2. QUE LE SUGIERO A MI PACIENTE: ???
Lo opero ???
No lo opero ???
LE Pido TAC, PET O US TE ???
Lo mando a radioterapia ???
Le sugiero Quimio ???
O mejor RT/QT ???
Y después lo opero ???
Vale la pena????

Neoadjuvante/Adjuvante/Concomitante
Ventajas

Relative change in incidence of esophageal adenocarcinoma and other malignancies (1975–2001). Data from the National Cancer Institute ’ s Surveillance, Epidemiology, and End Results program with age-adjustment using the 2000 U.S. standard population. Baseline was the average incidence between 1973 and 1975. Solid black line = esophageal adenocarcinoma; short dashed line = melanoma; line = prostate cancer; dashed line = breast cancer; dotted line = lung cancer; dashes and dotted line = colorectal cancer.
Histology and esophageal cancer incidence (1975–2001). Data from the National Cancer Institute ’ s Surveillance, Epidemiology, and End Results program with age-adjustment using the 2000 U.S. standard population. Solid black line = adenocarcinoma; dashed line = squamous cell carcinoma; dotted line = not otherwise specified.
Anatomic location and esophageal cancer incidence (1975–2001). Data from the National Cancer Institute ’ s Surveillance, Epidemiology, and End Results program with age-adjustment using the 2000 U.S. standard population. Solid black line = lower third of the esophagus; short dashed line = middle third of the esophagus; long dashed line =upper third of the esophagus; dotted line = not clearly categorized.
Trends in incidence of adenocarcinoma of the cardia and the esophagus (1975–2001). Data from the National Cancer Institute ’ s Surveillance, Epidemiology, and End Results program with age-adjustment using the 2000 U.S. standard population. Solid black line = adenocarcinoma of the esophagus; dotted line = adenocarcinoma of the gastric cardia.
Trends in stages of esophageal adenocarcinoma at diagnosis (1975–2001). Data from the National Cancer Institute ’ s Surveillance, Epidemiology, and End Results program with age-adjustment using the 2000 U.S. standard population. Dotted line = distant; dashed line = regional; solid black line = localized; line = in situ.
Disease-specific mortality and incidence of esophageal adenocarcinoma (1975–2001). Data from the National Cancer Institute ’ s Surveillance, Epidemiology, and End Results program with age-adjustment using the 2000 U.S. standard population. Baseline was the average incidence (or mortality) between 1973 and 1975. Line = incidence; solid black line = mortality.
Figure 1a. T4 adenocarcinoma of the midesophagus with invasion into the thoracic aorta. (a) Coronal fused PET/CT image shows a primary FDG-avid tumor in the midesophagus (arrowhead). (b) Axial contrast-enhanced CT image obtained at the level of the midesophagus shows marked esophageal wall thickening (arrowhead), which corresponds to the primary tumor. In addition, there is loss of the normal fat plane between the esophagus and thoracic aorta with tumor extension into the periesophageal fat. Note the broad interface with the aorta (arrow), a finding suggestive of locoregional invasion. (c, d) Axial PET (c) and fused PET/CT (d) images obtained at the same level as b show marked FDG uptake in the primary tumor (arrowhead) and in the component of adventitial invasion (arrow). Aortic invasion by the tumor was confirmed at attempted esophagectomy.
Figure 2. Kaplan–Meier Plots of Estimated Overall 5-Year Survival. Panel A shows a Kaplan–Meier plot of the estimated overall 5-year survival among patients with esophageal or esophagogastric-junction cancer who underwent neoadjuvant chemoradiotherapy (CRT) followed by surgery (178 patients) or surgery alone (188), according to an intention-to-treat analysis. Panel B shows a Kaplan–Meier plot of the estimated overall 5-year survival among the 134 patients with adenocarcinoma (AC) treated with neoadjuvant chemoradiotherapy followed by surgery and the 141 treated with surgery alone, and the 41 patients with squamous-cell carcinoma (SCC) treated with chemoradiotherapy followed by surgery and the 43 treated with surgery alone, according to an intention-to-treat analysis. Other tumor types were excluded from this analysis.
