14. BIOMEDICAL WASTE GENERATION
Application: GPCB Online Application after getting certificate
from Common Bio-Medical Waste Facilities (Distro Med Services)
Processes:
• Facility Inspection by GPCB Officer
• Comply to recommendation given if any
Issuing Certification by: GPCB will issue a certificate for waste
collection, disposal & storage.
Renewal: GPCB every two yearly & CBWF every three yearly
15. FIRE NOC
Application: To FSED for facility inspection
Processes:
• Recommendation will be given as per norms
• Comply to recommendation given
• Ask CFSO for final inspection
• To rectify any deviation
Certification Issues from: Fire No Objection Certificate will
be issued by FESD after successful inspection.
Renewal: Yearly
16. HOSPITAL REGISTRATION
Application: Municipal Corporation
Processes:
• Application can be done for
1to10bedded, 11to25bedded, 26to50bedded and 51 & above.
• To fulfill the application prerequisites
- Doctors degree certificate, Building Municipal Tax Bill & Paid Receipt, GPCB
Certificate, Validation certificate from Common Bio-Medical Waste Facilities, Nurses
qualification, BU Permission Certificate, Fire NOC etc.
Certification Issues from: Municipal will issue a Hospital
Registration certificate.
Renewal: Every yearly / five yearly
17. REGISTRATION OF DIAGNOSTIC X-RAY
Application: X-Ray Unit, Layout & Registration available on
website.
Processes:
• AERB type approved diagnostic x-ray equipment
• Get layout approval from AERB.
• QA Test report
• Personnel - TLD Badges
Certification Issues from: AERB will issue a X-Ray Registration

18. PRE-NATAL DIAGNOSTIC TECHNIQUES ACT

• Written application to DSO along with equipment details
