Prevalence of stroke

• 1 stroke very 53 seconds
• 1 death from stroke every 3.3 minutes (436/day)
• 750,000 new and recurrent strokes each year in
  US
• Mortality
   – 7.6% 30d
   – 16-23% 3 months
• Yellowstone County
   – 310 strokes expected
Stroke is a treatable condition.

• IV tPA is approved for use within 3 hours
  (NINDS)
• Intra-arterial therapy has proven to be safe
  and effective within 6 hours (PROACT II)
• Combined IV/IA may be more effective than
  IV t-PA (Interventional Management of
  Stroke -IMS)
• Mechanical and laser catheter
  technologies are showing great promise
  (Angio-Jet)
Stroke: The Challenge

• Only 1-3% of all stroke victims receive
  treatment with tPA in the US
• 25% of Acute MI patients receive treatment
  (lytics or PTCA) in the US
• Mean time to presentation
  – AMI: 3hrs
  – Acute Stroke: 4-10hrs
• 24-59% patients present within 3 hours
• 40-76% patients present within 6 hours

Reasons for lack of treatment:
1. Patient’s inability to recognize stroke symptoms
   – 40% of stroke patients can’t name a single sign
       or symptom of stroke or stroke risk factor.
   – 75% of stroke patients misinterpret their
       symptoms
   – 86% of patients believe that their symptoms
       aren’t serious enough to seek urgent care
1. Physician’s lack of experience with stroke
    treatment and therefore reluctance to “risk”
    treatment
2. Lack of organized delivery of care in many
    medical centers throughout the country.

Prehospital Management and Field Treatment

• New: Activation of the 9-1-1 systems by patients or other
  members of the public is strongly supported because it
  speeds treatment of stroke (Class I, Level B)

• New: To increase the number of patients who can be seen
  and treated within the first few hours after stroke,
  educational programs to increase public awareness of stroke
  are recommended (Class I, Level B)

• New: To increase the number of patients who are treated,
  educational programs for physicians, hospital personnel, and
  EMS personnel also are recommended (Class I, Level B)

• New : Brief assessment by EMS personnel are recommended

• New :The use of a stroke algorithm is encouraged (Class I,
  Level B)

Prehospital Cont’d
• New: The panel recommends that EMS personnel begin the
  initial management of stroke in the field (Class I, Level B)

• New : The development of stroke protocols to be used by
  EMS personnel is strongly encouraged. Patients should be
  transported rapidly for evaluation and treatment to the
  closest institution that provides emergency stroke care as
  described in the statement (Class I, Level B)

• New: Telemedicine can be an effective method to extend
  acute stroke care to rural areas. (Class IIb, Level B)

Designation of Stroke Centers
• The creation of PSCs is strongly recommended (Class I, Level
  B). The organization of such resources will depend on local
  variable.
• The development of CSC is recommended (Class I, Level C)
• Certification of stroke centers by an external body, such as
  JCAHO, is encouraged (Class I, Level B)
• For patients with suspected stroke, EMS should bypass
  hospital that do not have resources to treat stroke and got
  the closest facility capable of treating acute stroke (Class I
  Level B)

Emergency Diagnosis and Management: Class I
              recommendations

• The goal is to complete an evaluation and decide on
  treatment within 60 minutes of the patient’s arrival in the
  ED. Acute stroke teams are encouraged. (Class I, Level B)
• The use of a stroke rating scale is recommended (e.g., NIH
  Stroke Scale) (Class I, Level B)

• Patients with clinical evidence of acute cardiac or pulmonary
  disease may warrant a CXR (Class I, Level B)

             Recommendations


• New: Most patients with acute stroke do not require a CXR
  as part of their initial evaluation (Class III, Level B)
• Most patients with stroke do not need a CSF exam (consider
  if clinical picture suggestive of SAH and CT head normal)

Brain and Vascular Imaging : Class I
                recommendations
• Imaging of the brain is recommended before initiating
  specific therapy (Class I, Level A)
• In most instances, CT will provide the information to make
  decisions about emergency management (Class I, Level A)
• New: The imaging study should be interpreted by a physician
  with expertise in reading brain CT and MRI (Class I, Level C)

                recommendations


• Some findings on CT, such as a dense artery sign, are
  associated with poor prognosis (Class I, Level A)
• New: Multimodal CT and MRI may provide additional
  information that will improve diagnosis of ischemic stroke
  (Class I, Level A)

