ED PILOT PROGRAM
• Recruit children who are seen in ED due to asthma
• Piloting program to use CHWs and address housing-
based hazards and triggers

Community Survey) to better understand diabetes and
related behaviors

– Identify individuals most at risk for developing
heart disease
