2. History taking
case ผู้ป่วยชายไทย อายุ 30 ปี ภูมิลาเนา จ.นครราชสีมา
CC : ขับรถจักรยานยนต์ชนรถเก๋ง 5 hr PTA
PI : 5 hr PTA ผู้ป่วยขับรถจักรยานยนต์ชนรถเก๋ง รถจักรยานยนต์ล้มขาซ้ายกระแทก
พื้น ปวดขาซ้ายมากทันที ลุกเองไม่ได้ ขยับขาซ้ายไม่ได้เนื่องจากปวดมาก ศีรษะกระแทกพื้น
สลบ จาเหตุการณ์ไม่ได้ มีประวัติดื่มสุรา ไม่มีแผลเปิดบริเวณขาซ้าย จึงมา รพ.
PH : no underlying disease
ปฏิเสธประวัติแพ้ยา/แพ้อาหาร
3. Physical examinations
A – talk clearly, not tender along c-spine
B – normal breath sound, equal both lungs, CCT negative
C – BP 129/66, capillary refill < 2 sec
D – E4V5M6 , pupil 2 mm RTLBE
E - LW 1x2 cm at scalp
- Left leg : swelling, no wound, no active bleeding, limit ROM
due to pain
- Dorsalis pedis pulse 2+, capillary refill < 2 sec
Vital signs : T 37.5 C PR 118 bpm RR 20 times/min BP 129/66mmHg
4. Investigation
Film CXR – normal
Film skull AP/lateral – no fracture
Film pelvic AP – no fracture
Film left leg AP/lateral
7. Close fracture both bone left leg
Mechanism of injury
Direct injury : อุบัติเหตุจักรยานยนต์ ถูกยิง
Indirect injury : ตกจากที่สูง หกล้มขณะที่เท้าติดอยู่กับที่
Signs and symptoms
Severe pain
Marked swelling
Deformity (angulation, abnormal rotation)
Shortening of the limb
Ecchymosis
Loss of function of the affected part(unable to move or bear weight)
Open injury (visualization of bony fracture or fat globule)
8. Management
Non-operative
Close reduction แล้วใส่ functional brace
โดยทั่วไปสามารถ close reduction ได้ โดยเน้นการ reduction ที่ tibia เป็นหลัก long leg cast
6 wk patellar tendon bearing (PTB cast) 6 wk
Indication : closed low energy fxs with acceptable alignment
< 5 degrees varus-valgus angulation
< 10 degrees anterior/posterior angulation
> 50% cortical apposition
< 1 cm shortening
< 10 degrees rotational malalignment
if displaced perform closed reduction under general anesthesia
11. Management
Non-operative
outcomes
•high success rate if acceptable alignment maintained
•risk of shortening with oblique fracture patterns
•risk of varus malunion with midshaft tibia fractures and an intact fibula
•non-union occurs in 1.1% of patients treated with closed reduction
18. Management
Close reduction ; Tramol 50 mg IV stat
Posterior long leg slab for 2 wk + F/U
Long leg cast for 1 wk + F/U unacceptable alignment
Set OR for tibial plate and screw