Brain and Vascular Imaging: Class II
                Recommendations
• Other than hemorrhage, no specific CT finding should
  preclude use of IV rtPA within 3 hours of stroke onset (Class
  IIb, Level A)
• Vascular imaging is required before intra-arterial or
  endovascular interventions (Class IIa, Level B)

Brain and Vascular Imaging: Class III
                Recommendations

• New: Emergency treatment of stroke should not be delayed
  in order to obtain multimodal imaging studies (Class III, Level
  C)
• New: Vascular imaging studies should not delay treatment of
  acute stroke patients whose symptoms started <3 hours ago
  (Class III, Level B)

General Supportive Care: Class I
                Recommendations
• Airway support and ventilatory assistance are recommended
  for the treatment of acute stroke (Class I, Level C)
• Hypoxic patients with stroke should receive supplemental
  oxygen (Class I, Level C)
• Fever should be treated and antipyretic medications should
  administered to lower temperature in febrile patients (Class
  I, Level C)

• Cardiac monitoring to screen for atrial fibrillation and other
  potentially serious cardiac arrhythmias should be performed
  during the first 24 hours after onset of ischemic stroke (Class
  I Level B)
• The management of arterial hypertension remains
  controversial. It is generally agreed that a cautious
  approach to the treatment of arterial hypertension should
  be recommended (Class I, Level C)

General Supportive Care Cont’d

• Patients who have elevated blood pressure and are
  otherwise eligible for treatment of rtPA may have their
  blood pressure lowered so that their systolic is< 185 mm Hg
  and their diastolic blood pressure is < 110 mm Hg (Class I,
  Level B)
• New: Until other data become available, consensus exists
  that the previously described blood pressure
  recommendations should be followed in patients
  undergoing other acute interventions to recanalize occluded
  vessels, including intra-arterial thrombolysis (Class I, Level C)

General Supportive Care Cont’d
• New : Patients with markedly elevated blood pressure may
  have their blood pressure lowered. A reasonable goal is ~
  15% during the first 24 hours after onset of stroke.
  Medications should be withheld unless SPB > 220 or MAP
  >120 (Class I, Level C)
• The cause of arterial hypotension in the setting of acute
  stroke should be sought (Class I, Level C)

• New: Hypoglycemia should be treated in patients with acute
  ischemic stroke (Class I, Level C). Marked elevations in blood
  glucose should be avoided.

General Supportive Care: Class II
               Recommendations
• New: There is no data to guide selection of medications for
  the lowering of blood pressure in the setting of acute
  ischemic stroke. The recommended medications and doses
  are based on general consensus (Class IIa, Level C)
• New: For patients with preexisting hypertension evidence
  indicates antihypertensive therapy medications should be
  restarted at ~ 24 hours (Class IIa, Level B)
• New: Persistent hyperglycemia (>140 mg/dL) during the first
  24 hours after stroke is associated with poor outcomes
  (Class IIa, Level C)

General Supportive Care: Class III
                Recommendations
• Non-hypoxic patients with acute ischemic stroke do not
  need supplemental oxygen therapy (Class III, Level B
• New: Data on hyperbaric oxygen are inconclusive, and some
  data imply that the intervention may be harmful (Class III,
  Level B)

• Despite the efficacy of hypothermia for improving
  neurological outcomes after cardiac arrest, the utility of
  induced hypothermia for the treatment of patients with
  acute ischemic stroke is not established (Class III, Level B)

Intravenous Thrombolysis: Class I
                Recommendations

• IV rtPA (0.9 mg.kg, maximum dose 90 mg) is recommended
  for selected patients who may be treated within 3 hours of
  onset of ischemic stroke (Class I, Level A)
• New: Besides bleeding complications, physicians should be
  aware of the potential side effect of angioedema that may
  cause partial airway obstruction (Class I, Level C)

Intravenous Thrombolysis: Class II
                Recommendations

• A patient whose BP can be safely lowered with
  antihypertensive agents may be eligible for treatment. (Class
  IIa, Level B)
• New: A patient with a seizure at the time of onset may be
  eligible for treatment. (Class IIa, Level C)

Intravenous Thrombolysis: Class III
                Recommendations

• IV streptokinase for treatment of stroke IS NOT
  recommended. (Class III, Level A)

• New: Use of IV fibrinolytics other than tPA (reteplase,
  tenecteplase, etc) outside a clinical trial in NOT
  recommended. (Class III, Level C)

