3. Most of us retain a youthful look until we reach our forties and then, slowly
and inevitably, our faces begin to show the marks of aging. Although we
can’t roll the clock back, we can reduce the appearance of aging. And that’s
what most of my patients are eager to do. The people who visit my office are
healthy and in good shape, yet they often say they look older than they feel.
They want a change that seems natural and doesn’t signal“surgical proce-
dure.” Yet many don’t have the time for lengthy recovery after surgery.
That’s why I developed a new approach to the face and neck lift that refresh-
es the skin and eliminates years of aging.
I’m pleased to say that we can help patients achieve their goals because of
recent advances and innovations in surgical techniques and laser technolo-
gy. My approach considers and analyzes the way the face ages, and utilizes a
combination of techniques to improve overall appearance.
As we begin, it’s a good idea to understand how the face. the neck, skin and
the underlying architecture changes as we age, and where we need to focus
attention.

4. AREA 1:
THE NECK
You have probably noticed that the neck is often the first area to show age as
the skin and muscles become slack and sag. Some use unfortunate descrip-
tions like“double chin”and then when the skin gets looser they call it a“turkey
gobbler neck,”or a“wattle.”

We also see changes in the jaw line or jowls, as our skin loses elasticity and
the underlying muscles slacken.

We can blame the thinning ozone layer and the sun for wreaking havoc on our
skin. I find that even patients in their thirties often have sun damage. In
patients over forty, we begin to see more superficial lines, etching and deep
wrinkles as well as dark spots, dryness and a sallow appearance.

The center of the face also disappoints us when our cheeks seem to droop,
and the folds along the mouth, which we call the nasolabial folds, deepen.

APPROCH TO
THE FACELIFT

7. Traditionally, a surgical facelift combined with a neck lift
would be the solution to provide a more youthful and
rested appearance. A facelift tightens loose skin and mus-
cles and that’s good as far as it goes. But a facelift alone
doesn’t accomplish everything that we would like to do. I’ve
developed a new approach with the Three-In-One Facelift
that allows us to focus on the skin, the neck and the center
of the face using techniques that address each area. The
combination procedure generally takes three hours, and the
important advantage for the patient is that every area is
treated during one session. For most people recovery takes
two to three weeks.

8. PART 1:
CO LASER2
THE FRACTIONAL LASER
The“fractional CO2 laser”allows me to resurface the skin to
stimulate the underlying natural collegan that gives the face a
nice full look. It also helps to diminish fine lines, wrinkles, etch-
ing and dark marks on the skin.

9. PART 2:
SMARTLIPO
SmartLipo is new laser option to treat the neck and mild-to-mod-
erate looseness of the skin. I use this for patients who are in their
early forties to mid-fifties and who don’t want a great deal of
surgical downtime. It is now the first step in our 3-in-1 Face Lift.
With SmartLipo the laser heats and tightens the undersurface of
the skin, melts the fat under the surface of the skin, and coagu-
lates or seals small capillaries to lessen bleeding and bruising.
After I use the laser, I perform liposuction to remove the excess fat
in the neck. This process involves three small incisions only – one
in the midline of the chin hidden in a chin crease, and two small
incisions near each earlobe. The recovery and subsequent scar-
ring is much less than the traditional face-neck lift. Patients need
to wear a Velcro“Ace Bandage”around their neck and head for a
week to promote proper healing.

10. FACELIFT
PART 3:
The third step in the process is a deep plane, two-layered facelift
that tightens the skin, and also tightens the underlying supportive
muscle (the platysma muscle). I’m careful about placement of the
incisions so that visible scarring is minimal and camouflaged.
I believe the THREE-IN-ONE FACELIFT is the ideal solution for many
patients. It removes signs of aging in key areas and adds vibrancy in
a natural and attractive way. In addition, by performing the proce-
dures in one surgical session we reduce the downtime and allow a
patient to return to their normal activities relatively quickly.
