This risk assessment form summarizes the risks involved in filming a short film with two sequences: 1) an actor walking down a hallway, and 2) an actor being grabbed in the toilets and pushed into a toilet. Locations include Reigate College in Reigate. Hazards identified include slip, trip, and fall hazards. Controls are identified to minimize risks, such as ensuring dry floors, removing obstacles, and using protective gear. A potential hazard of driving two cars in a race start is also summarized, with controls such as only filming when the road is clear of traffic and wearing seatbelts. The risk level is determined to be negligible or slight after controls are implemented.

Negligible – Unlikely Make sure the floor is dry w ithin the toilets and if there is a w et patch, avoid it as much as
possible.
Move the bin out of the w ay of the direct path of the actors and any other obstacles.

get hurt. Make sure the actor being pushed into the locker is w earing protective gear on
his/her back to cushion the impact. Make sure the locker that is being used to be pushed
against is stable and w ont fallover, aswellas removing any sharp protruding objects such as
locks or knobs that could cause damage if the actor w as knocked into.

Members of the public
Slight – Very Unlikely Make sure the race start is on a road w here there are no other cars or members of the public
nearby. Only do the race start w hen we know for a fact that the road ahead is clear and there
w illbe no oncoming traffic. Make sure w e have crew members ahead of the cars checking
the coast is clear on the roads. Make sure the drivers of the cars are w earing their seatbelts.
