It is describe an advanced level of clinical
nursing practice that maximize the use of
graduate educational preparation ;in depth
nursing knowledge and expertise in
meeting individuals ;family and
communities

 New programs, courses, experiences and
changing
requirements for the development of new skills
pave the way for different opportunities for
students to prepare for initial and continuing
practice in a rapidly changing society.
These trends influence the number and types of
nursing programs for basic and experienced
students at the undergraduate and graduate
levels.

1-Rapidly knowledge
expansion.
 The rapid growth in information technology has
impact on health care delivery and the education
of nurses.
 The electronic medical record will replace
traditional documentation systems. Customers
will be able to get information about their patients
through internet .
 Nurses of the 21st century need to be skilled in
the use of computer technology by using
distance learning and can have a link with
faculty of nursing from different locales for
education.

2-Globalization of world’s
economy and society.
-As a transnational integration and expansion of
economic , social , political and cultural values
,globalization has a certain effects on nursing as well.
-the so-called age of globalization has already witnessed
rise of global health problems.
-The urgency of these problems demand the nurses ,as
the largest group of healthcare workers, attain global
view on health and become capable of creating
appropriate solutions.
-It’s important to nurses to have a wider knowledge to
enrich their care experiences, to be able to use
technology and to guide patients in this complicated
process with a strategic view on the means and ends
in nursing.
. Nursing education and research must become more
internationally focused to disseminate information and
benefit from the multicultural experience
3-community focused
interdisciplinary approaches.-wide range of knowledge and skills is required to effectively
manage all needs of patients and populations
The health care delivery system in future will depend on teams of
nurses ,nurse practitioners ,physicians,dentists,social workers
and pharmacies and other providers to work together.
-with care management ,acritical component in health care
delivery, nurses must demonstrate leadership and competence
in collaborative practice for continuous quality improvement.
-Teaching methods that incorporate opportunities for
interdisciplinary education and collaborative practice are
required to prepare nurses for their unique professional role and
to understand the role of other disciplines in care of patients.
-This preparation need cost programs and shortage of nursing
make that difficult.
-most nurses in Egypt are technical not bachelor ,so how to ask
them about quality of care due to their knowledge is low.
4- nursing shortage on health
care.-In most of countries nursing is a popular profession for men
and women ,but the number of facilities requiring nurses is
growing at the faster pace than the number of trained
nurses to staff them.
-Every graduating class is absorbed into the health care system
months before commencement exercises ,but nursing
shortage still persist.
-some of this shortage due to poor working conditions ,and
inadequate information, and support system of nurses.
-this result in nurses looking for migration, also the RN
workforce is aging , and they want to work less than full
time or retire early.
-Brain drain has a negative impact on health care system ,where
the recruited nurses may not be familiar with language, and
culture of people they are supposed to care.
-A possible intervention is for countries to collaborate with
each other in expanding training of professional nurses and
improve working conditions of nurses to alleviate the
problems of nursing staff shortage and turnover.
Nursing education must partner with the health care to develop innovative solutions that address the
nursing shortage and must initiate an intense media campaign to enhance nursing image .
5- Licensure and regulations.
-Issues surrounding health care are often complex, involving the
individuals' rights as well as access to health care .Each state
has its own set of administrative rules and regulation. --The
early laws provided for permissive licensure ,permissive
licensure , it is avoluntary system whereby individuals choose
to become licensed to provide evidence of competence , but
a license may is not required to practice .
the requirements of this included graduating from school that
satisfied certain standards established in the practice act for
that state and passing comprehensive examination.only those
who had met these standards could use the title RN.
others used the title ‘’nurse’’ .Under this system , no one was
require to have a license to practice nursing and there was
much variability in standards from one state to another.
-After permissive licensure laws came into being ,nurses activity
regarding license became less intense because most
individuals graduating from nursing schools sought and
received license to practice ., however they practice nursing
without have demonstrate skills and knowledge. They was
referred to as;’ ’graduate nurse’’ rather than RNs. Many
nurses began to call for mandatory license ,it require that all
person who wish to practice nursing meet established
standards for education ,pass standardized examination and
secure a license to practice in the state ,province ,or territory
in which they wish to work.
6-advanced nursing research,
and science.
-Nursing research is an integral part of the
scientific enterprise of improving the nation's
health.
-Nursing research and scholarship has received
significant funding by public and private
agencies in the last decade and is increasingly
recognized as an independent body of
knowledge.
- Schools of nursing are not sufficiently focused
on the scholarship and science of nursing as
top priorities, and, although graduate degrees in
nursing have become more common, doctoral
prepared nursing professionals are not being
produced in large enough numbers to meet the
growing need.
7-Changing demographics
and increasing diversity.
 Population shifts in the United States have affected
health care priorities as well as the practice of nursing.
Due to advances in public health and clinical care, the
average life span is increasing rapidly. By 2020, more
than 20 percent of the population will be 65 and older,
with those over 85 constituting the fastest growing age
group.
 Significant increases in the diversity of the population
affect the nature and the prevalence of illness and
disease, requiring changes in practice that reflect and
respect diverse values and beliefs.
 Schools of nursing must be prepared to confront the
challenges associated with today's more mature
student body, and educational methods and policies,
curriculum and case materials, clinical practice
settings, and research priorities need to value and
reflect the diversity of the student body, as well as the
population in general. At the same time, schools must
focus recruitment efforts on the more traditional,
younger student.
8-Ethics and bioethical
concerns.
-Today's patient is a well-informed consumer who
expects to participate in decisions affecting
personal and family health care. With advances in
information technology and quality measurement,
previously unavailable information is now public
information, and consumers are asked to play a
more active role in health care decision making
and management. The media and the Internet
have facilitated this trend.
-this trend affecting nursing education is related to
multicultural ,multiethnic population and patient
who have different ways to responding to
illness.,treatment and care provider.this raises
ethical issues of he is /right/and who has the
‘’right to decide.’
-.this is particularly relevant for freedom of choice
and end –of-life issue, As described , one difficult
issue for students and novices, is the ethical
necessity to differentiate personal beliefs, values,

