Relationship of Cerebral Blood Flow to Hematocrit Cerebral blood flow decreases with increases in hematocrit. Cerebral blood flow is optimum at a hematocrit of 45% or less. 106 106 Lahtinen R, Kuikka J. Cerebral blood flow in polycythaemia vera. Ann Clin Res 1983, 15(5-6):200-202.
Complications of ET • The complications and the incidences of the hyercoagulable and bleeding events associated with ET are defined by the Cortelazzo study. • In this study, Cortelazzo et al found a 20% incidence of arterial thrombosis, of which there were a significant number of myocardial infarctions (MI) and cerebrovascular accidents (CVA). • There was a 32% incidence of microvascular disturbances (transient ischemic attack [TIA] and erythromelalgia). • Hemorrhage was fairly prevalent at 18%, but only 5% of the cases required massive transfusion replacement. • Venous thrombosis occurred in 6% of the patients. 24 24Cortelazzo S, Viero P, Finazzi G, D’Emilio A, Rodeghiero F, Barbui T. Risk Factors for ET Complications What are the risk factors for ET complications? • Age is a definite factor. There is almost a three-fold increase in incidence of thrombotic complications when the patient is over 60. • Those individuals who had prior thrombotic events are at increased risk for recurring events. • Smoking as a risk factor has not been well characterized, but may be detrimental in decreasing thrombotic complications. • The use of oral contraceptives and the use of oral estrogen replacement therapy has not been examined. However, there are studies in the literature that oral estrogens increase hypercoagulability. 27 • The duration of thrombocytosis is important. However, whether the risk of thrombosis is greater for individuals with platelet counts >600,000/μL vs those with platelet counts >400,000/μL is not clear. • Other risk factors are coronary artery disease (CAD), factor V Leiden, prothrombin, gene mutation, and hyperhomocystinemia, all of which have not been well described. 1 27 Vandenbroucke JP, Rosing J, Bloemenkamp KW, Middeldorp S, Helmerhorst FM, Bouma BN, Rosendaal FR. Risk-Based Treatment for ET A low-risk patient is one who is young, that is, less than 60 years, with platelet counts ≤1,500,000/µL, no symptoms, and no comorbidities. Comorbidities are peripheral vascular disease, cardiovascular disease, diabetes mellitus, hypertension, and thrombophilia. ET: Risk of Thrombosis Another study, this time of 65 low-risk ET patients, was conducted to determine if treatment was required in this population 13 : The 65 low-risk ET patients were compared with 65 age- and sex-matched controls.The characteristics of the low-risk patients were as follows: • < 60 years • No history of thrombosis • Platelets <1.5 million/μL (mean 823,000/μL) After a median follow-up of 4.1 years, it was found that the incidence of thrombosis was: • 1.91 cases/100 patient years in ET • 1.50 cases/100 patient years in controls The authors concluded that low-risk patients do not need treatment. However, this study is subject to criticism because: • Aspirin usage was not controlled • Seven previous studies all had significantly higher rates of thrombosis in their groups of ET patients. 13 Ruggeri M, Finazzi G, Tosetto A, Riva S, Rodeghiero F, Barbui T. ET and Thrombosis Fifty-six ET patients were followed from 1976-1992 to compare the risk of complications after treatment with either platelet-reducing agents or antiaggregating agents. 22 Their median age was 66 years, median platelet count was 1.125 million/µL at diagnosis, and median follow-up was 45 months. Thirty-two patients received platelet-reducing agents and 41 received antiaggregating agents. • 46 (82%) had symptoms attributable to thrombocythemia: – 32 at platelet counts < 600,000/μL – 23 at platelet counts < 500,000/μL – 10 at platelet counts < 400,000/μL • 19 (34%) had severe complications (CVA, TIA, DVT, gangrene): – 10 at platelet counts < 600,000/μL – 7 at platelet counts < 500,000/μL – 2 at platelet counts < 400,000/μL In 42 (91%), improvement followed further reduction in platelets. In conclusion, thrombotic manifestations, including severe ones, are not uncommon in ET at relatively low platelet counts. The authors recommend that symptomatic patients with relatively low platelet counts should be treated to further reduce the platelet counts well into the lower normal range. 22 Regev A, Stark P, Blickstein D, Lahav M. Risk of Elevated Platelets: New Data There are several newer studies that provide insight into the pathophysiology of ET. Thrombotic complications were not common in individuals with low platelet counts. Symptomatic patients should be treated so that their platelet levels are reduced to normal range, 150,000-200,000/μL. 22 In a study with young patients, no thrombohemorrhagic complications were observed when the platelet counts were below 400,000/μL. 20 22 Regev A, Stark P, Blickstein D, Lahav M. Risk-Based Treatment for ET An intermediate-risk patient is one who is under 60 years of age, with platelet counts >1,500,000/μL, and without symptoms or comorbidities. Patients less than 60 years with platelet counts <1,500,000/μL are considered intermediate risk if they have cardiovascular risk factors. 73 73 Tefferi A. Risk-Based Treatment for ET For intermediate-risk patients, treat cardiovascular risk factors if they are present. Avoid aspirin if platelet counts are very high (>1,500,000/μL) so as to avoid bleeding disorders. Attack the abnormal clone and reduce platelet counts by treating with anagrelide, hydroxyurea, or IFN-α. 73 73 Tefferi A. ET: The High-Risk Patient The high-risk ET patient is: • One who is 60 years old or greater • Has had a previous incidence of thrombosis or a comorbidity such as diabetes, peripheral vascular disease, hypertension or thrombophilia. Thrombophilia should also include estrogen content (female preponderance, age). • Platelets >1.0 or 1.5 million/μL • Peripheral myeloid immaturity
Risk-Based Treatment for ET Age is a risk factor. A high-risk patient is one who is >60 years old. The patient has platelet counts >1,000,000/μL and is symptomatic with ischemic, thrombotic, embolic, or hemorrhagic complications. Treatment should be aimed at the pluripotent precursor cell. Hydroxyurea, IFN-α, and other myelosuppressive agents are effective choices of therapy. The patient should be protected against hyperuricemia. Conditions Suggesting Urgent Treatment Thrombotic risks, history of coronary artery disease, and comorbidities such as diabetes or cardiovascular disease are important risk factors.
