• Asumiendo que el riesgo está elevado: cuáles son los mecanismos
subyacentes?

• Beneficio de SGLT2I: control glucémico, reducción de peso y mejoría de
presión arterial.
• Puede ser usado con o en lugar de Metformina, si no la tolera o está
contraindicada.
• Reduce el requerimiento de insulina
• Falla renal:
• No se recomienda en pacientes con TFG <45cc/min/1.73m2
• Dapagliflozina no se recomienda si TFG <60cc/min/1.73m2

If SGLT2 reabsorbs ~97% of filtered glucose, then it is surprising that humans treated with SGLT2 inhibitors only excrete 50–60% of the filtered glucose [4]. This is mimicked in SGLT2 knockout mice, in which mean fractional glucose excretion was 64% [7]. Studies in SGLT1−/− mice have indicated that SGLT2 inhibition strongly increases SGLT1-mediated glucose reabsorption [21], and demonstrated that the increase in SGLT1-mediated glucose reabsorption fully explained the fractional glucose reabsorption of 40–50% during SGLT2 inhibition in euglycemic mice [11] (Figure 2). Recent studies further indicated that the SGLT2 inhibitors reach their target mostly through glomerular filtration and from the luminal side [11;22].

Regulación molecular de la exptesión y actividad de SGLT2
Studies in mice have shown that pharmacological SGLT2 inhibition itself also increases renal SGLT2 protein expression, i.e. under conditions when the physiologic uptake of glucose into the early proximal tubule is inhibited [14]. The mechanisms involved in SGLT2 upregulation under these conditions and in response to diabetes (see above) remain poorly understood. Studies in human embryonic kidney (HEK-293 T) cells indicated that insulin phosphorylates SGLT2 on Ser624 thereby increasing its activity [16]. Studies in cultured human proximal tubular cells reported that insulin, but not high glucose, enhanced SGLT2 protein expression. The insulin-induced increase in SGLT2 expression was associated with an increased reactive oxygen species content and attenuated by N-acetylcysteine [17]. Insulin binding in the kidney corresponds to sites of SGLT2 expression, and thus, the insulin release following ingestion of a glucose rich meal may activate SGLT2 to retain the ingested glucose in the body.
Other molecules implicated in the upregulation of SGLT2 activity include protein kinase C and protein kinase A, while angiotensin II AT1 receptors and hepatocyte nuclear factor HNF-1α have been proposed to increase SGLT2 expression in the diabetic state
The tubular hypothesis of diabetic glomerular hyperfiltration: effect of SGLT2 inhibition. Panel A) 1. Glucose reabsorption occurs via SGLT2 in early proximal tubule (PT) and to a small degree via SGLT1 in late PT; Na+ is reabsorbed via NHE3 (intercambiador Na/H). 2. NaCl is reabsorbed via NKCC2 in thick ascending limb (TAL). 3. Luminal NaCl concentration at macula densa ([NaCl]) inversely regulates GFR via tubuloglomerular feedback (TGF). 4. Fluid delivery to distal tubule (site of main tubular resistance) is proportional to hydrostatic pressure in Bowman space (PBow) which imposes inhibitory influence on GFR. Panel B) 1. Hyperglycemia and tubular growth increase Na+/ glucose reabsorption via SGLT2 and SGLT1 and increase Na+ reabsorption via NHE3. 2. & 3. In addition, SGLT2 inhibition increases load to and Na+ reabsorption via SGLT1 and NKCC2 in outer medulla. This may enhance medullary hypoxia unless counterbalanced by GFR reduction.

