operating flap gate manually for change over,FG
handle slipped and his forehead slightly touched with
Flap gate operating handle which caused minor injury

bandage.Finally he was taken to NTPC hospital for
checking and released giving proper medicines.As
remedial measure all FGs are to be checked and
serviced for normal operation within two months
period (30.06.2019).Peptalk given to all workers
working in that area.The agency supervisors

NTPC The Ash Extraction transformer-6B tripped on
winding temperature protection.On subsequent
checking,it was found that the corrugated sheet(01
no)& insulation cladding sheet(05 no)had fallen on
the transformer body/yard.The corrugated sheet &
the cladding got dislodged from ESP-6D pass near
ESP roof due to severe wind during FANI cyclone.No
injury or damage to the equipment had
occurred.Immediately PTW was taken with isolation of
transformer breaker and area was barricaded to
restrict access to the area.Fallen sheets

project area ,from truck by hydra crane ,in the mean
time support post became bent from truck bed and
pipes were slipped from truck.No casuality/injury has
happened.Immediate actions taken-Next time onwards
the support post will be checked before unloading and
proper supervision to be ensured.PEP talk was given to
workers to pay attention during unloading work and to
report any unsafe condition.

At “0” mtrs area below 17 mtrs ESP inlet duct of Unit-
3,minor fire broke out in one DA cylinder outlet.Due to
development of very small amount of leakage in the
flash back arrestors resulting in backward propagation
of flame from torch to cylinder and thereby catching fire
at the outlet of the cylinder.No one is directly
responsible in this incident as all safety precautions
were in place.Immediates actions taken-1.Stopping of
work in nearby area.2.Cordoning the area upto 10mtrs
on all sides by safety tape.3.Calling of fire
personnel.4.After stoppage of fire,replacement of faulty
flashback arrestors with good ones.

Sri Maguni Chandra Pradhan, agency worker of CHP
lighting group, age 52 of M/s-Tech Support was
walking up on a slant walkway beside conveyor 8B in
TP-6.He saw a gas cylinder sliding down from a
distance on the same walkway. The cylinder passed
beside him and landed on the floor. While trying to save
himself he collided with handrails and narrowly
escaped. The agency personnel of M/s-Thejo
Engineering who were engaged in gas cutting and
maintainance job were counselled to properly secure
gas cylinders and tools at work place. They were also
given pep talk to block the staircase/slant walkways
during shifting of cylinders on it.

Sri Lokanath Sahoo,a worker in upkeeping of main
plant office was coming from ground floor through lift
number 2 of stage-2 service building at 14.30hrs.The
lift got stuck at 3rd floor with door closed and remain
inside the lift for more than 40 minutes.He could not
contact outside over mobile,as no signal was
available.No intercom phone was provided inside lift.Sri
B K Sahoo,AGM(O),while coming from lift no-01,had
noticed him and opened the lift door at 3rd floor from
outside by pressing the “UP” button of lift.EMD has
been asked for thorough checking of the lift and
provision of intercom in all lifts inside plant.

coming to the plant by car (Maruti Suzuki Alto)
regd.No.OR 22C 1787,the front left tyre got ruptured
.She lost balance and the car toppled after hitting the
divider.She had put the seat belt on,She escaped
unhurt. FQA department employees were given a pep
talk on road safety and advised to check their vehicles
periodically as per the vehicle manual. Further,everyone
was counselled to visually check the vehicles before
each ride.Tyre pressure need to be checked regularly
with calibrated pressure gauge.

cleaning near the pipe line area.He was nearly escaped
unhurt as a steam pipe line was charged for curing of
inner rubber line of hot water tank of DM-1.MM OFS
was informed for isolation of steam line and insulation
to be provided to the steam pipe line near DM plant

As stated by the driver,Bablu Raut,the truck was
parked by him by the side of the road near to ICH at
about 2pm.He along with the helper took lunch in the
ICH and after return at around 2.30pm found the truck
in the tilted position because of sliding of slidding of
drain wall. Nobody has witnessed when the incident
occurred. He informed his supervisor, who arranged
Hydras and took out the truck at about 5pm.There was
no loss to the truck or of any kind. There after the truck
was driven away by the driver. The driver and helper
were counselled and advised not to park any truck in
area other than the designated parking area and adhere
to all safety norms.

