2. INTRODUCTION
Social determinants of health are conditions in the environments in which
people are born, live, learn, work, play, worship, and age that affect a wide
range of health, functioning, and quality-of-life outcomes and risks. Conditions
(e.g., social, economic, and physical) in these various environments and settings
(e.g., school, church, workplace, and neighborhood) have been referred to as
“place”.In addition to the more material attributes of “place,” the patterns of
social engagement and sense of security and well-being are also affected by
where people live. Resources that enhance quality of life can have a significant
influence on population health outcomes. Examples of these resources include
safe and affordable housing, access to education, public safety, availability of
healthy foods, local emergency/health services, and environments free of life-
threatening toxins.
3. Why emphasize social determinants?
1. Social determinants of health have a direct impact on health.
2. Social determinants predict the greatest proportion of health status
variance(health inequity).
3. Social determinants of health structure health behaviours.
4. Social determinants of health interact with each other to produce health.
Why Social determinant of health?
As per Healthy People 2020 ,Phase I report defined these terms as follows:
• The social environment is the aggregate of social and cultural institutions,
norms, patterns, beliefs, and processes that influence the life of an individual
or community.
• The physical environment , which comprises both the natural and built
environments, is the structure and function of the environment and how it
impacts health
Social determinants refers to both the social and the physical
environmental realms
4. Healthy People 2020 Approach to Social Determinants of Health
A “place-based” organizing framework, reflecting five (5) key areas of social determinants
of health (SDOH), was developed by Healthy People 2020.
Economic
Stability
Education Social and
Community
Context
Health and Health
Care
Neighborhood and
Built Environment
1.Employment 1.Early Childhood
Education and Development
1.Civic Participation 1.Access to Health Care 1.Access to Foods that
Support Healthy Eating
Patterns
2.Food Insecurity 2.Enrollment in Higher
Education
2.Discrimination 2.Access to Primary Care 2.Crime and Violence
3.Housing Instability 3.High School Graduation 3.Incarceration 3.Health Literacy 3.Environmental Conditions
4.Poverty 4.Language and Literacy 4.Social Cohesion 4.Quality of Housing
Each of these five determinant areas reflects a number of key issues that make up the
underlying factors in the arena of SDOH:
These five key areas (determinants) include:
1. Economic Stability
2. Education
3. Social and Community Context
4. Health and Health Care
5. Neighborhood and Built Environment
5. Social Environments that Affect Health:
Social Economic Conditions
Physical Material Conditions
Built Environments
Social Environments
Social Institutions/Policies
Psycho Social Environments
6. CONCLUSION
human health is determined by the interaction of several
factors, including the social environment, genetic
inheritance, and personal behaviors. Socioeconomic
status, race/ethnicity, social networks/social support, and
the psychosocial work environment all have been shown
to affect health outcomes .These social determinants
influence health at multiple levels throughout the life
course. In addition to the vast array of social determinants
that influence health, a person inherits a complete set of
genes from each parent that contributes both directly and
indirectly to the pathogenesis of disease.