Intraarterial Thrombolysis: Class I
                  Recommendations

• Intraarterial thrombolysis is an option for major stroke if
  administered within 6 hours of onset.        ( Class I, Level B)

• Intraarterial thrombolysis should only be attempted at
  experienced stroke centers (Class I, Level C)

Intra-Arterial Thrombolysis : Class II – III
                recommendations
• New: Intra-arterial thrombolysis is reasonable in patients
  who have contraindications to use of IV thrombolysis, such
  as recent surgery (Class II, Lvel C)
• The availability of intra-arterial thrombolysis should
  generally NOT preclude the use of IV rtPA in eligible patients
  (Class III, Level C)

Combination Reperfusion Therapies : Class III
              recommendation




• New: Combinations of interventions to restore perfusion
  cannot be recommended outside the setting of clinical trials
  (Class III, Level B)

Anticoagulation: Class III Recommendations

• Urgent anticoagulation for the prevention of early
  recurrence, halting worsening or improving outcomes, is
  NOT recommended. (Class III, Level A)
• Urgent anticoagulation is NOT recommended for moderate
  to severe strokes because of an increased risk of serious
  intracranial hemorrhage. (Class III, Level A)
• Initiation of anticoagulant therapy within 24 h of IV rtPA is
  NOT recommended. (Class III, Level B)

Antiplatelet
         Therapy: Class I Recommendation

• New Dose Added: Oral aspirin (initial dose of 325 mg) within
  24 to 48 hours of stroke onset is recommended for most
  patients (Class I, Level A)

Antiplatelet Therapy: Class III Recommendations

• Aspirin should NOT be considered a substitute for IV rtPA.
  (Class III, Level A)

• Aspirin is NOT recommended within 24 h of IV rtPA. (Class
  III, Level A)

• New: Clopidogrel alone or in combination with aspirin is NOT
  recommended. (Class III, Level C)

• New: Use of IV GPIIbIIIa receptor antagonists outside of
  clinical trials NOT recommended. (Class III, Level B)

Hemodilution : Class III Recommendation

• Hemodilution with or without volume expansion is NOT
  recommended for patients with acute ischemic stroke (Class
  III, Level A)

Vasodilators in Acute Ischemic Stroke

• Methylxanthine derivatives (e.g., pentoxifylline) are
  vasodilators that also inhibit platelet aggregation
• Several studies have evaluated the use of pentoxifylline for
  the reduction of 30-day mortality
• Neither pentoxifylline nor pentofylline has been shown to
  improve outcomes after stroke

Vasodilators: Class III Recommendation


• Medications such as pentoxifylline are NOT recommended
  for patients with acute ischemic stroke (Class III, Level A)

Induced Hypertension Recommendations

Class I:
• New: In exceptional cases, vasopressors may be used to
   improve cerebral blood flow, but this use requires close
   neurological and cardiac monitoring (Class I, Level C)

Class III:
• New: Drug-induced hypertension is NOT recommended for
   most patients with acute ischemic stroke (Class III, Level B)

No Surgical Recommendations

• Safety and effectiveness data about surgical interventions
  for patients with acute ischemic stroke are insufficient to
  make recommendations

Endovascular Intervention : Class II
                Recommendations

• New: Although the MERCI device is a reasonable intervention for
  extraction of intra-arterial thrombi in carefully selected patients,
  the utility of the device in improving outcomes after stroke is
  unclear; additional clinical trials are needed to define its role in
  the emergency management of stroke (Class IIb, Level B)


• The usefulness of other endovascular treatments is not
  established (Class IIb, Level C)

Neuroprotection: Class III Recommendation




• NO intervention with putative neuroprotective actions has
  been established as effective in improving outcomes after
  stroke, and therefore none currently can be recommended
  (Class III, Level A)

Hospitalization: Class I Recommendations


• The use of stroke units incorporating rehabilitation is
  recommended (Class I, Level A)
• SC anticoagulants for prevention of DVT in immobilized
  patients is recommended (Class I, Level A)
• New: The use of standardized stroke care order sets in
  recommended (Class I, Level B)
• Early mobilization to prevent subacute complications of
  stroke is recommended (Class I, Level C)

Hospitalization: Class I Recommendations

• New: Assessment of swallowing before starting eating or
  drinking is recommended (Class I, Level B)

• Patients with pneumonia or UTI should receive antibiotics
  (Class I, Level B).

