  2. 2. INCIDENT REPORTS Is a risk management tool used to describe and report UNUSUAL event (falls, medication errors, forgotten treatment, injuries) that occurs to a client, visitor or staff member
  3. 3. TWO FUNCTIONS 1. It informs the facility’s administration of the incident and allows risk management personnel to consider ways to prevent similar occurrences in the future. 2. It alerts the facility’s insurance company to a potential claim and the possible need for investigation.
  4. 4. INFORMATION INCLUDED ON THE IR  The date, exact time, and place the nurse discovered the occurrence.  The person(s) involved in the occurrence, including witnesses.  The exact occurrences witnessed by the nurse (e.g., “Found the client sitting on the floor, client stated that...,”rather than “Client fell.”)  The exact details and time sequence of what happened and the consequences for the persons involved.

INFORMATION INCLUDED ON THE IR  The nurse’s actions to provide care and the results of the nurse’s assessment for injuries and client complaints.  The supervisor o duty who was notified and the time and name of the physician notified; if telephone orders were received from the physician, these should be documented as previously discussed and the orders implemented.
  6. 6. INFORMATION INCLUDED ON THE IR  Never record personal opinions, assumptions, judgments, or conclusions about what happened; point blame; or suggest ways to prevent similar occurrences. Forward the incident report to the designated person defined in the facility’s policy.
