To Parents and Guardians:
Please review the Student Admission Form and return it to the school with your student
tomorrow or Monday. Information provided on the form should be corrected or filled in if
missing. These forms need to be returned with proof of residence attached as soon as possible.
Proof of residence may be a water bill, electric bill, lease or mortgage statement. Phone and
cable bills are not acceptable proof of residence. Your student’s name and grade should be hand-
written in the upper left corner of the proof of residence with both first and last names provided.
Included is the picture form for fall school pictures. An application for free and reduce lunch is
also being sent home for those who would like to apply.
All items are to be returned to your student’s first period teacher.
