SUBMISSION OF CID FORMS 4 & 5 AND DTR

1. CID Form 4 was not submitted before
conducting the instructional supervision.
Submit a week before the conduct of IS.
2. Late submission of CID Form 4. Submit on time.
3. CID Forms 4 & 5 are submitted only as
attachment to travelling documents.

-The time allotment for in-person class hours shall be
based on Annex 1 (Elem. Educ.) and Annex 2 (Sec. Educ.)
of DO 21, s. 2019.
-Teachers shall adhere to the stipulations under DO 8, s.
2015, which requires the provision of intervention
through remediation or extra lessons to learners who
receive a grade below 75. To ensure that teachers
perform their task, they must be spared of any
administrative and finance-related tasks, particularly in
schools where qualified non-teaching personnel can be
designated to perform such duties.

SHs are reminded of the following:
- The MELCS shall continue to apply for SY 2022-2023.
- Classroom assessment shall follow DO 8, s. 2015
- To assure the quality of learning, formative
assessments shall be regularly conducted by the
teachers.
- All elementary schools shall administer the ECCD for
Kindergarten during the opening of classes, midyear
and end of the SY. The results will be used as the basis
for planning and formulating appropriate
interventions for children.
- The Bureau of Alternative Education (BAE) shall
develop a policy on assessment for the ALS program in
coordination with the BEA.

SHs are reminded of the following:
-As provided for in DO 14, s. 2018, the Phil-IRI Group
Screening Test (GST) for Fil Gr 3-6 and for English Gr. 4-6
shall be conducted within the 1st or 2nd month of the SY
while the Individualized Phil-IRI Assessment shall be
conducted within a month after the GST is administered.
The schedule of administration of the Posttest shall be
announced in a separate issuance.
-The MFAT shall be administered to Gr 1 learners a month
after the opening of classes as provided for in DO 29, s.
2018. The result will be used as the basis for instructional
planning and in formulating appropriate intervention
strategies for learners who exhibit development
advancement or delays or show manifestations of
learning disabilities.

SHs are reminded of the following:
-Classes are immediately suspended during observances
of regular and special nat’l holidays as stipulated by the
law, as well as local holidays which have been duly
declared by law or through Presidential Proclamation
-The no. of school days shall be solely dedicated to
academics and related co-curricular activities. The
conduct of extra-curricular activities shall be strictly
prohibited.
-Virtual activities through different social media
platforms which require reactions or sharing may be
done for advocacy, awareness, or collaboration purposes,
but shall not form part of the learner’s scholastic grade.

SHs are reminded of the following:
-Curricular and co-curricular activities that involve the in-
person gathering of a large number of participants like
the NSPC, NSTF, NFOT and other similar undertakings,
may be conducted in hybrid modality subject to the
declared alert levels of the country.
-Curricular activities shall be conducted during the
academic quarter to ensure that learners can seamlessly
connect and integrate learning within and across learning
areas while providing manageable breaks consistent with
MemoOUCI-2020-307 entitled “Suggested Measures to
Foster Academic Ease during the COVID-19 Pandemic.”
On the other hand, co-curricular activities shall be
conducted after the quarter examinations to avoid
disruption of classes.

SHs are reminded of the following:
-Schools shall strictly devote ten weeks of every academic
quarter to actual classroom teaching. The 11th week of each
quarter may be used for the conduct of co-curricular activities.
-The 5-day mid-year break shall be spent for the conduct of
the Midyear Performance review and Evaluation and School-
Based INSET. The first 2 days shall be spent in evaluating the
school’s progress in the implementation of the educ’l PPAs as
well as in reviewing the performance of teachers and the
school staff to address concerns through cooperative effort.
The last 3 days shall be devoted to the conduct of the school-
based INSET activities for teachers’ continued prof’l dev’t and
the preparation of their IMs.

-The LAC shall be sustained by conducting it at least once
a month on an in-person learning modality; however, it

schools. DO 35, s. 2016 suggests the following broad
areas of discussion where specific topics may be deduced
:
a. Learner Diversity and Student Inclusion
b. Content and Pedagogy of the Kto12 BEP
c. Assessment & reporting in the Kto12 BEP
d. 21st century Skills and ICT Integration in Instruction
and Assessment

b. Coordinate the transfer of the Learner’s Permanent
Academic record and validate its authenticity for those
learners who came from another school
c. Observe properly the guidelines for the transfer of
learner’s academic records per DO 54, s. 2016

individually or by small group.
SHs ensure that the time allotment for other health and
safety- related activities, such as handwashing,
temperature checking, and other related preliminary
activities, shall not affect the actual contact/teaching
time.

  • 15. SUBMISSION OF CID FORMS 4 & 5 AND DTR FINDINGS RECOMMENDATIONS 1. CID Form 4 was not submitted before conducting the instructional supervision. Submit a week before the conduct of IS. 2. Late submission of CID Form 4. Submit on time. 3. CID Forms 4 & 5 are submitted only as attachment to travelling documents. Only approved CID Forms 4 & 5 are to be submitted and attached to travelling documents. 4. No submission of CID Forms 4 & 5. Need to submit not only as attachment for travel documents but as MOV for the IPCRF. Rating period this CY 2022 shall be done face- to-face (one-on-one).

Preparati on for SY 2022- 2023 DO 34, s. 2022- School Calendar for SY 2022-2023 -The time allotment for in-person class hours shall be based on Annex 1 (Elem. Educ.) and Annex 2 (Sec. Educ.) of DO 21, s. 2019. -Teachers shall adhere to the stipulations under DO 8, s. 2015, which requires the provision of intervention through remediation or extra lessons to learners who receive a grade below 75. To ensure that teachers perform their task, they must be spared of any administrative and finance-related tasks, particularly in schools where qualified non-teaching personnel can be designated to perform such duties.