• Treatment of concomitant medical diseases is recommended
  (Class I, Level C)

• Early institution of interventions to prevent recurrent stroke is
  recommended (Class I, Level C)

Hospitalization: Class II Recommendations

• New: Patients who cannot take food and fluids orally should
  receive NG, ND or PEG feedings to maintain hydration and
  nutrition while undergoing efforts to restore swallowing
  (Class IIa, Level B)

• ASA can be used for DVT prophylaxis but is less effective
  than anticoagulation (Class IIa, Level A)

• Intermittent pneumatic compression devices are
  recommended for patients who cannot receive
  anticoagulation (Class IIa, Level B)

Hospitalization: Class III Recommendations

• New: Nutritional supplements are NOT needed (Class III,
  Level B)

• New: Prophylactic antibiotics are NOT recommended (Class
  III, Level B)
• If possible, placement of an indwelling bladder catheter
  should be avoided because of risk of UTI (Class III, Level C)

Treatment of Neurologic Complications : Class I
              Recommendations

• Patients with major infarctions affecting the cerebral hemisphere or
   cerebellum are at high risk of brain edema and increased ICP. Measures
   to lessen the risk of edema and close monitoring of the patient for
   worsening during the first days are recommended (Class I, Level B) New:
   Transfer of the patient to a center with neurosurgical expertise should
   be considered


• Patients with acute hydrocephalus secondary to ischemic stroke can be
   treated with a ventricular drain (Class I, Level B)

Treatment of Neurologic Complications : Class I
              Recommendations


• Decompressive surgical evacuation of a space-occupying
  cerebellar infarction is a potentially life-saving measure, and
  clinical recovery may be very good (Class I, Level B)
• Recurrent seizures after stroke should be treated as with
  other acute neurological conditions (Class I, Level B)

Treatment of Neurologic Complications : Class II
              Recommendations


• Aggressive medical measures, including osmotherapy, are UNPROVEN
   for management of malignant ischemic cerebral edema (Class IIa, Level
   C). Hyperventilation is short-lived and medical measures could delay
   decompressive surgery.

• Decompressive surgery for malignant edema in the cerebral hemisphere
   may be lifesaving but impact on morbidity is unknown. For severely
   afflicted patients, advice about the possibility of life with severe
   disability should be given to the family (Class IIa, Level B)

• New: No specific recommendation is made for treatment of
  asymptomatic hemorrhagic transformation (Class IIb, Level
  C)

  • 2. Prevalence of stroke • 1 stroke very 53 seconds • 1 death from stroke every 3.3 minutes (436/day) • 750,000 new and recurrent strokes each year in US • Mortality – 7.6% 30d – 16-23% 3 months • Yellowstone County – 310 strokes expected
  • 3. Stroke is a treatable condition. • IV tPA is approved for use within 3 hours (NINDS) • Intra-arterial therapy has proven to be safe and effective within 6 hours (PROACT II) • Combined IV/IA may be more effective than IV t-PA (Interventional Management of Stroke -IMS) • Mechanical and laser catheter technologies are showing great promise (Angio-Jet)

Stroke: The Challenge • Only 1-3% of all stroke victims receive treatment with tPA in the US • 25% of Acute MI patients receive treatment (lytics or PTCA) in the US • Mean time to presentation – AMI: 3hrs – Acute Stroke: 4-10hrs • 24-59% patients present within 3 hours • 40-76% patients present within 6 hours Stroke 2007; 38(5): 1655-1711

Patient’s inability to recognize stroke symptoms – 40% of stroke patients can’t name a single sign or symptom of stroke or stroke risk factor. – 75% of stroke patients misinterpret their symptoms – 86% of patients believe that their symptoms aren’t serious enough to seek urgent care 1. Physician’s lack of experience with stroke treatment and therefore reluctance to “risk” treatment 2. Lack of organized delivery of care in many medical centers throughout the country.   • 14. Designation of Stroke Centers • The creation of PSCs is strongly recommended (Class I, Level B). The organization of such resources will depend on local variable. • The development of CSC is recommended (Class I, Level C) • Certification of stroke centers by an external body, such as JCAHO, is encouraged (Class I, Level B) • For patients with suspected stroke, EMS should bypass hospital that do not have resources to treat stroke and got the closest facility capable of treating acute stroke (Class I Level B) Stroke 2007; 38(5): 1655-1711
  • 15. Emergency Diagnosis and Management: Class I recommendations • The goal is to complete an evaluation and decide on treatment within 60 minutes of the patient’s arrival in the ED. Acute stroke teams are encouraged. (Class I, Level B) • The use of a stroke rating scale is recommended (e.g., NIH Stroke Scale) (Class I, Level B) • Patients with clinical evidence of acute cardiac or pulmonary disease may warrant a CXR (Class I, Level B) Stroke 2007; 38(5): 1655-1711

