Where did this data come from... This data relates to the audit data being stored by the Rhapsody for Podiatry system.  The data was calculated automatically. This data was entered manually after automatically requesting the data from the owner of the control record. This data was generated automatically from the Rhapsody Service Review system.  It was checked and confirmed by Caroline before this KPI was submitted to dashboards. This data was generated automatically from data feeds from the PCIU.  Only a few are show here and this is real data.
So what are we controlling... Risks associated with not reaching ... Service targets such as referrals, discharges, contacts and much more for all services where data is collected by the PCIU The same data as above but also from new Rhapsody based systems such as Podiatry and Heart Failure Service Reviews Anything else ...

Control Freak The system is based on automatically reviewing Rhapsody controls.  These controls manage the risks associated with not reaching key performance indicator targets. The Rhapsody control engine automatically processes controls when they are due for review. This review process does the following...
What’s it doing? Checks the control records every night Processes any that are due for review Calculates the new KPI value for the period Works out if any warnings about the KPI should be emailed to the control owner Updates the related risk & objective Updates the evidence /  assurance record

What activities generate KPI changes? Changes to data within core Rhapsody currently include Service Reviewsbut the system can be extended to include the complete management of PCT wide incidents and much more. Patient activities processed in the Rhapsody for Podiatry and Rhapsody for Heart Failure systems. Refreshed data feeds from PCIU data such as CPAS.
