This document provides instructions for creating a new patient chart in an electronic medical record system. It details the steps to log in to the system, navigate to the patient charts page, click the button to add a new patient, fill in the required fields marked in red, and then either save and continue or save and view the new patient chart. The completed chart can then be used to view the patient's referral history, add new referrals, or print the chart.
