New Coder as a Biller
Question: I am a NEW coder, now working
as a biller. I see errors often, typically
sequencing. How do I protect my license as
I am being utilized as a biller and where is
the line drawn?

Question: I would like for you to go over
TCM leveling for the physician's office. My
understanding is that it is based on the
medical decision making only, but I would
like to learn more about it. Could you
possibly discuss this in the next monthly
Q&A webinar?

Readmission after a C-Section
Question: What code would be billed if a
patient had a C-section and then after
discharge was admitted back to the hospital
a week in a half later for seroma and the
delivering physician is called in to see the
patient by the attending/admitting physician?

Gastric Bypass / Obesity Coding
Question: A patient is ordered to have “Obesity
Counseling” for 6 months prior to her Gastric Bypass
surgery. G0447 is not a covered benefit with her
insurance. For this type of counseling, where her
Obesity is considered an illness in and of itself –
wouldn’t you bill 99408 (brief intervention services),
or would you bill 99401 (preventive counseling)?

Question: What CPT code should be used for
Family Planning / Sterilization / Vasectomy
Consults? I have found some Coders use E&M
codes while others use Consult codes and others
use Preventive Counseling codes.

Fetal Echocardiography Ultrasound Documentation
Question: Is any specific report documentation
(other than a heading of fetal echocardiography
and listing of what was viewed and its findings)
that is required to bill for a fetal
echocardiography (76825) or doppler fetal
echocardiography (76826) and to show it was not
just an ultrasonic view of the heart?

ICD-10 Infant Weight Chart
Question: I am not familiar with the weight of
infants, especially now in ICD-10 .. it is by grams.
Is there a chart that can be used as a reference?
I guess it could be a cross-reference weight into
grams. Do you know a site to get a chart? When
they talk 4.6 lbs,oz in their questions, the book is
by grams....
