This document provides instructions for taking a faith questionnaire to understand one's beliefs. It contains 42 statements about faith and spirituality. Respondents rate how much each statement describes them from 0-5. They then tally their responses in different categories related to faith paths like relativistic, traditional, authoritarian, intuitive, mystical, and evidential. The highest scores indicate which faith path or paths currently describe the respondent's beliefs.
