Successful PSM implementation requires either adaptations to the
local culture or change in the culture. Therefore, cultural differences
must first be recognized, followed by development of a PSM
implementation strategy that takes these differences into account.
Inability to culturally adapt the effort can result in an organization
that does not fully realize the benefits of Process Safety
Management.

Reliance on the low personal injury rate at Texas City as a safety indicator failed to provide a true picture of process safety performance and the health of the safety culture.
A “check the box” mentality was prevalent at Texas City, where personnel completed paperwork and checked off on safety policy and procedural requirements even when
those requirements had not been met.
BP Texas City lacked a reporting and learning culture. Personnel were not encouraged to report safety problems and some feared retaliation for doing so. The lessons from
incidents and nearmisses, therefore, were generally not captured or acted upon. Important relevant safety lessons from a British government investigation of incidents at
BP’s Grangemouth, Scotland, refinery

The approach that WXPERT always propose to companies is to adopt a Process Safety
Management based on Risk
For the purpose of this WEBINAR I am working with a RBPS approach of 20 Elements,
however my advise to Organizations is to be sure that you have the main pillars in place to
develop you proper elements in case you would not like to use the CCPS RBPS model
Main Pillars are:
1. Commitment to Process Safety (The most important one typically
treated one the system is already running)
2. Understanding of Hazard and Risks (Not many people understand
the difference)
3. Risk Management in Place (the complete process)
4. Learn from Experiences
Understanding the risk associated with an activity requires answering the following
questions:
1. What can go wrong?
2. How bad could it be?
3. How often might it happen?
Based upon the level of understanding of these answers, a company can decide what
actions, if any, are needed to eliminate, reduce, or control existing risk.
