Client Info and Background
Liberty Pediatrics:
Small pediatric clinic
Three primary care physicians
Population of ADHD patients (~100) with special administrative needs
Current (incomplete) information stored in Excel file

Database Needs:
Keep it SIMPLE!!!
View all patients, all info
Flag Rx date, annual Well Child check-up, medication check-up
Add/edit patient info (dates, prescriptions, physicians, etc.)
Do NOT need: login/security

Process

Met with Beth as a team

Created schema

Discussed options & which user input forms and features
were best for our client

Created database structure with MySQL

Populated database with sample data from LP

Worked on PHP/HTML forms simultaneously

Removed confidential data, inserted fake data for demo

Challenges

Inserting patient data from Excel
Linking patient to prescription to drug type (many to many)

Linking foreign keys together

PHP/MySQL to "flag" overdue medicine/annual check-ups.
