Case Manager
Provided care management to guardianship, trust and power of attorney clients to assure
optimum care of client.
● Performed comprehensive assessments of individual needs and plan of care with on going
monitoring.
● Worked with clients and families to develop an appreciation of the client's personal
preferences for care, assistance and housing. Researched housing options and assisted
clients with transitioning into appropriate adult family home, assisted and supportive
living settings.
● Arranged for service providers, counseling and other therapies, advised on managing
problems, crisis management, and when necessary, managed contacts with law
enforcement or mental services.
● Held regular meetings with clients, families, GSS staff. Coordinated with GSS financial
staff and assisted clients with developing a budget.
