YUDISIUM
JURUSAN TEKNIK GEOLOGI FT. UGM
Yudisium dikeluarkan oleh Fakultas berdasarkan Surat Keputusan Lulus dari Ketua Jurusan Teknik
Geologi. Untuk mendapatkan Surat Keputusan Lulus dan yudisium, mahasiswa harus melengkapi semua
persyaratan akademik dan administratif.
1. Rapat yudisium dilaksanakan setiap bulan pada akhir minggu ketiga.
2. Mahasiswa yang akan mengajukan yudisium wajib menyerahkan kelengkapan dokumen pada Bagian
Pengajaran dua hari sebelum yudisium dilaksanakan yang terdiri dari :
a. Paspoto berwarna (tanpa editan) tiga bulan terakhir (3 X 4) sebanyak 4 lembar. Pria berkemeja
putih polos, berdasi &berjas hitam, wanita berkemeja putih dan berjas hitam
b. Kartu mahasiswa yang masih berlaku
c. Form data wisudawan Jurusan Teknik Geologi (Form 2)
d. Surat keterangan bebas pinjam Perpustakaan Fakultas
e. Surat keterangan bebas pinjam Perpustakaan Pusat (UGM)

Mengisi dan menyerahkan dokumen tracer study dari website http://tracer.kjm.ugm.ac.id/
atau dari formulir yang telah disediakan di bagian Akademik.
Prosedur Pengurusan :
1. Berkas formulir Surat Persetujuan Yudisium, diminta ke bagian akademik
2. Lampiran 2d diminta ke perpustakaan Fakultas Teknik UGM
3. Lampiran 2e diminta ke perpustakaan Pusat UGM
4. Berkas lengkap, dimasukkan dalam map warna biru (S-1)/warna merah (S-2) luar diberi nama, NIM
dan Jurusan.
5. Berkas disusun berurutan

Persyaratan Yudisium di Fakultas Teknik UGM :
* Khusus untuk S-2 melengkapi persyaratan wisuda khusus S-2 (Form 6)
1. Yudisium/Surat Keputusan Lulus dari Ketua Jurusan
2. Fotokopi Ijazah SLTA/S1
3. Menyerahkan surat bebas pinjam fasilitas dari :
a. Perpustakaan Pusat (UGM)
b. Perpustakaan Fakultas Teknik
4. Menyerahkan pasfoto ukuran 3x4 cm sebanyak 7 lembar
5. Menyerahkan fotokopi kartu mahasiswa yang berlaku
6. Mengisi formulir isian wisuda yang disediakan di Sub Bagian Pendidikan Fakultas
7. Membayar Wisuda di bank dan menyerahkan bukti pembayaran ke Sub Bagian Pendidikan

JUDICIUM
Department of Geological Engineering
Faculty of Engineering, Gadjah Mada University
Judicium is issued by The Faculty based on Reference Letter of Graduation from Head of
Department of Geological Engineering. To get Reference Letter of Graduation, students must
complete all academic and administrative requirements.
1. Judicium meeting is held on third week in every month.
2. Two days before judicium meeting, students should submit all requirement documents below:
a. Colour passphoto (without editing) the last three months (3 X 4) 4 pieces. Man should
wears white shirt, tie and black suit, woman should wears white shirt and black suit.
b. A valid student card
c. Graduation Data Form (form 2)
d. Reference Letter of Library Indebtedness Free from Library of The Faculty
e. Reference Letter of Library Indebtedness Free from Central Library of The University
f. Copy of certificate of high school/undergraduate/master
g. Copy of birth certificate
h. Copy of cover script/thesis/dissertation, script/thesis/dissertation approval sheet and
publications on CD and hard file.
i. Academic requirements, include:
• Progress report, approved by Academic Advisor/Program Manager
• Copy of abstract script/thesis in English and bahasa Indonesia (includes: Title,
Name, Student Number, Advisor)
• Reference Letter of Script/Thesis revision (Form 4)
• Reference Letter for submission documents of Script/Thesis (Form 5)
• Fasilities Indebtedness Free Form (Form 3)
• Satisfaction Questionnaire
• Fill and submit print of The Tracer Study form via website
http://tracer.kjm.ugm.ac.id/ or available from Academic Secretariate.

Submission Procedures:
1. Get The Judicium Approval Form from academic secretariate
2. Get The Attachment file no. 2d from Library of Faculty of Engineering
3. Get The Attachment file no. 2e from Central Library of Gadjah Mada University
4. All documents should be put in a blue folder (undergraduate)/red (master). Write in the cover
of folder: Name, NIM and Name of Department
5. Files are arranged as order.
Terms of Judicium in Faculty of Engineering:
* For Master should complete graduation requirements list (Form 6)
1. Reference Letter of Graduation from Head of Department of Geological Engineering.
2. Copy of certificate of high school/undergraduate
3. Submit a Reference Letter of Library Indebtedness Free:
a. Central Library of Gadjah Mada University
b. Library of Faculty of Engineering
4. Submit passphoto size 3x4 cm, 7 pieces
5. Submit a copy of valid student card
6. Fill the graduation form which provided in Education Unit in Faculty of Engineering
7. Pay Graduation Fee Payment in Bank Account and submit the receipt to Education Unit in
Faculty of Engineering
