MEET THE NEW APP EMPOWERING PATIENTS TO
TAKE HOLD OF THEIR DIABETES – WIRED MAG
Kaiser Permanente's new preventative health app Health+ is changing lives
for patients living with prediabetes. The app, which is free for
all KP subscribers, empowers users to take their health into their own hands
through positive, shame-free education around exercise and healthy habits.
Just one year after launch, Kaiser Permanente is already seeing a 12% drop
among new diabetes diagnoses.
When I was diagnosed with prediabetes, I didn't know where to turn. My
doctor recommended Health+ and the encouraging support staff have meant
everything for me. I never exercised in the past but now I can run a 5k and
my doctor says I am almost out of the woods.

ideas that came out of the first 4 stages of the design sprint.
This prototype includes two common flows that customers
will follow which is continuing towards a goal of running a
5K and viewing healthy recipes to cook.
Assumptions • Users will keep their phones with them when working out

• See progress towards current workout goals
• Continue towards a workout goal
• View profile and achievements