General Supportive Care Cont’d • Patients who have elevated blood pressure and are otherwise eligible for treatment of rtPA may have their blood pressure lowered so that their systolic is< 185 mm Hg and their diastolic blood pressure is < 110 mm Hg (Class I, Level B) • New: Until other data become available, consensus exists that the previously described blood pressure recommendations should be followed in patients undergoing other acute interventions to recanalize occluded vessels, including intra-arterial thrombolysis (Class I, Level C) Stroke 2007; 38(5): 1655-1711
  • 25. General Supportive Care Cont’d • New : Patients with markedly elevated blood pressure may have their blood pressure lowered. A reasonable goal is ~ 15% during the first 24 hours after onset of stroke. Medications should be withheld unless SPB > 220 or MAP >120 (Class I, Level C) • The cause of arterial hypotension in the setting of acute stroke should be sought (Class I, Level C) • New: Hypoglycemia should be treated in patients with acute ischemic stroke (Class I, Level C).   • 26. General Supportive Care: Class II Recommendations • New: There is no data to guide selection of medications for the lowering of blood pressure in the setting of acute ischemic stroke. General Supportive Care: Class III Recommendations • Non-hypoxic patients with acute ischemic stroke do not need supplemental oxygen therapy (Class III, Level B • New: Data on hyperbaric oxygen are inconclusive, and some data imply that the intervention may be harmful (Class III, Level B) • Despite the efficacy of hypothermia for improving neurological outcomes after cardiac arrest, the utility of induced hypothermia for the treatment of patients with acute ischemic stroke is not established (Class III, Level B) Stroke 2007; 38(5): 1655-1711

  • 31. Intraarterial Thrombolysis: Class I Recommendations • Intraarterial thrombolysis is an option for major stroke if administered within 6 hours of onset. Intra-Arterial Thrombolysis : Class II – III recommendations • New: Intra-arterial thrombolysis is reasonable in patients who have contraindications to use of IV thrombolysis, such as recent surgery (Class II, Lvel C) • The availability of intra-arterial thrombolysis should generally NOT preclude the use of IV rtPA in eligible patients (Class III, Level C) Stroke 2007; 38(5): 1655-1711

