A360 revolutionizes the way adolescent girls access contraceptives by flipping the model and starting with what matters to her. In Tanzania, A360 has co-designed with girls a program called Kuwa Mjanja, where girls can "be smart" (Swahili translation) and learn about themselves both their bodies and their dreams. Kuwa Mjanja inspires and opens possibilities, empowering girls to take action by considering contraception as "the first step to taking control of your future now." Connecting with Sara (CwS) is a DHIS2-powered, open-source platform to track, link, and engage with Sara (our client) through use of mobile phones to facilitate engagement with community mobilizers, service providers, and follow-up efforts. CwS enables a new way to engage with and bring service delivery closer to Sara. The platform powers program teams to engage more dynamically with Sara and to actively use data to inform pathways to method adoption.

Reaching People is currently managing the Health+ Project, an initiative funded by NHS charities with the aim of improving digital health provision for over 50s in Leicester, Leicestershire and Rutland. The work is mainly focused on engaging GP surgeries to host Health+ digital surgeries for the over 50’s population. Patients are supported to gain digital skills confidence so that they can go online to book appointments, order repeat prescriptions and find health information. Friday Chidlow, project manager for Health+, will provide an overview of how the project is supporting patient’s digital literacy, share the reflections of Health+ volunteer’s on their health literacy training and provide details of plans to train social prescribers in health literacy and extend training to GP surgery staff.

Design is shaping the world. Many examples illustrate that designers can change the world for the better. We are entrusted with more and more complex challenges and develop ever-novel approaches. Nowadays a plethora of design discipline exists and it seems fitting to clarify what it is that unites the designers today to then discuss how we can connect design to other fields better in order to maximise the impact of our work. One field that is proving a particularly rich complement to design is Systems Thinking, which essentially studying the interdependencies between elements in complex structures. While design thinking is a bottom up approach, systems thinking can supply the big picture context. Combining the two allows us to be more targeted in how we apply our design efforts.

The Supply Chains for Community Case Management project (SC4CCM), managed by JSI with funding from the Bill & Melinda Gates Foundation, partnered with three district health teams in Rwanda to implement Supply Chain Quality Improvement Collaboratives (QCs), establishing and training a network of 44 Quality Improvement Teams (QITs). Teams comprised CHWs and health center staff who identified team-led solutions for improving community resupply procedures. The QITs used a systematic approach to identify key challenges, set targets, and test their own innovations to address performance gaps. Thoracotomy is the surgical procedure with an incision made to access the pleural space and the contents of the thoracic cavity. Given the structures to be accessed with in the cavity, different incisions have been established to easy the procedure and these incisions qualify the types of thoracotomies to be studied hereunder. PRE OP OPTIMIZATION FOR SPECIFIC FACTORS The outcome of surgical procedures is not measured only by clinical end points but also shorter stays and lower costs. Patients’ discharge is delayed commonly due to inadequate pain relief, infection, arrhythmias, prolonged air leak and debility. Many complications that occur from thoracic operations can be anticipated. An aggressive preoperative work up mitigates morbidity and shortens convalescence. APPROACH CONSIDERATIONS There are about three principles that can guides the choice of the thoracotomy incision to be used I. Adequate exposure must be achieved. The choice of incision is aided by a thorough understanding of the surface anatomy and a comprehensive review of the radiographic images that are obtained preoperatively. II. Chest-wall function and appearance should be preserved to the extent possible. This principle include non-spreading video-assisted thoracoscopic surgery (VATS) procedures, muscle-sparing techniques, avoidance of excessive rib retraction, and rib preservation when possible. III. The third principle is that closure must be meticulous and appropriate. Strict layered closure is the rule for thoracic surgical incisions. Every effort should be made to approximate the individual divided chest-wall muscles in appropriate layers; otherwise, a significant delay in the recovery of range of motion (ROM) may result. Care must be taken to avoid over approximating the ribs and to prevent an override; this will help minimize postoperative pain. POST-OPERATIVE CARE AND MONITORING
