The early stages of puberty are characterized by nocturnal surges of

Hypothalamic GnRH regulates the production of the pituitary gonado-
tropins, LH and FSH (Fig. 340-2). GnRH is released in discrete pulses
approximately every 2 h, resulting in corresponding pulses of LH and Spermatogonium
FSH. These dynamic hormone pulses account in part for the wide vari-
ations in LH and testosterone, even within the same individual. LH acts FIGURE 340-2 Human pituitary gonadotropin axis, structure of tes-
primarily on the Leydig cell to stimulate testosterone synthesis. The tis, seminiferous tubule. E2, 17β estradiol; DHT, dihydrotestosterone.
4. Testosterone
• durante lo sviluppo embrionale: induce
formazione dei genitali esterni (DHT) e dei dotti
caratteristiche secondarie sessuali maschili
• nell’individuo adulto nesessario per
spermatogenesi, indude libido e normale funzione
sessuale
5. Apparato riproduttivo maschile
• Sertoli producono anche MIS (Muhellerian
Inhibiting substance)
• Disfunzione dovuta a a scarsa produzione
androgeni:
• primaria
• secondaria per deficienza gonadotropine
6. • Adrenarca: sviluppo caratteri sessuali secondari a 6-8
• GH aumenta in pubertà e determina aumento IGF-1
con crescita lineare ossa. Chiusura epifisi dovuta a
estrogeni.
aumento massa musclolare e stempiamento
8. Trasporto
• Testostone quasi comppletamente legato alla Sex
Hormone Binding Protein e all’albumina
• DHT e estradiolo risultano da conversione
testosterone
• metabolizzato a livello epatico

16. Differenziamento sessuale
• Pubertà precoce (nei maschi prima di 9 anni)
• isosessuale
• dipendente da gonadotropine: attivazione
precoce di secrezione GnRH pulsatoria (centrale)
• indipendente da gonadatropine: tumore hCG-
secernente, iperplasia surrenalica, tumore
tosticolo, surrenale, ovaio steroidogenico
• eterosessuale effetti estrogeni come ginectomasia
17. Differenziamento sessuale
• Pubertà ritardata (nei mashi dopo ~14 anni)
• constituzionale (c’è una storia familiare)
• ipogonadismo ipogonadotrofico (malnutrizione,
malattia)
• ipogonadismo ipogonadotrofico per difetto
genetico o lesione acquisita
• ipogonadismo ipergonadotrofico per deficit
gonade
18. Disordini durante età adulta
• deficit gonadotropine ha come consegenza un
ipogonadismo secondario con ↓ testosterone oltre
a ↓gonadotropine (spesso difetto sintesi GnRH)
• può essere congenito, spesso idiopatico o legato
a sindromi come kallman, prader-willi,
mutazione recettore leptina.
19. Ipogonadismo secondario acquisito
• in obesità c’è meno SHBG con minor testosterone totale
nel plasma e c’è effetto aromatasi in tessuto adiposo.
• in iperprolattinemia c’è inibizione produzione
insufficienza ipofisi
20. Ipogonadismo Primario
• Ipogonadismo primario: Kilnefelter,
criptoarchidismo (gonadi non discese da cavità
addominale) non corretto, malattie che
coinvolgono il testicolo come paramixovirus e
altre infezioni virali, trauma come torsione,
radiazioni nel corso di radioterapia, insuff.
polighiandolare autoimmune
• resistenza a testosterone o DHT
21. Ginecomastia
• ginecomastia indica lo sviluppo delle mammelle
nell’uomo
• correla positivamente con BMI (↑aromatasi in t.
adiposo) ed età
• è caratteristica di Klinefelter, resistenza agli
androgeni, eccesso di estrogeni per tumore
secernente.