  • 34. Anticoagulation: Class III Recommendations • Urgent anticoagulation for the prevention of early recurrence, halting worsening or improving outcomes, is NOT recommended. (Class III, Level A) • Urgent anticoagulation is NOT recommended for moderate to severe strokes because of an increased risk of serious intracranial hemorrhage.   • 35. Antiplatelet Therapy: Class I Recommendation • New Dose Added: Oral aspirin (initial dose of 325 mg) within 24 to 48 hours of stroke onset is recommended for most patients (Class I, Level A) Stroke 2007; 38(5): 1655-1711
  • 36. Antiplatelet Therapy: Class III Recommendations • Aspirin should NOT be considered a substitute for IV rtPA. (Class III, Level A) • Aspirin is NOT recommended within 24 h of IV rtPA.   1. Stroke is a common problem, as I’m sure you’re aware of. Even more common is the question “is this or could this be a stroke” often entertained in the ER or out-patient clinic. It has a significant mortality associated with it, yet people with stroke most often die from cardiac disease. The incidence of stroke significantly increases with age, with a doubling for each decade after age 55.
  2. The recommendations for the approaches to the acute management of ischemic stroke included in these guidelines are listed on this slide. At the top are the recommended approaches to emergency response. Below the top line are the recommendations regarding interventions, both pharmacologic and nonpharmacologic. Lastly, an overall approach to the hospitalized patient, including the management of the most common complications, are included.
  3. Recent data indicate that 29% to 65% of patients with signs or symptoms of acute stroke access their initial medical care via local EMS with confirms the role of EMS in the chain of survival . 1-9 Although just over half of all stroke patients use EMS access to health care, those who do utilize EMS comprise the majority of patients within the 3-hour window. 9-12 The benefits of EMS activation by patients with stroke symptoms appear to occur in both the prehospital and in-hospital settings. Hospital arrival is faster for patients who use EMS/9-1-1 as their initial medical contact than for those who contact their primary physician or hospital directly. EMS use is strongly associated with decreased time to initial physician examination, initial tomography (CT) imaging, and neurological evaluation. 12 On the basis of the information mentioned above, communities should encourage 9-1-1 activation and use for patients with symptoms of acute stroke. Public educational programs likely will increase the proportion of patients with stroke who will utilize EMS as their first contact with the healthcare system. 1. Data from the Temple Foundation Stroke Project controlled trial indicate that educational interventions on stroke identification and management targeting patients, EMS, hospitals, and community physicians increased thrombolytic use in patients with ischemic stoke from 2.21% to 8.65% as compared with communities that did not have such programs, which saw only a 0.06% increase. The benefit form this aggressive intervention program was sustained at 6 months after intervention. 1,2 4. EMS should have protocols in place to rapidly assess, treat, and transport patients. EMS assessment should begin with the initial 9-1-1 contact. The role of the dispatch of appropriate EMS providers when acute stroke is suspected by either the caller or the dispatcher. 3 Data from 2 systems indicate that dispatchers correctly suspected or identified 52 % of patients ultimately proven to have had a stroke on initial telephone evaluation. These data imply that educational programs should be aimed at dispatchers to increase their awareness of stroke symptoms. 4,6 Stroke should be given a priority dispatch similar to that for acute myocardial infarction or trauma. EMS providers should obtain a focused history and patient assessment. Critical elements of the patient’s history must include information on time of symptom onset. After the patient&apos;s airway, breathing, and circulation are assessed and stabilized, common presenting signs of stroke should be sought and a focused examination completed. 5. Prehospital stroke assessment tools have proved effective in identifying stroke patients in the field. The Los Angeles Prehospital Stroke Screen uses patient history, physical findings, and finger glucose determination to identify stroke patients.   5. 6. The objectives of the EMS phase of stroke care should be to have protocols in place to rapidly assess, treat, and transport patients. The objectives of the EMS phase of stroke care are as follows: 1. rapid identification of stroke as the cause of the patient’s findings, 2. elimination of co morbid conditions that could mimic stroke, 3. stabilization, 4.rapid transportation of the patient to the closest appropriate ED, and 5. notification of the receiving institution about impending arrival of a patient with a suspected stroke. To maximize therapeutic options, treatment guidelines and transfer protocols should be established in advance to ensure orderly patient transition from a prehospital to a hospital environment. 1-5 7. The availability of resources to care for patients with acute stroke varies widely both among and within communities. The National Institutes of Health (NIH) Task Force report, “Improving the Chain of Recovery for Acute Stroke in Your Community,” recommends identifying hospitals capable of providing acute stroke care and creating a transport system to these centers based on patient location. Such systems require advanced planning and frequent updating and should incorporate EMS representatives, community leaders, hospitals, and physicians to ensure clear guidance for EMS providers with regard to patient destination. 6 Frequency and accuracy of prehospital diagnosis of acute stroke. In the attempt to improve the organization and delivery of care to stroke patients the Brain Attack Coalition published 2 sets of recommendations, one for primary stroke centers (PSCs) and, more recently one for comprehensive stroke centers (CSCs). A PSC has the personnel, programs, expertise, and infrastructure to care for many patients with uncomplicated strokes, uses many acute therapies (such as intravenous rtPA), and admits such patients into a stroke unit. 1,2 The utility of a CSC is beginning to emerge. The CSC is designed to care for patients with complicated types of strokes, patients with intracerebral hemorrhage or subarachnoid hemorrhage, and those requiring specific interventions (eg, surgery or endovascular procedures) or an intensive care unit type setting. Studies show that a CSC increases the use of lytic agents and that a CSC may improve overall care and outcomes. In-hospital death rates were reduced by almost 50% in hospitals with a vascular neurologist and were reduced by 24% in those with a stroke team. 3,4 3. The certification or designation of some hospitals as PSCs or CSCs is progressing rapidly. The American Stroke Association convened an expert panel to study this issue for PSCc, with the conclusion that a variety of certification processes might be developed and lead to improved care and outcomes. One study showed that self-certification was likely to lead to a significant overestimation of a hospital’s compliance with published for a PSC. Thus, these data and anecdotal experience suggest that outside independent evaluations of hospitals as stroke centers should lead to more accurate assessment of a facility’s true capabilities. 5,6 4. The preferential routing of acute stroke patients to a PSC has been demonstrated to increase the proportion of patients care for at stroke-capable centers and to increase the proportion of patients treated with thrombolytic therapy to &gt;10%. 7 Direct routing of stroke patients whose symptoms started &lt;3 hours ago to a PSC or a CSC has been implemented or is in the process of implementation in 7 states covering &gt;25% of the US population. Stroke centers should be viewed as a large support network sometimes referred to as a stroke system of care.   8. Hypoglycemia can cause stroke-like symptoms and is an important stroke mimic. Determination of the platelet count and PT/PTT is important to avoid hemorrhagic complications. For patients on warfarin, IV thrombolysis can be considered if the INR is ≤1.5. An ECG is recommended since cardiac disorders are frequent in stroke patients such as cardiac ischemia or atrial fibrillation.