Safety Data on SGLT2 Inhibitors[11-13]
The older the patient, the more likely he or she has renal impairment
Use caution with dose changes of loop diuretics in these patients
Reports of bone fractures seen with canagliflozin may be a result of older age and renal dysfunction[14]
SGLT2 Inhibitors and Genitourinary Infections in T2D[26-28]
There is an increased risk of genitourinary infection with the use of SGLT2 inhibitors
Infections are not severe and are easily treatable, usually with topical medications
Important to educate patients on possible occurrence of these infections and encourage good hygiene
Can help with patients’ adherence to the treatment regimen
The sodium‐glucose transporter SGLT2 mediates glucose reabsorption in the early proximal tubule and SGLT1 in the late proximal tubule. Sodium‐monocarboxylate transporter (SMCT) reabsorb lactate and ketone bodies across the luminal membrane of the proximal tubule with SMCT2 and SMCT1 dominating in the early and late proximal tubule,respectively, establishing some symmetry to SGLT2/SGLT1. Monocarboxylate transporters (MCT) facilitate the basolateral exit of lactate and
ketone bodies, which can be used for hepatic gluconeogenesis and as energy substrates in brain and heart, respectively. Lactate can also be used
for renal gluconeogenesis whereas ketone bodies are a preferred renal tubular energy source. The urate anion transporter 1 (URAT1) in the luminal
membrane of the proximal tubule takes up urate in exchange for an intracellular monovalent anion such as lactate or ketone bodies. Thus, the
activities of SMCT2 and SMCT1 facilitate urate uptake by URAT1. SGLT2 inhibitors and the associated lowering in blood glucose levels can induce
secondary effects on these pathways as discussed in more detail in the text.
OJO= Renal ketone body excretion involves the balance
of glomerular filtration and tubular reabsorption. Rat studies revealed
that AcAc and βOHB are freely filtered in the glomeruli followed by
reabsorption of ~80% of the filtered ketone bodies in the proximal
convoluted tubule, with most of the remainder being reabsorbed more
distally, such that less than 1% is excreted into the urine
Separation of Curves in EMPA-REG OUTCOME Trial[4]
Wide separation of curves for outcomes started at approximately 3 months
This has not been seen in previous studies with blood pressure agents or statins, for example
Proposed hypothesis: related to fuel metabolism, which begins to change very quickly
There were no dramatic differences in glucose-lowering, blood pressure-lowering, or weight-lowering effects
For the cardiovascular outcomes:
The heart normally uses free fatty acids (FFA) in the fasted state and glucose in the fed state (both of which are good fuels)
In T2D, there is increased myocardial FFA oxidation and reduced glucose oxidation; however, this is less energy efficient
Ketone bodies are elevated in patients treated with SGLT2 inhibitors, as well as in patients with heart failure (below level of ketoacidosis)
With the use of empagliflozin in these patients and the constant hyperketonemia, myocardial and renal fuel metabolism shifts toward ketone bodies (super fuel), which are more energy efficient than glucose or FFA
Diabetic Ketoacidosis: Potential Triggering Factors[29]
Incidence of diabetic ketoacidosis (DKA) in T2D is low[30]
Less than 1 per 1000 patient-years
Can occur with marked insulin reduction and low-carbohydrate diet
Glucagon levels increase, which can lead to DKA
Recognizing Signs of Euglycemic DKA[31]
Important for healthcare provider to be aware of mechanism of action of SGLT2 inhibitors
In a patient with euglycemic DKA, glucose levels will not be extremely elevated due to glycosuric effects of SGLT2 inhibitors
As with any other drug, inform the patient of the risks and benefits of treatment and educate on what to expect once beginning therapy
Among 9 217 555 adverse event reports in the FAERS up to March 31, 2017, 66 were SGLT2 inhibitor-associated amputations. Most of the available reports (57 [86%] of 66) listed canagliflozin as a suspect or concomitant drug.
The most common level of amputation was the toe, but there were 13 above-ankle leg or limb amputations, two multiple amputations, one hand amputation, and three fatal cases.
Incorporating SGLT2 Inhibitors Into Individualized Treatment Plans[11-13,16,17]
As per American Diabetes Association (ADA) and American Association of Clinical Endocrinologists (AACE) guidelines, SGLT2 inhibitors can be used as initial drug therapy if metformin is not tolerated or is contraindicated, as well as with other antihyperglycemics as dual or triple combination therapy[16,17]
It is important to measure renal function in patients before starting them on an SGLT2 inhibitor
