This document describes a mobile survey platform being developed to collect information from turkey farms in Minnesota about disease risks. Previous paper and web-based surveys from 2014-2015 had low participation rates and partial responses. The new mobile tool aims to simplify data collection, provide a user-friendly interface, and allow for rapid updating of survey questions. It utilizes mobile devices to more easily gather location data, photos, videos and other information directly from farms. This versatile platform seeks to increase farmer participation and compliance to better identify factors driving the introduction of influenza A virus to domestic turkey flocks.