-while natural disasters have been reported internationally
in relation to injury burden ,role of rescuers and
responders, there is little known about impact on
education in adult professional populations .
-The main goal of disaster nursing education is to prepare
nurses for challenges of unexpected events and enable
them to take more active roles in disasters. As each
disaster is unique and complex on its own, success in
overcoming it to a large extent depends on
preparedness of individuals for disasters.
-According to violence,,,there’s lateral violence .nurse-to-
nurse and vertical one is a term used to describe the
way a senior colleague behaves toward subordinates .in
clinical setting , many trainee nurses and new graduate
nurses may feel embarrassed , intimidated, and
humiliated by their teachers, doctors, or more senior
colleagues.
-This finding is particularly alarming for its potential impact
on future professional skills., the perception of isolation
, stress, and organizational injustice .
-Educating with violence may result in perpetuating
violence in workplace.

-Managed Care and Reimbursement Challenges Within both
the public and private sectors, managed care is quickly
becoming the dominant reimbursement mechanism for
health care providers, bringing both opportunities and
problems.
- Nursing professionals, who have historically taken the
lead in health education and health promotion, are
disappointed by the lack of financing and reimbursement
available through managed care organizations for these
vital services
- advanced practice nurses - nurse practitioners and case
managers in particular - have benefited greatly by the
managed care movement. Demand for these lower cost
providers has dramatically increased, and their
competence and quality in the primary care field have
been acknowledged.
- More research is needed to demonstrate the value to
the health care delivery system, as well as the cost
effectiveness, of health education, health promotion, and
advanced nursing practice. Nursing education programs
must prepare students at all levels for roles in case
management and employment in the managed care
environment.