brick paving in the shoulder portion of the road.Near
Evershine Nursery School ,one parent while bringing
his kid to the school tried to stop his Honda Activa
vehicle at the shoulder portion but due to the slippery
nature of the road the vehicle slided and could have
fallen into the drain.Due to the prompt action of the
security present over there,the vehicle along with the
father and kid were stopped from entering drain.The
two wheeler driver was educated not to drive his vehicle
on fungus area.The Township Civil department
informed to cover drain by iron grating/concrete slab

M/s-Rabindra Enterprises working in U#4 O/H of APH
area stored gas cylinders near Boiler ‘0’ mtr beside the
lift.While entering the lift ,the leg of a worker hit one of
the cylinders.As weak coir rope tied the cylinders,the
cylinder fell spreading there.No injury reported.The
agency personnel who were engaged in gas cutting and
maintainance job were counselled to properly secure
gas cylinders and tools at work place.They were also
given pep talk to avoid keeping cylinders near
walkways.

Inside Unit-IV pent house ,a helper named Shri Bharat
Kumar Lakda of grinder Naresh Kumar of M/s-Prabhat
Engg.Works sustained injury in his left leg by a broken
piece of grinder wheel.During cutting,the cutting wheel
got broken due to jerk from the tube.He was taken to
hospital and released after first aid.The grinder escaped
unhurt due to the presence of wheel guard and use of
face shield & other PPEs.Also all the workers at site
were given impromptu safety pep talk.Root cause
analysis is under progress.

Departmental hydra from auto base was engaged to
shift the 15KL oil tank from unit #IV GT bay to
transformer repair bay nearby.The hydra lifted the
tanker and marched backward to move.There was a
small sand heap on the path.While moving on the sand
heap the boom of the hydra hit the mulsifier pipeline
ahead.The hydra operator and the outside co-ordinating
staff overlooked the mulsifier pipeline.The mulsifier pipe
line got damaged.No injury occurred to anybody.The
Mulsifier pipeline isolated and rectification work taken
up.The hydra operators were given a pep talk and

Two agency workers of plant civil department were
working for providing hand rails on ramp Unit-3 boiler
ground floor area.A two feet long channel fell from 15
meter height adjacent to the working area.No injury
occurred.During investigation it was found that the
channel was used for support of cable tray.It fell under
its weight due to rusting.Nobody was found working
near cable tray.Civil work was stopped immediately and
the area was barricaded.EMD was informed to inspect
all the cable tray supports in that area.

During downward movement of a HYWA(Regd.No-
OR06M9537)carrying ash from lagoon-2 on ramp(from
RL139 toRL111)at a chainage 400-500 at A zone
side,driver(Sh.Prasanta Sahoo,age-45) has lost his
control due to brake failure and tilted at RL111.00.In
this incident,driver escaped unhurt.there was no helper
with him.Immediately,manager(AHD-Civil)rushed to the
location and found that the driver came out safely.The
driver and the supervisor of the agency were advised for
safety precautions to be taken during transportation of
ash.Also they were advised to take special care during
movement in the ramp.Safety peptalk was recently
administered on 14.11.19 to all the drivers of M/s-
Durga Condev Pvt.Ltd.Safety peptalk for all the drivers
of M/s-Samal Builders will be done on 19.11.2019.

a precautionary measure he was taken to first aid
centre and discharged.Earlier the driver was given
peptalk on Chemical and PPEs and was given peptalk
for non-recurrance of the incident.

belt while driving.Immediate action taken-The car was
safely parked at the footpath to avoid road obstruction
and glow parking light.The employee was advised to
periodically check the healthiness of his vehicle on
regular basis.

Suresh Ch. Pradhan, due to loss of balance while
descending from stairs ,fell down and got scratch injury

at NTPC Kaniha hospital ,he was released from hospital
and resumed duty at 17.30 hrs.All the contract workers
of the agency and other agencies were given peptalk for
safe use of stairs and use of handrails during climbing
and descending.

While Sri SVS Narashimha Sai,Emp No-104730 on
regular monitoring of sites,Ash water tank stage-2 near
ASPH-2 area was visited by the executive.As going up
the stairs above ash water tank, got bitten by the honey
bee(Near the railing of the stair case) on index finger of
right hand side. Swelling of right hand index finger was
due to sting of honey bee. The person was immediately
given first aid at NTPC hospital and resumed duty after
discharge from hospital. The concerned departments
were informed for removal of bee hives in that area
immediately.