Classroom- based and Systems Assessment DO 34, s. 2022- School Calendar for SY 2022-2023 SHs are reminded of the following: - The MELCS shall continue to apply for SY 2022-2023. - Classroom assessment shall follow DO 8, s. 2015 - To assure the quality of learning, formative assessments shall be regularly conducted by the teachers. - All elementary schools shall administer the ECCD for Kindergarten during the opening of classes, midyear and end of the SY. The results will be used as the basis for planning and formulating appropriate interventions for children. - The Bureau of Alternative Education (BAE) shall develop a policy on assessment for the ALS program in coordination with the BEA.
  • 22. CONCERNS BACKGROUND INFORMATION RECOMMENDATION 2. Classroom- based and Systems Assessment DO 34, s. 2022- School Calendar for SY 2022-2023 SHs are reminded of the following: -As provided for in DO 14, s. 2018, the Phil-IRI Group Screening Test (GST) for Fil Gr 3-6 and for English Gr. 4-6 shall be conducted within the 1st or 2nd month of the SY while the Individualized Phil-IRI Assessment shall be conducted within a month after the GST is administered. The schedule of administration of the Posttest shall be announced in a separate issuance. -The MFAT shall be administered to Gr 1 learners a month after the opening of classes as provided for in DO 29, s. 2018. The result will be used as the basis for instructional planning and in formulating appropriate intervention strategies for learners who exhibit development advancement or delays or show manifestations of learning disabilities.
  • 23. CONCERNS BACKGROUND INFORMATION RECOMMENDATION 2. Curricular and CO- curricular Activities DO 34, s. 2022- School Calendar for SY 2022-2023 SHs are reminded of the following: -Classes are immediately suspended during observances of regular and special nat’l holidays as stipulated by the law, as well as local holidays which have been duly declared by law or through Presidential Proclamation -The no. of school days shall be solely dedicated to academics and related co-curricular activities. The conduct of extra-curricular activities shall be strictly prohibited. -Virtual activities through different social media platforms which require reactions or sharing may be done for advocacy, awareness, or collaboration purposes, but shall not form part of the learner’s scholastic grade.
  • 24. CONCERNS BACKGROUND INFORMATION RECOMMENDATION 2. Curricular and CO- curricular Activities DO 34, s. 2022- School Calendar for SY 2022-2023 SHs are reminded of the following: -Curricular and co-curricular activities that involve the in- person gathering of a large number of participants like the NSPC, NSTF, NFOT and other similar undertakings, may be conducted in hybrid modality subject to the declared alert levels of the country. -Curricular activities shall be conducted during the academic quarter to ensure that learners can seamlessly connect and integrate learning within and across learning areas while providing manageable breaks consistent with MemoOUCI-2020-307 entitled “Suggested Measures to Foster Academic Ease during the COVID-19 Pandemic.” On the other hand, co-curricular activities shall be conducted after the quarter examinations to avoid disruption of classes.
  • 25. CONCERNS BACKGROUND INFORMATION RECOMMENDATION 2. Curricular and CO- curricular Activities 3. Five-Day Mid-Year Break (Feb. 6-10, 2023) DO 34, s. 2022- School Calendar for SY 2022-2023 SHs are reminded of the following: -Schools shall strictly devote ten weeks of every academic quarter to actual classroom teaching. The 11th week of each quarter may be used for the conduct of co-curricular activities. -The 5-day mid-year break shall be spent for the conduct of the Midyear Performance review and Evaluation and School- Based INSET. The first 2 days shall be spent in evaluating the school’s progress in the implementation of the educ’l PPAs as well as in reviewing the performance of teachers and the school staff to address concerns through cooperative effort. The last 3 days shall be devoted to the conduct of the school- based INSET activities for teachers’ continued prof’l dev’t and the preparation of their IMs.

Conduct of LAC DO 34, s. 2022- School Calendar for SY 2022-2023 SHs are reminded of the following: -The LAC shall be sustained by conducting it at least once a month on an in-person learning modality; however, it may also be done virtually when it is difficult to have in- person sessions, particularly when involving clustered schools. DO 35, s. 2016 suggests the following broad areas of discussion where specific topics may be deduced : a. Learner Diversity and Student Inclusion b. Content and Pedagogy of the Kto12 BEP c. Assessment & reporting in the Kto12 BEP d. 21st century Skills and ICT Integration in Instruction and Assessment e. Curriculum Contextualization, Localization and Indigenization

Initial Tasks of the Class Advisers DO 11, s. 2018 SHs are reminded of the following: -Inform class advisers of their initial tasks at the beginning of SY such as; a. collect documents to establish the identity of each learner assigned to his/her class (PSA birth certificate, Baptismal Certificate or any equivalent document b. Coordinate the transfer of the Learner’s Permanent Academic record and validate its authenticity for those learners who came from another school c. Observe properly the guidelines for the transfer of learner’s academic records per DO 54, s. 2016

Remediation/ Intervention Class scheduling DO 17, s. 2022 SHs to ensure that learning remediation/ intervention shall be part of the regular class schedule and daily teaching time. Remediation/ intervention shall be a minimum of one hour depending on the needs of the learners. Learning remediation/ intervention may be done with learners individually or by small group. SHs ensure that the time allotment for other health and safety- related activities, such as handwashing, temperature checking, and other related preliminary activities, shall not affect the actual contact/teaching time.