  13. Although mutimodal CT and MRI can provide additional information, time is of the essence in acute stroke treatment. Further, it has not been shown that vascular imaging patterns affect response to treatment. Therefore, it is recommended that treatment not be delayed in order to achieve these other imaging tests.
  14. 1 and 2. Airway support is needed for patients who have decreased consciousness or who have bulbar dysfunction causing compromise of the airway. Maintaining adequate tissue oxygenation is important in the setting of acute cerebral edema. The goals are to prevent hypoxia and potential worsening of the brain injury. The most common causes of hypoxia are partial airway obstruction, hypoventilation, aspiration pneumonia, and atelectasis. 1,2,3 After ischemic stroke, some patients develop Cheyne-Stokes pattern of respiration, with decreases in oxygen saturation that can be readily reversed with oxygen supplementation. 4 The results of a controlled study do not support the use of supplemental oxygen at 3 L/min for most patients with acute stroke. However, patients with acute stroke should be monitored with pulse oximetry with a target oxygen saturation level &gt; 92%. 4,5 3. Increased body temperature (fever) in the setting of acute ischemic stroke is associated with poor neurological outcome (increased risk of morbidity and mortality), possibly secondary to increased metabolic demands, enhanced release of neurotransmitters, and increased free radical production. The source of any fever should be ascertained.   15. 4. General agreement supports the use of cardiac monitoring for screening of serious cardiac arrhythmias. The most common arrhythmia detected in the setting of stroke is atrial fibrillation, which either may be related to the cause of stroke or may be a complication. 1,2 No clinical trials have tested the utility of cardiac monitoring for most patients with ischemic stroke or the use of cardiac protective agents or medications to prevent serious cardiac arrhythmias. 5.Data to guide recommendations for treatment are inconclusive or conflicting regarding the management of arterial hypertension in the setting of an acute stroke. Many patients have an elevated blood pressure in the first hours after stroke. Elevations in blood pressure &gt;160 mm Hg are detected in &gt;60% of patients with acute stroke. Both elevated and low blood pressures are associated with poor outcome after stroke. 3 Roe every 10-mm Hg increase &gt; 180 Hg, the risk of neurological deterioration increased by 40% and the risk of poor outcome increased by 23%. The elevation in blood pressure may be secondary to the stress of cerebrovascular event, a full bladder, nausea, pain, per-existing hypertension, a physiological response to hypoxia, or a response to increased intracranial pressure. 4,5 Many patients have spontaneous declines in blood pressure during the first 24 hours after the onset of stroke. Although severe hypertension may be considered an indication for treatment, no data define the levels of arterial hypertension that mandate emergency management. 6 However, the aforementioned data suggest that the systolic blood pressure level that would prompt treatment would be &gt;180 mm Hg. A systolic blood pressure &gt;185 mm Hg or a diastolic blood pressure &gt;110 mm Hg is a contraindication to intravenous administration. 7,8 1. If medications are given to lower blood pressure, the clinician should be sure that the blood pressure is stabilized at the lower level before treating with rtPA and maintained below 180/105 mm Hg for at least 24 hours after intravenous rtPA treatment. Because the maximum interval from stroke onset to until treatment with rtPA is short, many patients with sustained hypertension above recommended levels cannot be treated with intravenous rtPA. 1,2,3 7. Research testing the effects of early treatment of arterial hypertension on outcomes after stroke is under way. The level of blood pressure that would mandate such treatment as mentioned above is not known, but consensus exists that medications should be withheld unless the systolic blood pressure is &gt;220 mm Hg or the mean blood pressure is &gt;120 mm Hg. 1 9.Persistent arterial hypotension is rare among patients with acute ischemic stroke, but it is associated with an increased likelihood of an unfavorable outcome. 2,3 One study noted that the rates of neurological worsening, poor neurological outcomes, or death increased when the baseline systolic blood pressure was &lt;100 mm Hg or the diastolic blood pressure was &lt;70 mm Hg. The cause of hypotension should be sought; among the potential causes are aortic dissection, volume depletion, blood loss, and decreased cardiac output secondary to myocardial ischemia or cardiac arrhythmias. 10.   18. 