I created mockups for the Distributed Health Labs @ CalIT2's Patient Health Record mobile app. The application aims to increase health literacy by making sense of confusing health records, and by placing medical data in the hands of patients.

Over 85% of adults have less than proficient health literacy.
Medical records are needed to help doctors and nurses make the right
decisions for your health. But that’s just the problem. Medical records are
just records, and we often deliver them from doctor to doctor without
understanding what’s inside.
This app uncovers the information buried in your medical records,
empowering you to better understand your body and make smarter
decisions about your health.

problems to your doctor more accurately.

also be added to your health planner in
the app.
