7. ! Twin, linkage and family studies indicate that heritable
etiology of ADHD and Autism Spectrum Disorders (ASD) are
shared. Autism or autistic traits have been identified in 20 to
30% of children with ADHD in recent studies.ADHD children
exhibiting autistic traits (ATs) have been shown to have
greater severity of illness and dysfunction.
(Kotte et al. (2013) Autistic Traits in Children With and Without ADHD. Pediatrics 132 (3): e612-
e622)
@yankiyazgancom
8. ! Objectives:
1. The primary purpose of this study was to investigate
the autistic traits in children and adolescents with a
primary diagnosis of ADHD, in a clinical sample, in
Istanbul, Turkey
2. The second aim was to examine the relationship of
the «autistic traits» with the emotional dysregulation
(EdR) and social responsiveness, in this clinical
sample without an ASD diagnosis
@yankiyazgancom
9. ! Participants:
! Study sample were children with a primary diagnosis of
ADHD from a private child and adolescent psychiatry clinic,
who were in clinical follow-up. This private clinic serves to a
population with above average-to-high parental education and
socioeconomic status. Most common reasons for referral are
neurodevelopmental and neuropsychiatric disorders, such as
ADHD, autism spectrum disorders (ASDs), learning disorders
(LD), tic disorders, obsessive compulsive disorder (OCD),
depression and various anxiety disorders.
! The study design was retrospective. Children with co-occuring
ASD were excluded from the study.
@yankiyazgancom
10. ! Measures
! Baseline assessments in addition to a semistructured interview include;
! Sociodemographic Form: Parent-rated form developed for the study
! Child Behavior Checklist for ages 6-18 (CBCL 6-18): This parent-rated
instrument measures competencies and problem behaviors of children and
adolescents aged 4 to 18. Problem behaviors in the past 6 months are rated
on a scale from 0 (never true) to 2 (almost always true) and items are
grouped under several subscales.
! SNAP IV: An 18-item scale derived from DSM-IV ADHD criteria rated by
parents or teachers from 0 to 3. The per item mean scores permits direct
comparison to community surveys in other countries.
! Social Responsiveness Scale (SRS): This is a 65-item scale which
measures motivation and ability for age-appropriate social interaction.
Each item on the scale inquires about an observed aspect of reciprocal
social behavior that is rated on a scale from “0” (never true) to “3” (almost
always true).

12. CBCL-AT profile: An empirically derived profile from the Child Behavior
Checklist (CBCL) to define autistic traits (AT) by using a cutoff of 195
from the combined T scores of the withdrawn, social problems, and the
thought problems subscales that correctly classified 78% of all subjects
with ASD from a psychiatrically referred sample with and without ASD
(Biederman et al. 2010). This profile has been used in a recent study by
Kotte and colleagues (2013).
CBCL-AT (+) : ≥ 195 on CBCL wtihdrawn T + CBCL social problems T
+ CBCL thought problems T
CBCL-EdR profile: This profile was created from the CBCL by summing
the anxiety/ depression, aggression, and attention scales (severe
dysregulation: sum of T scores ≥210; deficient emotional self-
regulation: sum of T scores of 180–210)

13. Comparison of parent-rated SRS and SNAP IV and teacher-rated SNAP-IV
scores for ADHD children according to CBCL-AT groups
ADHD children with the AT profile (N=64, 19% of the total sample) had significantly
higher scores on parent-SNAP and SRS (higher scores mean worse social functioning)
than ADHD children without ATs. AT olanlarda DEHB okulda değil evde.
@yankiyazgancom
14. Comparison of parent-rated SRS and SNAP IV and teacher-rated SNAP-IV
scores for ADHD children according to CBCL-EdR groups
Significant between-group differences for EdR profile on parent-SNAP and SRS
scores; severely dysregulated group (N=34) had significantly higher scores than
the group with deficient emotional self-regulation (N=106) and the group without
EdR (N=121); deficient self-regulation group had significantly higher scores than
the EdR-negative group (Tukey post hoc test). EdR yine evde.@yankiyazgancom
17. Parent-rated ADHD symptoms in the ADHD and ADHD + CBCL-AT groups
Scores on individual ADHD symptoms rated by SNAP-IV are higher for
CBCL-AT positive ADHD subjects. Differences were significant for 14
out of 18 symptoms on parent-rated SNAP
@yankiyazgancom
18. Teacher-rated ADHD symptoms in the ADHD and ADHD + CBCL-AT groups
Scores on individual ADHD symptoms rated by SNAP-IV are higher for
CBCL-AT positive ADHD subjects. Differences were significant for 2
out of 18 on teacher-rated SNAP.

25. ! Sosyal işlevsellik önemli
! DEHB sx okulda işlevselliğin bozulmasının ana
sebeplerinden birisi
! DEHBsi ‘zayıf’ ancak işlevselliği bozuk olanlar
! Kliniğe başvuran DEHB’nin okul ortamında değil
aile ortamında işlevsellik bozucu tipte AT ve EdR
problemleri var.
! Okul ortamında ise öğretmen açısından AT ve
EdR DEHB’ye göre daha az ‘işlev bozucu’
@yankiyazgancom
26. ! OSB ile DEHB arasındaki devamlılık ilişkisi ?
@yankiyazgancom
vermek zor. DSM IV tanı kategorisindeki Asperger
sendromunun bazı özellikleri yanısıra sosyal iletişim
bozukluğunda olduğu gibi (OSB’ye özgü tekrarlayıcı
hareketler dışındaki) sosyal pragmatik bozuklukları da
taşıyor.
! Ancak otizmde eksikliği özgül sayılan ilişkiden zevk alma,
karşılıklı beraberliğe ilgi gösterme gibi eksik yanlarının
önemli ölçüde geliştiğini, dürtüsellik ve dikkatle ilgili
sorunlarının daha ön plana geçtiğini anlıyoruz.
! Gelişimsel Koordinasyon Bozukluğu özellikleri de ayrıca
süreçte kendini önemli ölçüde göstermiş oluyor.
