beginning of what is now Life Management Center with the start of the Bay
County Child Guidance Clinic.
In celebrating our 60th anniversary, we are enjoying learning more about our history and seeing the impact our
organization has had on the community and how much impact the community has had on our organization. Over
those 60 years, Life Management Center has merged with the mental health facilities in our surrounding counties and
expanded the scope of whom and how we provide services. You can see more about our history through these pages.
For us, the most exciting part of reflecting on the 60 years has been seeing our commitment and service to our
community continue. We hope as you look through this annual report that you will see not only the significant events in
our history, but also the strides that have been made in the past year and are continuing into our current year. Here are
just a few of those accomplishments:
»» Served 12,268 children and adults
»» Began providing services through our CAT Team, for youth
with severe mental illness,
»» Delivered $597,981 in charity care
»» Received funding to launch the EPIC program, providing early
intervention services to those with first episodes of psychosis
»» Provided certification training in Mental Health First Aid to 84 individuals.
This report is only a small glimpse into the services provided by Life Management Center last year but, hopefully, it gives
you a clear picture of what work is being done for our community. As you browse the report, we hope you will be proud
of the work Life Management Center is able to accomplish with your help.
Thank you for your continued support.

A History of Our Board of Directors Presidents
Sixty years ago a group of local school personnel,
physicians, and private citizens saw a need for mental
health services for children in Bay County, Florida. The
community saw a need and began the services that
started it all. That same community stands behind us
today as we expand our services to provide for even more
of the needs of our friends and family.
What a difference 60 years can make! Today Life
Management Center serves over 12,000 individuals each
year and provides those services in six counties: Bay,
Calhoun, Gulf, Holmes, Jackson and Washington.
One thing that has not changed over those 60 years is the
community support that makes our services possible. The
community saw a need and began the services that began
it all and the community stands behind us today as we
expand our services to fill even more of the needs of our
friends and family. For that reason, we say thank you to all
of you.
We hope you enjoy this quick look back at our history.

Milestones are important times in all
of our lives. What a milestone is varies
according to each individual and some
are harder than others to reach. For
those completing the requirements in
our Group Home program, graduation
is one of those milestones and Brian
is proud to have reached it. Brian is
a client in our FACT program and a
recent graduate of the Group Home.
The FACT program provides intensive,
community-based treatment,
rehabilitation and support services
for adults with severe and persistent
mental illness who have not responded
well to traditional treatment. As a
part of that treatment, some FACT
clients also participate in our Group
Home program, a 24-hour residential
program for adults experiencing
symptoms of mental illness which may
be interfering with their life activities
and preventing them from being
successful at living independently.
The average length of stay in the
Group Home is one year and clients
must complete a 5-step program to
graduate.
Brian is one of those clients who
is happy to say he completed the
requirements and has graduated
from the Group Home. He explains
that the final stage of his time in the
Group Home included learning how
to do things such as budgeting and
managing his medications. He says
they make sure you are “fully capable
of handling anything that can come up
to you in life.”
He also points out that each client has
to put in a lot of effort to reach the
goals set before them. “It’s not a cake
walk,” said Brian. “It’s hard work.”
Brian explains that he was in the Group
Home one time before but was unable
to maintain the requirements to stay
in the home and did not graduate. This
last time he was determined that he
would succeed.
“The key is to find ways to cope with
whatever is troubling you,” said Brian.
“If it’s a work thing, then you find a
way to cope with that. If it’s a living
situation, you change it if it can be
changed.”

Another important part of reaching this
milestone is having a support system
around you. While the Group Home
serves as a support system during
a client’s stay there, clients are also
encouraged to build a support system
outside of the program so they still have
that support after graduating from the
Group Home.
Brian is quick to state that his father
has always been his biggest support
and has always been there. His father
saw there were problems that needed
to be addressed when Brian was still
in school. “He knew something was
wrong,” said Brian. “I had problems
with taking tests and with school in
general and was even held back a year.
He encouraged me to see someone. Dad
is one hundred percent a part of my
support system.”
For those who
wonder if the
time spent in
the Group Home
is beneficial to
clients, they can
just ask Brian.
“The Group Home
changes lives,”
said Brian.

8. Building Healthy Families
When you look into the eyes of the
Self family you know there is love in
their home. Joshua and Crystal have
so many dreams for their lives that
they sometimes find it hard to see
how far they have already come. Just
a few short years ago, the couple was
living in a two-bedroom apartment
that had only a kitchen table and a
TV. They slept on an air mattress on
the floor of one of their bedrooms.
Through the hard times, Life
Management Center employees
have been there for them through
our Healthy Families program.
When Crystal was 8 months into
her pregnancy with their son, J.J.,
they were referred to the program
and staff began working with them
to know what to expect with a new
baby.
“We have been to the bottom of
the bottom, of the bottom,” stated
Joshua. “Now we are fortunate
enough to have a program that is
there when we need it.”
The Healthy Families program is a
nationally accredited, community
based, voluntary home visitation
program that is proven to prevent
child abuse and neglect and other
poor childhood outcomes by
promoting positive parent-child
relationships and child health and
development. Families are also linked
to a medical provider and other
family support services they need
during their participation in the
program.
Cathy Harcus, program director,
describes the program as having
your own personal parenting coach
walking you through each milestone
of your child’s early years.
Crystal Self is quick to point out the
program has helped them anytime
they had questions or had another
need. She says the Healthy Families
staff is quick to step in with advice
or whatever help they needed. A
couple of examples that Crystal share
are when they wanted J.J. to begin
sleeping in his own bed, and then
again when they wanted to wean him
off of formula, and even when they
were concerned about when he would
start talking. “We started wondering
why isn’t he talking,” said Crystal.
“Healthy Families helped us through
that and now we sometimes wish he
would stop talking.”
Joshua explained that there were
times money was tight and they had
to pawn some of their belongings just
to buy diapers for J.J. The Healthy
Families program helped them with
this too and they were able to get
diapers and other necessities from
the program through “Baby Bucks”
they earned as they reached goals set
through the program.
Despite their own hardships, or
maybe because of their hardships,
Josh and Crystal give back when
they can. They explain when they
no longer have a need for something
that they give it to someone who
WE HAVE BEEN TO THE BOTTOM OF THE
BOTTOM...NOW WE ARE FORTUNATE
ENOUGH TO HAVE A PROGRAM THAT IS
THERE WHEN WE NEED IT
Caption
9. does need it. For example,
when J.J. no longer needed
baby food they donated
their remaining supply
back to the Healthy
Families program to help
other families.
“That’s the way a
community and friends
has got to be,” explained
Joshua. “We know how
it feels to be at the very,
very bottom.”
The Healthy Families
program works with each
family to set goals and
then provides support
for them as they work
toward reaching those
goals. Joshua and Crystal
have met several goals
already including J.J.
walking, talking, and
sleeping in his own bed;
Crystal obtaining her
driver’s license; and
Joshua maintaining
steady employment.
Future goals include
moving into a larger
house to have room for
a new baby on the way
and Crystal’s father who is living with
them.
The family welcomed an addition in
December. They have named their new
son Lucius Cole, which means “bringer
of light.”