  • 10. 3-community focused interdisciplinary approaches.-wide range of knowledge and skills is required to effectively manage all needs of patients and populations The health care delivery system in future will depend on teams of nurses ,nurse practitioners ,physicians,dentists,social workers and pharmacies and other providers to work together. -with care management ,acritical component in health care delivery, nurses must demonstrate leadership and competence in collaborative practice for continuous quality improvement. -Teaching methods that incorporate opportunities for interdisciplinary education and collaborative practice are required to prepare nurses for their unique professional role and to understand the role of other disciplines in care of patients. -This preparation need cost programs and shortage of nursing make that difficult. -most nurses in Egypt are technical not bachelor ,so how to ask them about quality of care due to their knowledge is low.
  • 11. 4- nursing shortage on health care.-In most of countries nursing is a popular profession for men and women ,but the number of facilities requiring nurses is growing at the faster pace than the number of trained nurses to staff them. -Every graduating class is absorbed into the health care system months before commencement exercises ,but nursing shortage still persist. -some of this shortage due to poor working conditions ,and inadequate information, and support system of nurses. -this result in nurses looking for migration, also the RN workforce is aging , and they want to work less than full time or retire early. -Brain drain has a negative impact on health care system ,where the recruited nurses may not be familiar with language, and culture of people they are supposed to care. -A possible intervention is for countries to collaborate with each other in expanding training of professional nurses and improve working conditions of nurses to alleviate the problems of nursing staff shortage and turnover. Nursing education must partner with the health care to develop innovative solutions that address the nursing shortage and must initiate an intense media campaign to enhance nursing image .
  • 12. 5- Licensure and regulations. -Issues surrounding health care are often complex, involving the individuals' rights as well as access to health care .Each state has its own set of administrative rules and regulation. --The early laws provided for permissive licensure ,permissive licensure , it is avoluntary system whereby individuals choose to become licensed to provide evidence of competence , but a license may is not required to practice . the requirements of this included graduating from school that satisfied certain standards established in the practice act for that state and passing comprehensive examination.only those who had met these standards could use the title RN. others used the title ‘’nurse’’ .Under this system , no one was require to have a license to practice nursing and there was much variability in standards from one state to another. -After permissive licensure laws came into being ,nurses activity regarding license became less intense because most individuals graduating from nursing schools sought and received license to practice ., however they practice nursing without have demonstrate skills and knowledge. They was referred to as;’ ’graduate nurse’’ rather than RNs. Many nurses began to call for mandatory license ,it require that all person who wish to practice nursing meet established standards for education ,pass standardized examination and secure a license to practice in the state ,province ,or territory in which they wish to work.
  • 13. 6-advanced nursing research, and science. -Nursing research is an integral part of the scientific enterprise of improving the nation's health. -Nursing research and scholarship has received significant funding by public and private agencies in the last decade and is increasingly recognized as an independent body of knowledge. - Schools of nursing are not sufficiently focused on the scholarship and science of nursing as top priorities, and, although graduate degrees in nursing have become more common, doctoral prepared nursing professionals are not being produced in large enough numbers to meet the growing need.
  • 14. 7-Changing demographics and increasing diversity.  Population shifts in the United States have affected health care priorities as well as the practice of nursing. Due to advances in public health and clinical care, the average life span is increasing rapidly. By 2020, more than 20 percent of the population will be 65 and older, with those over 85 constituting the fastest growing age group.  Significant increases in the diversity of the population affect the nature and the prevalence of illness and disease, requiring changes in practice that reflect and respect diverse values and beliefs.  Schools of nursing must be prepared to confront the challenges associated with today's more mature student body, and educational methods and policies, curriculum and case materials, clinical practice settings, and research priorities need to value and reflect the diversity of the student body, as well as the population in general. At the same time, schools must focus recruitment efforts on the more traditional, younger student.

With advances in information technology and quality measurement, previously unavailable information is now public information, and consumers are asked to play a more active role in health care decision making and management. The media and the Internet have facilitated this trend. -this trend affecting nursing education is related to multicultural ,multiethnic population and patient who have different ways to responding to illness.,treatment and care provider.this raises ethical issues of he is /right/and who has the ‘’right to decide.’ -.this is particularly relevant for freedom of choice and end –of-life issue, As described , one difficult issue for students and novices, is the ethical necessity to differentiate personal beliefs, values,
  • 16. 9-Disasters and violence. -while natural disasters have been reported internationally in relation to injury burden ,role of rescuers and responders, there is little known about impact on education in adult professional populations . -The main goal of disaster nursing education is to prepare nurses for challenges of unexpected events and enable them to take more active roles in disasters. As each disaster is unique and complex on its own, success in overcoming it to a large extent depends on preparedness of individuals for disasters. -According to violence,,,there’s lateral violence .nurse-to- nurse and vertical one is a term used to describe the way a senior colleague behaves toward subordinates .in clinical setting , many trainee nurses and new graduate nurses may feel embarrassed , intimidated, and humiliated by their teachers, doctors, or more senior colleagues. -This finding is particularly alarming for its potential impact on future professional skills., the perception of isolation , stress, and organizational injustice . -Educating with violence may result in perpetuating violence in workplace.
  • 17. challenges of managed care. -Managed Care and Reimbursement Challenges Within both the public and private sectors, managed care is quickly becoming the dominant reimbursement mechanism for health care providers, bringing both opportunities and problems. - Nursing professionals, who have historically taken the lead in health education and health promotion, are disappointed by the lack of financing and reimbursement available through managed care organizations for these vital services - advanced practice nurses - nurse practitioners and case managers in particular - have benefited greatly by the managed care movement. Demand for these lower cost providers has dramatically increased, and their competence and quality in the primary care field have been acknowledged. - More research is needed to demonstrate the value to the health care delivery system, as well as the cost effectiveness, of health education, health promotion, and advanced nursing practice. Nursing education programs must prepare students at all levels for roles in case management and employment in the managed care environment.