1 and 2. The recommended medications and doses included in the new guidelines are based on general consensus for lowering BP. These recommendations can be found in table 10, page 1671 of the new guidelines. Arterial hypertension is a recognized risk factor for stroke and recurrent stroke. Many patients were taking medications before their stroke or are found to have sustained hypertension after their stroke. These patients will need long-term antihypertensive treatment. Limited data is available on the timing of instituting this therapy. The timing of the reinstitution of treatment and the selection of medications will depend on the patient’s neurological status, the underlying stroke mechanism, the patient’s ability to swallow medications, and the presence of concomitant diseases. 1,2 Most patients with mild to moderate strokes who are not at high risk for increased intracranial pressure may have prestroke antihypertensive medications restarted 24 hours after their vascular event. 3 The panel strongly endorses clinical research that will provide information about the safety and efficacy of restarting antihypertensive therapy among patients with stroke. 1 1.   19. Most patients with acute stroke do not need supplemental oxygen; however, if pulse oximetry or blood gas determination indicates hypoxia, oxygen should be administered. Patients with acute stroke should be monitored with a target oxygen saturation level &gt; 92%. 1,2 Studies that have tested the use of hyperbaric oxygen in stroke have been inconclusive or have shown that the intervention does not improve outcome. 3 Data from a small trial suggest that hyperbaric oxygen therapy might be harmful. 3 A systematic review found no evidence that hyperbaric oxygen improved outcomes after stroke or brain injury. 4 At present, data do not support the routine use of hyperbaric oxygen in the treatment of patients. Hypothermia has been shown to be neuroprotective in experimental and focal hypoxic brain injury models. Hypothermia may delay depletion of energy reserves, lessen intracellular acidosis, slow influx of calcium into ischemic cells, suppress production of oxygen free radicals, and lessen the impact of excitatory amino acids. 5 Several small clinical studies have evaluated the feasibility of inducing modest hypothermia for treatment of patients with acute ischemic stroke. The panel still recommends, that patients with BP above the eligibility cut-off for IV rtPA may still be treated if their BP can be lowered safely with antihypertensive agents and still meet the 3 hour window. Patients requiring nitroprusside for BP lowering, however, may not be sufficiently stable to receive treatment with rtPA and stability of BP should be assessed prior to treating with rtPA. This is based on experience of the NINDS tPA Stroke Trial. 1 The NINDS protocol explicitly stated that seizure at onset excluded patients from receiving rtPA. This is based on the data from two large, randomized trials (IST and CAST). 1,2 In this version of the guidelines, however, the panel has recommended a dose of 325 mg daily acutely. In the CAST study, 160 mg/daily was used and in the IST, 300 mg was used. Unchanged from the previous versions of the guidelines is the recommendation that aspirin should not be administered instead of other acute interventions, such as IV rtPA and should not be administered within 24 h of IV rtPA. In this version of the guidelines, however, the panel has recommended against using clopidogrel alone or in combination with aspirin as an acute treatment due to a lack of date in acute ischemic stroke patients and a potential for harm.   32. THE PRIMARY MESSAGE: The best management of blood pressure in patients with acute ischemic stroke remains controversial. The data in support of induced hypertension is from small and preliminary studies. Induced hypertension can be achieved using medications such as phenylephrine, withdrawing previous antihypertensive therapy, or augmenting intravenous fluids. In some studies the induced hypertension is intended to achieve a target systolic blood pressure or mean arterial pressure of 20% to 30% above baseline. As part of the treatment paradigm, serial neurological examinations are performed to determine whether neurological improvement occurs; patients who show no improvement in a 30- to 60-minute period are considered nonresponders, and the intervention is stopped. Patients who respond to this therapy are maintained at that level and weaned according to neurological examination and functional imaging. Preliminary and small clinical studies suggest that drug-induced hypertension could be used in the management of some patients with acute ischemic stroke. The authors reviewed many of the previously promising compounds that failed including nimodipine, NMDA receptor antagonists, lubeluzole, clomethiazole, naloxone, citicoline, and most recently, NXY-059. In addition, they reviewed still promising therapies including hypothermia, but concluded none to be ready for “prime time”.
